miRNAs as diagnostic markers after treatment with glucocorticoids in rheumatology

PharmDr. Petr Pávek, Ph.D. Title of diploma thesis: miRNAs as diagnostic markers after treatment with glucocorticoids in rheumatology MicroRNAs are important class of non-coding RNAs that play important role in modulation of expression of multiple genes at a post-transcriptional level. Their deregulation contributes to many immune disorders including rheumatoid arthritis, systemic lupus erythematosus and systemic sclerosis. This thesis represents the most recent knowledge about functions of microRNAs in pathogenesis of these disorders and results were obtained by review of scientific literature published on PubMed database. The most perspective microRNAs in rheumatoid arthritis seem to be miR-16, miR-21, miR- 146a, miR-150 a miR-223. In lupus miR-148, miR-126, miR-21, miR-155, miR-125a a miR- 146 will probably find their useage as biomarkers. Systemic sclerosis is less examined diseases and we know most about miR-29 in the disease. Since the research of microRNA as diagnostic biomarkers is only at the beginning, it is most likely, that with the time there will be more and more of new microRNAs helping us clarify pathogenesis of each disorder. Nursing care of child with juvenilie idiopathic arthritis

Juvenile idiopathic arthritis is one of the most common rheumatic diseases that affects children less than 16 years of age. The illness is considered irrecoverable because of its unclear aetiology. My work is focused on nursing care of a child with juvenile idiopathic arthritis due to the very low awarness of this disease. The main objective was to describe the nursing care of a child with a given disease and to increase awareness of juvenile idiopathic arthritis. Another objective was to create information material for the public about the illness. I choose a case study method to meet these goals. I described the nursing process of the patient with the disease in the practical part of the bachelor thesis, supplemented by Mary Henderson model. Information was drew mainly from medical records, interviews with parents, medical staff, and by providing nursing care to the chosen patient. The specifics of nursing care of a child with juvenile idiopathic arthritis were described on the basis of the information and knowledge gained during the hospitalization of the patient. PharmDr. Petr Pávek, Ph.D. Title of master thesis: Pharmacogenetics in rheumatology - role of miRNA Rheumatoid arthritis (RA) is a systemic inflammatory autoimmune disease that causes progressive joint damage and can result in to life-long depreciation of life. The influence on the onset and course of the disease is not only genetic, but due to the heterogeneous character of the disease, it is also strongly influenced by lifestyle. This disease, based on the malfunction of our immune system in RA, is still incurable. The treatment of RA uses conventional synthetic drugs as well as biological treatment. To diagnose the effect of anti-rheumatic therapy, monitoring and evaluating the response to treatment is necessary. Important indicators of RA activity, functional status, quality of life, and structural progression of the disease are important. In clinical practice, we use DAS 28 composite system according to recommendation of ČRS. Recent discoveries in the area of diagnostics raise the question of whether some miRNAs could be appropriate biomarkers of RA progression. Title of bachelor's thesis: Case of physiotherapy care for patient with rheumatoid arthritis diagnosed Objective: The objective of this thesis is to process theoretical information about rheumatic arthritis and the case study of a patient diagnosed with rheumatic arthritis after resection of the metatarsal heads by Hoffmann on both lower extremities. Method: This bachelor thesis was processed on the basis of my month's practice in the Rheumatology Institute in Prague in the period from January 2 to January 27, 2017, under the supervision of Mgr. Iva Zinková. This thesis has two parts - the general and the special one. The general part is focused on the basic characteristics of the disease, epidemiology, etiology, clinical picture, diagnostics, prognosis and treatment of the rheumatic arthritis. The special part is divided in the anamnesis of the patient, entrance kinesiology analysis, description of individual therapies with their effect, final kinesiology analysis and the conclusion of the whole thesis. Title: A case of physiotherapeutic care of a patient with rheumatoid arthritis Objectives: The major aim of this thesis is to highlight the role of a physiotherapist as a part of the interdisciplinary team in the complex treatment of rheumatoid arthritis and to demonstrate the effect of special physiotherapeutical methods in practice. The thesis includes a detailed case report of a patient with rheumatoid arthritis. Methods: This thesis was made within the four weeks of practical training at the Institute of Rheumatology in Prague in the period from 2. 1. 2017 to 27. 1. 2017 under the guidance of Mgr. Iva Zinková. The thesis is divided into two parts: general and special. The general part is focused on processing the issues of the disease rheumatoid arthritis, deals with the characteristics, origin, symptoms, course of disease, diagnosis, examination and treatment. Included in the special part is a case report of physiotherapy care of the patient, including case history, entry kinesiological analysis, target, design and course of treatment, control kinesiology analysis and evaluation of the effect of therapy. Title: Case of physiotherapy care of patient with diagnosis rheumatoid arthritis Objective: Objective of this thesis is to point out the diagnosis of rheumatoid arthritis. It focuses on the etiology, diagnosis, treatment and rehabilitation of the patient. The second part is dedicated to the processing of case histories of patients. The aim was to design an appropriate treatment and assess its results. Method: This bachelor thesis was based on monthly practice in the Institute of Rheumatology in Prague, where I was in the period from 4. 1. 2016 to 29. 1. 2016. The whole practice was carried out under the supervision of Eva Taušová. The thesis is devided into two parts: general and special. The general part is focused on processing issues of rheumatoid arthritis, dealing with its characteristics, origin, classification, pathogenesis, medical examination and treatment. The special part is then processed as a case study of a patient with a diagnosis of rheumatoid arthritis. It begins with the patient's history and the entry kinesiological analysis, which is followed by final evaluation. Svatava Neuwirthová Year: 2014 Aim of the thesis: The aim of the thesis is to summarize theoretical knowledge of rheumatoid arthritis in general part of the thesis and to elaborate a case report on a patient diagnosed with the disease, to emphasize physiotherapeutical care within the terms of a complex treatment of the patient and to assess the effects of the therapy in specialized part. Methodology: The first part of the thesis is general one. It deals with etiology and pathogenesis, clinical picture and symptoms, diagnostics and evaluation of patients, course of the disease and komplex treatment of rheumatoid arthritis, namely pharmacotherapy and application of physiotherapeutical procedures. Autor: Jana Martiňáková Title: Case study of physiotherapy treatment of a patient with systemic scleroderma Objective: The main aim of this thesis is a summary of theoretical informations about the diagnosis of systemic scleroderma, case reports of physiotherapy treatment of a patient with this diagnosis and evaluation of the effect of therapy. Methods: The work is divided into a general and a special part. The general part deals with the distribution of scleroderma, further analyzes the problem of systemic scleroderma with a focus on physiotherapy treatment and physical therapy. Special section includes a case study of physiotherapy treatment of a patient with this disease. The part of the special section are initial examination, setting goals and treatment plan, therapies, final examination and evaluation of the effect of therapy. This case study was created during continuous professional clinical practice held from 5. 1. till 30. 1. 2015 in Revmatologický ústav in Prague. Results: After performed therapies were achieved positive results especially during walking, improved respiratory stereotype, increase joint mobility acres upper limbs and improve grips.
