Physiotherapy after facial palsy

The subject of this bachelor?s thesis is physiotherapy for peripheral paralysis of a facial nerve (nervus facialis), specifically Bell?s palsy. This is a paralysis of the mimetic muscles affecting an entire half of a face. It is typified by a rapid onset, usually on waking in the morning, and normally follows a predictable course. The most notable objective symptom is a marked facial asymmetry together with a complete change in the facial expression. For most patients prognosis is good, many recover fully without any lasting effects, but in some patients symptoms can persist. Patient education and healthy lifestyle, together with appropriate physiotherapy, are equally important in the treatment of this unpleasant disorder, which, if it persists, can have a negative psychological impact on the patient. The first part of this bachelor?s thesis deals with the theoretical background of this problem area. It contains a summary of findings from the fields of anatomy, physiology and pathology of the facial nerve. Specifically, this work focuses on the peripheral paralysis of the nervus facialis, known as Bells palsy, including its definition, subtypes, symptoms, diagnostics, clinical picture, examination and treatment options. The aim of the research part of this project was to identify patients suffering from peripheral paralysis of the nervus facialis, their examination and a subsequent therapy designed for each individual patient, reflecting their specific needs. Second aim was to evaluate the extent to which patients have been informed by health personnel about the role of a healthy lifestyle. The practical part of this bachelor?s thesis presents case studies in which qualitative research methods were used. The research techniques used included one-to-one in-depth interviews, taking patients? medical history, personal observation, applied therapy and secondary data analysis. The research took place in České Budějovice Hospital Physiotherapy department, and in the Polyclinic Jih in České Budějovice. The research sample consists of three patients suffering from peripheral paralysis of the nervus facialis. The therapy results are presented as three separate case studies. Two of the patients recovered well, having lost most signs of paralysis. In the third patients the paralysis persisted. Research results show that the elected therapy and its application, in patients afflicted by nervus facialis palsy were successful. Further more, the results showed that medical staff had adequately informed the patients about the role of an appropriate life style, although patients did not always fully follow it. This bachelor?s thesis could be used during physiotherapy in a clinical setting, and as educational material for patients suffering from nervus facialis paralysis.
Specific methods of physiotherapy treatment as part of a woman in the puerperium

This bachelor's thesis focuses on physiotherapeutic techniques as part of care for women during the postpartum period. Most women actively attend prenatal classes, where they are instructed on how to exercise and perform routine activities to provide relieve to their musculoskeletal system. Unfortunately, the majority of women are not informed of how to continue after having given birth, when they experience considerable weight loss and the burden on the skeleton changes completely within several hours. The objective of my thesis is to provide an overview of the possibilities and utilization of special physiotherapy techniques to accelerate the normalization and stabilization of women's musculoskeletal system. The first part of the thesis focuses on the physiological postpartum period, anatomy and postpartum complications. In addition, I provide an overview of techniques that can be used in clinical practice. The second objective was to discover whether individual exercise during the postpartum period makes it easier for women to cope with postpartum changes, thereby having a positive influence on the course of the postpartum period. This was confirmed by the results of this work. Qualitative research techniques were used, including interviewing and observation techniques. The study group comprised 3 patients aged from 25 to 34 years old. The women were followed up during the entire postpartum period, which was free of any complications. The patients improved their breathing stereotypes and pelvic floor functions. All three women reported gradual improvement of their subjective feelings during treatment. Unfortunately, I learned that many women in the postpartum ward refuse/do not do any exercise. The rooming-in system, although the most beneficial for both mother and child, makes the physiotherapist´s work very difficult. It is very hard to coordinate the mother´s free time and the physiotherapist's time reserved for the postpartum ward. In my opinion, the best solution would be to begin education mothers during pregnancy, preferably during the prenatal courses. The expected practical use of this thesis is for the needs of physiotherapists as well as other healthcare professionals and women in the postpartum period.
Physiotherapy at treatment of chronic pain

The title of my bachelor thesis is "Physiotherapy at Chronic Pain Treatment". In my thesis I have focused on chronic back pain. Ambulatory practice indicates the above by general diagnosis as vertebral algic syndrome (VAS). First part of my work consists of processing of theoretical basis related to the problem. Main sections focus on pain, function of the spine, vertebral algic syndrome, spine examination methods, physiotherapeutic procedures and prevention of VAS origination. The research part of my work includes two case reports with diagnosis of chronic vertebral algic syndrome. The objective of the theoretical part of the work was to focus from the physiotherapist's view on most common causes of chronic pain, namely vertebral etiology. This part is also aimed at analyzing basic causes of VAS, processing of current information for diagnostic, therapeutic and preventive procedures related to VAS. The objective of practical part of the work was to find out, based on the case reports, whether the physiotherapy relieves the patient from pain. In the practical part of the thesis I applied the method of qualitative research, case study. The research was carried out in Malvazinky Rehabilitation Center and in Medicentrum Chodov. The tested set consisted of two probands diganosed with chronic vertebral algic syndrome. The diagnoses were given by the doctor treating the probands. Results have been prepared in the form of case reports. Both monitored patients showed a relief from pain and rehabilitation objectives set in advance have been met. Providing that the patients shall practise self-therapy and pay attention to measures preventing origination of problems. The thesis may serve as a theoretical document for medical science students. For physiotherapists it may serve as an inspiration when preparing rehabilitation plans for chronic back pain treatment.
PHYSIOTHERAPY AS A PART OF FERTILITY DISORDERS TREATMENT

Infertility is defined as inability of a couple to conceive a child within one year of having unprotected sex at least two times a week. In 1970s the World Health Organization classified the condition as a disease. The data indicate that approximately 20-25% of couples in the Czech Republic are involuntarily childless. There are many methods used to address this condition. One of them is a physiotherapeutic approach. The purpose of this thesis was to find new (modern) views of the issue or potentially also some of its causes, and to point to the potential use of modern physiotherapeutic approaches to infertility treatment. To achieve the objective I have focused in the theoretical section on various causes of fertility disorders and their diagnostics. Further, I have tried to map some methods used in infertility treatment. I have strongly emphasized the physiotherapeutic approach, particularly methods developed by Ludmila Mojžíšová. In the practical section I have used a qualitative research method {--} the case study. The research was conducted in the Rehabilitation and Regeneration Center in Borovany near České Budějovice. Women selected for the survey were diagnosed with infertility and palpation sensitivity of pelvic bottom muscles. Three women treated with a rehabilitation method for some types of functional female infertility. The objective of the research was to map changes in the condition of muscular dysbalances in their pelvis bottom areas. In the surveyed patients improvements were found in terms of posture, breathing stereotype and pelvic bottom function. During the treatment all the three women reported gradual improvement of subjective feelings. One of the women got pregnant during the treatment. The main contribution of this thesis is the mapping of the infertility phenomenon. From the viewpoint of physiotherapy this work may be used in clinical practice by physiotherapists involved in this area.
Options of physiotherapy treatment in patients after stroke.

The topic of this thesis is Options of physiotherapy treatment in patients after stroke. The objective of the thesis is to survey physiotherapy-nursing care for patients after stroke and to describe current possibilities and quality of physiotherapy care for patients after stroke The thesis deals with possibilities of physiotherapy treatment and cooperation between nursing and physiotherapy staff. One of the targets is description of physiotherapy care for patients after stroke which is documented by patients{\crq} medical records. The other target is physiotherapy care monitoring supported by the quantitative research using the questionnaire method. The aim of this thesis was to develop an appropriate methodology to be used as an instrument for physiotherapy care for patients who had sustained acute stroke. Another objective was to determine the level of cooperation and awareness of staff members involved in the physiotherapy-nursing process. To meet the objectives the method of quantitative research was used. The methods of participated observation, semi-standardized interview and documents evaluation were included. Complementary data were obtained by the quantitative research using the questionnaire method. The research was conducted in patients after stroke who were hospitalized at the neurological department in Ceske Budejovice hospital. Additional quantitative research, using the questionnaire method, was conducted at the neurological and internal departments in hospitals in Ceske Budejovice, Cesky Krumlov and Pribram. On the basis of the research results, methodology of physiotherapy care for patients after stroke in acute condition was developed and the lack of awareness and cooperation of physiotherapy and nursing staff was found. The thesis will be used as a study and information material for all members of the medical staff involved in treatment of patients after stroke, but also for general public. Information can be forwarded through lectures, leaflets, etc.
Physiotherapy after the forearm and the hand injuries

My bachelor thesis dealing with the topic Physiotherapy after forearm and hand injuries, describes not only the complete anatomy of the forearm and hand, characteristics of the particular injuries and the complications thereof, but also examination methods and possibilities of rehabilitation, by means of which the patients frequently regain former functions of the extremity affected. The thesis aims to find out whether patients are able to return to their original occupations as well as their original hobbies. The research part, in which the casuistic method and that of a free conversation with patients in the course of their attendance to rehabilitation procedures were used, contains descriptions of diagnoses and examinations of two patients with a different therapy success. The conclusion of the thesis resulted in the fact that the convalescence success of patients and their return to work largely depends on how complicated their diagnoses are and on the character of their work. In my opinion patients should be provided with a complex care, which includes timely medical help and subsequent physiotherapy and ergotherapy. A very substantial problem that is omitted is the patients{\crq} mental state, and for that reason the convalescence process should involve possible visits to a psychologist. Another essential thing is to inform the patient systematically on his/her state and future, and his/her motivation for his/her effort of improvement.
The Training System of Pilates and Effects on Movement Body System

At present there are a fast growing number of physiotherapists and their clients who discover Pilates, a form of body and mind exercises that have a positive influence on the rehabilitation of injuries (Wilson, 2005). The principles of the Pilates method are identical with some physiotherapeutic concepts and methods (e.g. Mensendieck, Feldenkrais). The application of the Pilates exercises is spread especially among physiotherapists in the USA. In this country it is practiced a part of fitness exercises. This thesis deals with connecting the principles of the Pilates method with physiotherapeutic methods, their application in practice and confirming their positive effect on the change in body posture. The theoretical part deals with the analysis of the information as yet achieved on the Pilates exercises. It defines the Pilates exercises, sums up information from the history of the exercises, mentions the basic principles and their principal connection. The principles of breathing and describes the ``centre{\crqq} by the effort for explaining the subject matter from the professional point of view, and finally, it deals with the subject matter of the incorrect body posture. The practical part presents a set of exercises applicable in the prevention of the incorrect body posture, as a reason of vertebrogenic troubles. The quantitative strategy was used for the reasons of the empirical part. To confirm the effect of the Pilates exercises on the change in body posture, the development of the thoracic vertebral column in flexion and extension, I used Otto{\crq}s test. The results were statistically evaluated by the pair t-test. RESULTS: the positive influence of Pilates on body posture was confirmed.
Injuries of knee joint and consequential physiotherapy after crossed ligaments plastic

This work deals with questions of injuries of the fibrous apparatus of the knee joint, especially with the injury of ligamentum cruciatum anterius (LCA). The theoretical part summarizes the basic information relating to the problems of injuries of the fibrous apparatus of the knee. In the following, it offers an abstract of recent methods of physiotherapy used while dealing with patients with LCA plastic. The evaluation of the research, which compares the efficiency of the physiotherapy between two patients with LCA plastic (one active sportsman and the other a non sportsman), discovered that the therapy was more effective in the sportsman case, especially in term of faster growth of muscles.
Physioterapeutic impact on apparatus of movement during pregnancy and preparation to the labour

This bachelor work was aimed at the effect of individual physiotherapy on apparatus of movement during pregnancy and preparation to the labour. The first objective of my research was to find out whether the women who underwent individual physiotherapy in their pregnancy, in addition to the above stated courses, suffered minor problems connected with the locomotive apparatus before and during the parturition than the women who did not undergo individual therapy. I also addressed parturition, particularly duration of the first delivery period and pains during the delivery. Both objectives of my research have been attained. The anamneses of the women were obtained, kinesiological analyses were made and for one of the women a short-term plan was elaborated according to which the therapy was undertaken; subsequently the final analysis of the obtained results was performed. Two hypotheses have followed from my research. According to my first hypothesis, individually controlled physiotherapy undertaken during pregnancy can positively influence the locomotive apparatus of every pregnant woman. The second hypothesis claims that individual physiotherapeutic care has no significant effect on the course of the parturition.
The Progress of Muscular Dysbalances by University Students

This bachelor work was aimed at the progress of muscular dysbalances by university students. The aim of the thesis was to survey a rough structured profile of weekly physical activities of a university student and determine the most critical motoric activities leading to the muscle imbalance development. To achieve the objectives quantitative research was used. The case study technique was applied when the person observed took a record of his physical activities during a twenty-four hour period on a work day.
