Evaluation of Psychosocial Clima by Woman in a Small Maternity Hospital

This thesis focuses on mapping and evaluation of psychosocial climate by expectant mothers in the "Česko-německá horská nemocnice s.r.o.". It is a theoretical and empirical project. The theoretical part provides knowledge about psychological changes, which the woman goes through during her pregnancy, childbirth and the postpartum period. The further space is devoted to the needs of the expectant mother, the stress in the perinatal period and woman's psychopathology in the mentioned periods. Socio-cultural links in the perinatal period are also discussed. The vital importance lies in describing psychosocial aspects in the perinatal care as well as communication with the woman in the perinatal period. The research part of the thesis is aimed at the analysis of psychosocial climate in the maternity hospital in Vrchlabí. A questionnaire of psychosocial climate of the KLI-P maternity hospital was used for the research. 51 mothers participated in the evaluation plus extra 10 to complete the qualitative questionnaire. The analysis is enriched by two midwives' point of view of the approach to delivering mothers in the Vrchlabí maternity hospital. This thesis focuses on mapping and evaluation of psychosocial climate by expectant mothers in the "Česko-německá horská nemocnice s.r.o.". This work is based on the results of the research project "Psychosocial climate in maternity hospitals from the perspective of parturients - theory and diagnostics" (project no. 316111), realized in the Department of Psychology, Faculty of Arts, Charles University in Prague. The thesis is a theoretical and empirical work. The theoretical part provides knowledge about psychological changes, which the woman goes through during her pregnancy, childbirth and the postpartum period. The further space is devoted to the historical look at the development of inclusion of the psychosocial aspects into the health care and the current approach to the patients in general basis and to the women in labour especially. The theoretical part touches also the pathological mental states of women occurring in this time most frequently. However, the psychosocial aspects of the care for the woman in the perinatal period are the core of the theoretical part. Maternity hospital

A new concept of maternity hospital creates a place, where future mother is not stressed and where she feels free. A place, where birth is perceived as a common part of everyday life. It searches a compromise between birth at classical hospital environment on one side and birth at home on the other. Both aforementioned possibilities bring certain risks that are intended to be minimized using this concept. The idea is based on practice from birth facilities from Germany and creates new concept of so called „birth houses“ that is consistent with law in the Czech Republic. Design of maternity hospital should not be based solely on meeting technical requirements of a bulding, but first of all comfort of mothers should be taken into account to make them feel home.
Choice of a maternity hospital by expectant mother

The birth of a child is one of the most beautiful experiences that is remembered by the woman throughout her life. Therefore it is very important to find the right place in which she wants to give birth and to understand what she expects from such a place. The theoretical part summarizes information about maternity facilities, as well as about various alternative childbirth options. It also deals with factors that influence the choice of the maternity hospital. The practical part investigates how the mothers are informed about maternity facilities and about the factors that influence their choices of where to give birth. In order to examine the topic and to achieve the objectives a quantitative research method using anonymous questionnaires was used; the questionnaires were distributed to women who had just given birth and were hospitalized at maternity wards in the hospitals in Benešov (Nemocnice Rudolfa a Stefanie Benešov, a.s.) and in České Budějovice (Nemocnice České Budějovice, a.s.) Two objectives and related hypotheses were established. The first objective was to find out which factors influence the women when they choose maternity facilities. The other objective was to find out whether mothers preparing for the first birth decide differently than mothers who already have a child or more children. Both objectives have been met. The hypothesis 1 states that ?Clients choose such maternity facilities where they feel that they and their children are safe? and it has been confirmed. Also the hypothesis 2, stating that ?Clients prefer maternity facilities which maintain privacy of mothers giving birth?, has been confirmed. The hypothesis 3 to the effect that mothers preparing for the first childbirth (primiparas) choose maternity facilities differently than mothers who already have a child (multiparas) has not been confirmed. The theory and results of the survey may be beneficial for nurses and for development of information brochures for pregnant women attending pre-birth classes to help them to select the maternity facility, as well as for midwives in practice.
Comparison of the Work of Midwives in the Czech Republic and Austria

The bachelor is divided into two parts, namely the theoretical and the research part. The theoretical part is focused in particular on the nursing care provided by midwives in delivery rooms in the Czech Republic and Austria during individual delivery periods. It also describes what complications a midwife may encounter during a delivery and mentions alternative methods that may be used by a midwife in respect of woman in labour.
Midwife´s opinion onto labours at domicilary environment

Delivery at home is not a non {--} standart way to due, but that is also not legal. Someone who is helping with due at home on his own is absolutely responsible for all and he is support to answer all aftermacht if some complication will come. This is a decision of Czech gynekology-delivery copany and this decision is full law binding. Childbearing at home doesn´t the alternative for every woman. We thing that childbearing has plenty of merits, but we must know risks of childbearing at home for para and baby. The survey was quantitative by the way of questioners. My goal was to find what do midwifes thing about delivery at domicilary environment. This one says, midwifes have negativ position to delivery at home. They rate this way like dangerous. The inference is confirmed, all midwifes said the best place to delivery is Materna hospital. This work can be use for education and source informations for people, students and midwifes.
