Bone remodeling in rheumatic diseases: Bone loss in juvenile idiopathic arthritis

Introduction: The inflammation plays the essential role in the bone loss in juvenile idiopathic arthritis (JIA). Proinflammatory cytokines and also glucocorticoids (GCs) may activate bone resorption by osteoclasts. Simultaneously, bone formation can be attenuated, especially by inhibitors of proteins, which control the osteoblast differentiation. The aim was to verify the hypothesis that in patients with highly active JIA, reduction of bone formation via Wingless (Wnt) proteins inhibitors - Dickkopf 1 (Dkk-1) and sclerostin could be found. Except the densitometry measurements of bone and lean mass, we assessed markers of disease activity, bone metabolism and remodeling in young adult patients with JIA before and during 2 years of anti TNFα (tumour necrosis factor α) treatment, which decreases disease activity. Results: In patients with JIA before antiTNFα treatment, bone mineral density (BMD, g/cmš) was significantly reduced compared to controls. Values of BMD and body composition in JIA significantly depended on disease duration and GCs treatment. Serum concentration of sclerostin was significantly elevated in JIA compared to values in healthy controls. Values of the other monitored markers did not differ between JIA and controls. Juvenile Rheumatoid Arthritis and Physical Activity

Bachelor thesis discusses the issue of juvenile idiopathic arthritis (JIA). It deals specifically with principles, selection and application of appropriate physical activities that are a significant part of the overall treatment of patients affected by JIA. The theoretical part focuses on basic terms and the summarization of findings about JIA; it further deals with causing factors, clinical manifestation, diagnosis and treatment options. The purpose and importance of physical therapy is described is the special part. For each stage of the disease appropriate set of exercises and principles of movements for individual human body joints are presented, with more options of physical activities suitable for a complex physical treatment. The practical part then presents a case study of an individual with juvenile idiopathic arthritis who has undergone a kinesiological examination which was a basis for creating a short-term and a long-term exercise plan.

Title of bachelor's thesis: Case study of physiotherapy treatment of patient with Juvenile Idiopathic Arthritis Summary: I focus on rheumatic diseases in childhoodin my thesis, especially the issue of juvenilie idiopathic arthritis. This work contains free parts; the first consists of the condition, the second is theoretical, focusing on a more detailed description of the deseases, including screening and subsenquest therapy and the third is a special part that contains a case study of physiotherapy treatment of patient with juvenile rheumatoid arthritis. I worked on this case study during my month experience in the Rhematology Institute in Prague. The special part consists of the input and output kinesiological analysis, the short- and long- term physiotherapy plan, therapy suggestions and description of ten exercise units. HLA complex and association with autoimmune diseases

Autoimmune diseases are serious and sometimes life-threatening disorders that affect approximately 5 % of the world population and therefore represent a major socio-medical problem. The HLA complex is the best studied part of the human genome that encodes the HLA antigens, whose function is to present peptides to immune system (T-lymphocytes). During the study of the HLA complex it has been found that certain HLA antigens are strongly associated with the occurence of autoimmune diseases, despite the great progress in clarifying disease etiopathogenesis the mechanism of these associations is, however, not fully explained. Sometimes the association of autoimmune disease with the HLA complex is very strong, such as in ankylosing spondylitis or celiac disease, while in other diseases the association is relatively weak, suggesting a role of other genetic and / or environmental factors. The aim of this work is to briefly explain the structure and function of the HLA complex and its association with some severe autoimmune diseases.
Case study of physiotherapy treatment of a patient with Juvenile Rheumatoid Arthritis

Title of bachelor's thesis Case Study of physiotherapy treatment of patient with Juvenile Rheumatoid Arthritis Summary The goal of my work is to characterize the issue of the juvenile idiopathic arthritis. In the first more theoretical part, I specifically describe the above mentioned disease and make a general distinction among the rheumatic diseases. The second part elaborates on a special case report of a patient being diagnosed with the juvenile idiopathic arthritis, which I had been working on during the four-week work experience at the Rheumatology Institute in Prague. This section includes the input and output kinesiology analysis, short- and long-term therapeutic plan, design/proposal and description of particular treatment therapies.
