Title: Case study of physiotherapy treatment of a patient with the diagnosis of ankylosing spondylitis. Aim: The aim of this thesis is to introduce the issue of disease of ankylosing spondylitis, focusing on physiotherapy treatment in this disease. Furthermore the thesis is introducing a case study of a patient diagnosed with ankylosing spondylitis. Summary: This thesis is divided into two parts, general part and special part. General part introduces the issue of ankylosing spondylitis. The general part describes symptoms of the disease, its diagnostics and ways of treatment, focusing on physiotherapy and physical exercise in general, furthermore possibilities drug treatment and surgery is mentioned as well as other ways of treatment. Finally it describes the prognosis of the disease. Special part introduces a case study of a patient diagnosed with ankylosing spondylitis. Based on the initial biomechanical assessment the short-term and long-term rehabilitation plan is evaluated, the therapy carried out according to rehabilitation plan. By comparing both initial and final biomechanical assessment, the effect of therapy was concluded. The materials to create this thesis, was gathered during my work experience in Revmatologický ústav, Prague which I undertook from 15. January 9. to February 2018. Key...
Population differences in the distribution of HLA-B27 allele in ankylosing spondylitis
Lennerová, Tereza ; Hušáková, Markéta (advisor) ; Novota, Peter (referee)
Ankylosing spondylitis (AS) is a chronic inflammatory disease affecting mainly the spine and the sacroiliac joints. This disorder has a genetic background and is strongly associated with HLA-B27 gene which occurs in about 90 % of patients. The prevalence of ankylosing spondylitis usually correlates with the frequency of HLA-B27. The strength of association HLA-B27 with AS varies between different populations and is also distinct for individual alleles of HLA-B27 gene. Some alleles can increase a risk of ankylosing spondylitis while others may have a protective effect. This work deals with the population differences in the occurrence of HLA-B27 alleles and their relation to development of ankylosing spondylitis. Key words: ankylosing spondylitis, HLA-B27, allele, subtype, population differences
Comparison of educational and compensation exercise programmes for ankylosing spondylitis patients
Levitová, Andrea ; Strnad, Pavel (advisor) ; Bartůňková, Staša (referee) ; Bečvář, Radim (referee)
Title: Comparison of educational and compensation exercise programmes for ankylosing spondylitis patients Objective: The objective of the research was to determine the effect of two educational and compensation exercise programmes on the mobility of the axial system, the functional status and disease activity (including inflammatory process activity) in individuals with ankylosing spondylitis. Methods: The research group included men and women (average age of 35.42 ± 7.15 years), all out-patients of the Institute of Rheumatology in Prague. This characteristic sample (n ═ 38) consisted of respondents who were randomised into three groups: The first experimental group (n ═ 13) attended an educational and compensation exercise programme in a group setting in a gym (twice a week) and an educational and compensation exercise programme in the form of a group exercise in a pool - hydrokinesiotherapy (once a week); the second experimental group (n ═ 13) attended the same educational and compensation exercise programme in a group setting in a gym (twice a week); the control group educational and compensation exercise programme in a group setting in a gym (twice a week) received no exercise intervention but its members were allowed to use "passive" physiotherapeutic procedures (e.g. hydrotherapy or...
Biomarkers of subchondral bone damage caused by inflammation in axial spondyloarthritis.
Bubová, Kristýna ; Pavelka, Karel (advisor) ; Horák, Pavel (referee) ; Hrnčíř, Zbyněk (referee)
Background: Axial spondyloarthritis (axSpA) is a chronic inflammatory rheumatic disease affecting primarily the spine and its adjacent structures. The disease is characterized not only by destructive joint changes but also by excessive osteoproduction, which can lead to gradual ankylosis of the spine and thus significantly reduce the mobility and quality of life. The pathogenesis of the disease is not yet fully understood, but a strong genetic background is suggested, along with dysregulation of tissue metabolism resulting from an imbalance of pro- and anti-inflammatory immune mechanisms. We are still lacking biomarker with sufficient sensitivity and specificity which could help to identify early diagnosis, to monitor subchondral damage, and to differentiate rapidly progressing patients. The aim of this work was to determine the levels of potential biomarkers of connective tissue metabolism, fat metabolism and new promising biomarkers for both disease subtypes, their relationship to disease activity and progressive structural changes. Results: We have shown increased serum/plasma levels of connective tissue metabolism biomarkers (especially matrix metalloproteinase mediated metabolites), which were able to differentiate patients with early and late forms of axSpA from healthy individuals (HC), were...
Physiotherapy and life quality promotions patients with Bechterew disease
Strašáková, Anežka ; Zinková, Iva (advisor) ; Michalčinová, Klaudia (referee)
IN ENGLISH Name of title: Physiotherapy and life quality promotions patients with Bechterew disease Abstract: Subject of this bachelor's thesis is lifelong rheumatic disease - ankylosing spondylitis - from the perspective of patients and possible influence of physical therapy on disease's outcomes. Theoretical part deals with anatomical findings on axial skeleton, classification of ankylosing spondylitis within the group of rheumatic diseases, ethiopathogenesis, clinical manifestations, course of disease based on pathological and anatomical changes, examination, diagnostics, possibilities of medical treatment and therapy. Therapy is independant chapter, posing the first objective of this bachelor's thesis - description of different physical therapy approaches that can support the quality of life of patients with ankylosing spondylitis. Practical part deals with examination and description of subjective perception of quality of life of patients with axial form of ankylosing spondylitis during the course of disease. This bachelor's thesis can be classified as a descriptive type. Semi-structured non-standardized interview was used for data collection in practical part. Patients, who fulfilled criteria and signed informed consent, were interviewed. Recorded interviews were authentically transcripted....

Title: Casuistry of physiotherapeutic treatment of a patient with ankylosing spondylitis diagnosis and in the condition after total hip replacement Aim: The aim of this bachelor thesis is to become acquainted and deepen the knowledge regarding the issue of total hip arthroplasty and ankylosing spondylitis as well as repeating the acquired skills I've gained during the study of physiotherapy at the UK FTVS in the practical part of my thesis. Summary: This bachelor thesis is devided into general and practical part. The theoretical knowledge of hip joint and ankylosing spondylitis are summarised in the general part of the thesis. I was describing joint construction, osteoarthritis, coxarthrosis, conservative and surgical treatment of coxarthrosis and the follow up care after the total hip arthroplasty. In the theoretical issue of ankylosing spondylitis, I focused on the clinical picture, treatment, therapy and possible performances of this disease. The practical part is describing the case report of the pacient, contains initial kinesiological analysis, short- term and long-term therapeutic plan, the process of individual therapeutic meetings, the output kinesiological analysis and evaluation of therapy results. Keywords: physiotherapy, total hip arthroplasty, ankylosing spondylitis
