The Historical Survey and the Current Trends of Total Hip Replacement

The bachelor thesis is focused on the historical development of total hip replacement, prothesis used in current surgical practice, materials review, and artificial acetabulum load analysis. FEM system ANSYS will be use to analyze three-dimensional model. The result of ANSYS simulation is strain-stress displayed to compile the acetabulum.

Title: Case study of physiotherapy care of a patient with rheumatoid arthritis and after hip replacement surgery Author: Vanda Páleníková Aim: Summary of theoretical knowledge, performing a kinesiological analysis, design treatment and monitoring of the patient during rheumatoid arthritis, and before and after right hip replacement surgery and subsequently summation effects by therapy. Methods and abstract: This thesis was created during professional practice, which was held at the Institute of Rheumatology in Prague from 14.1. 2013 to 8.2 2013. The thesis is divided into two parts - general and special. The first (general) part deals with the theoretical knowledge of rheumatic diseases, particularly rheumatoid arthritis and hip replacement surgery and methods of treatment, including physiotherapy practice. The second (special) case report describes a patient with rheumatoid arthritis before and after total hip replacement surgery, the design and the process of therapy and its evaluation. Result: The patient's condition was improved. After the operation has been modified posture and position in right hip joint. Svatava Neuwirthová Title: Case Study of Physiotherapy Treatment of Patient after total hip arthroplasty Objectives: The aim of this thesis is a case study of physiotherapeutic care of a patient after total hip arthroplasty, evaluation of the effect of therapy and theoretical treatment of this diagnosis. Methods: The bachelor thesis consists of a general part, which uses the literature to discuss the topic of total hip arthroplasty, and a special part, which contains a case report of a patient with this diagnosis. The special part includes a comprehensive initial kinesiological examination, description of the individual therapeutic units, an output kinesiological examination and an evaluation of the effect of therapy. Results: The final kinesiological examination confirmed that the therapies had a positive effect on the patient's health. The greatest improvement is seen in the increase in muscle strength in the right hip joint and the increase in range of motion. Another improvement is the reduction of swelling and the effect on the shortened muscles. These results prove that the therapies were effective. Title: Case report of a patient after total hip arthroplasty Objectives: The aim of this case study is to obtain and elaborate the current knowledge about the diagnosis of total hip arthroplasty. In the special part, the aim is to elaborate a case study of a patient after total hip arthroplasty with the proposal of therapy, its implementation and evaluation of the results. Methods: This case study is divided into two parts, theoretical and practical. The theoretical part contains the current knowledge related to the diagnosis of total hip arthroplasty, including anatomy, kinesiology, the operation itself, preoperative rehabilitation and postoperative rehabilitation. The practical part includes the analysis of a specific case of a patient after total hip arthroplasty and includes the initial kinesiological analysis, the course of therapy and the outcome kinesiological analysis with the evaluation of therapy. Results: After almost three weeks of rehabilitation in the Rehabilitation Hospital Beroun, the patient's subjective and objective condition improved, her pain was eliminated and her joint range and muscle strength were increased. Patients' Experience with Pain after Total Hip Replacement Surgery

Fear of postoperative pain is one of the most important concern of patients undergoing total hip arthroplasty and it is therefore important to address this issue in medical, and nursing care. In this research we focus on how patients experience pain after total hip arthroplasty. The diploma thesis is divided into two consecutive parts, namely the theoretical, and empirical part. The theoretical part describes the current state of total hip replacement, coxarthrosis, and postoperative pain management. From this part it is clear that the treatment of postoperative pain is dominated by pharmacological methods. In the empirical part, hypotheses were established which dealt with the measurement of the intensity of postoperative pain after total hip replacement surgery the use of pharmacological and non-pharmacological methods of pain treatment and the timeliness of postoperative pain relief. In the practical part of the work a quantitative survey was used. A query sheet was created which was then distributed to patients in the orthopedic patient department. The query sheets were filled out in the first, third, and fifth postoperative day and participation in it was voluntary. From a non-standardized query sheet the data were analyzed by statistics and the results are shown graphically in the thesis. The research survey shows that female and male respondents rate postoperative pain in the same way. The gradual reduction of postoperative pain is also the same for both sexes of the respondents. At the initial meeting with postoperative pain the medical staff instructed patients about the possibilities of pain treatment as the use of pharmacological and non-pharmacological pain treatment. In the query sheet the 87 (81 %) respondents choose the combination of both therapies on the first postoperative day. The 101 (94 %) respondents stated that they used 3 or more methods of non-pharmacological pain treatment on the fifth day. Research has shown that patients experienced less pain after surgery every day and the treatment of the postoperative pain was successful in terms of the rate of analgesic administration by a nurse according to the doctor's instruction. The use of non-pharmacological interventions in the treatment of pain also increased every day.

Title: Case study of physiotherapeutic treatment of a patient after implantation of total hip replacement Aims: The aim of this thesis is to provide a case study of physiotherapeutic treatment of a patient after implantation of a total hip replacement which had been preceded by hip dysplasia. Methods: Examination techniques and methods, which are taught at the bachelor's degree level at FTVS UK were used within this case study observed at the Malvazinky Rehabilitation Clinic in Prague. As part of the patient's treatment the medical history was established and an initinal kinesiological analysis was performed, based on which the aims of the following therapy were determined. The therapy consisted of sort tissue techniques, joint play mobilization, individual kinesiotherapy and locomotion training. Emphasis was also placed on autotherapy. At the end of the treatment, a final kinesiological analysis was performed after which the effect of the therapy was evaluated. Results: During the treatment the swelling of the lower left leg and of the surroundings of the surgical wound dissipated. The pain of the operated joint and the left thigh alleviated. Work placement: Fakultní Nemocnice Královské Vinohrady Summary: This bachelor thesis consists of two parts, one theoretical and one special part. The first theoretical part includes anatomy, kinesiology, biomechanics and pathology of the hip joint. The special part consists of a case study that is based on observations and clinical work at FNKV of a 59- year old woman after operation of total hip replacement. It includes initial and final kinesiological examination, day-to-day therapy and a conclusion of the whole process. The main goal is for the patient to became self-sufficient and be able to do activities of daily living without pain. The methods used in the therapy for this thesis were mainly manual techniques and exercises based on active movements to strengthen and increase range of motion in the operated hip joint. Title of bachelor's thesis: Case study of physiotherapeutic treatment of a patient with the diagnosis of total hip replacement Aims: The aims of the bachelor thesis are to improve theoretical knowledge as well as practical skills connected with the diagnosis of total hip replacement and physiotherapeutic methods used after the implantation of an artificial hip joint. Summary: The thesis is divided into two parts. The first, general part, deals with the anatomy and kinesiology of the hip joint; it also deals with the osteoarthritis, which is the most common cause leading to the total hip replacement. Particular types of implants and operating approaches are dealt with, as well. A significant part of the thesis deals with the rehabilitation plan for patients with this diagnosis, including preoperative rehabilitation, early rehabilitation after the operation and following rehabilitation. The second, specialisted part, deals with the casuistry of a patient after the implantation of an artificial hip joint, including evaluation of the effects of the therapy by comparing the kinesiological analysis before and after the treatment. Human locomotion after total hip replacement

Title: Human locomotion after total hip replacement Objectives: The aim of this thesis is to perform a gait analysis of patients after total hip replacement and to find out, how chosen temporo-spatial, kinetic and kinematic parameters change during the first half a year after the surgery. The deviations found in gait stereotypes of individual patients are further compared with results of international studies. Methods: The thesis is theoretical-empirical, with small number of patients. 3D kinematic gait analysis was performed by Qualisys system and ground reaction forces by two Kistler force plates. The following parameters are being monitored: length of step, stance phase duration, cadence, walking speed, lateral shift of the pelvis and trunk in standing phase, contralateral pelvic drop in standing phase of the leg, maximum range of motion of the hip joint in sagital plain, maximum of vertical and medio-lateral component of the ground reaction forces in the standing phase. Results: The highest side deviations between legs were found in 3 months after the surgery and remained until 6,5 months after the surgery. In the theoretical part the disease of osteoarthritis will be described as well as the examination and therapeutic approaches from physician and physiotherapist point of view. It will be also a detailed analysis of the procedure of total hip replacement. In the practical part it will be explained in details all the examination and therapeutic procedures I provided for the patient in Military Hospital of Prague (Ústřední vojenská nemocnice Praha) where my clinical work placement took place. The patient is a woman, 65 years old who had an operation of total right hip replacement on September 2010 because of osteoarthritis.
