Ensuring intimacy and dignity for patients in the Emergency Department, granted or extras?

Abstract This diploma thesis, titled "Ensuring intimacy and dignity in patients on urgent reception, self-evident or a luxury?", highlights the issue of respecting human dignity and ensuring intimacy in treating patients on urgent admission. The above issues are currently lagging the prestige of modern healing and nursing practices. In intensive care and therefore in urgent arrivals, an inpatient health care system is being developed, where maintaining the dignity of a person and ensuring his intimacy is very complicated. At a slow pace, this issue moves to the forefront of the public's interests, both professional and lay, particularly from the point of view of patients who have some experience with healthcare. The aim of this diploma thesis was to map the provision of intimacy and respect for dignity in patients on emergency reception by treating staff. The theoretical part of this thesis describes areas such as dignity, intimacy, holism, the needs of the sick, as well as communication and burnout syndrome and the functioning of urgent income as such in a brief description. The practical part of the work was elaborated by the method of qualitative research using the techniques of semi-structured interview with patients who were treated for urgent income and with emergency care staff, combined with the secret observation of the work of the emergency care staff. Interviews and observation took place in April 2017, at the Emergency in Hospital. Permit for research was issued by the Deputy Minister for Nursing. The research group consisted of 10 nurses working on urgent admissions and 10 patients treated in this department. The research shows that nurses have a very good knowledge of dignified care and patient intimacy, but in terms of putting this knowledge into practice, this area is under the supervision of the department's staff to work on it and to deal with it more closely. In patients pointed out to insufficient awareness of the course of treatment, applied medications, and a lack of privacy in nursing. Almost the most discussed issue is communicating personal data and health problems in the emergency waiting room to a nurse, without any possibility of privacy. Some factors that interfere with dignified and intimate care relate to areas of rather structural and organizational adjustments to the department and will be proposed for further solutions.
Quality of comunication in the emergency department for children

Bacelor thesis called Quality of communication of The Emergency Department for Children deals with the matters of communication between the nurses and parents of children during the child's acceptance in The Emergency Department for Children. This bachelor thesis is composed of a theoretical and empirical part. Theoretical part describes communication at general and it is also focused on a communication in public health. Further thesis analyses development of a communication abilities during the childhood and describes specificity of a communication with ill or injured children. This chapter inclused also a communication with the parents of a ill child. Last chapter of theoretical part describes The Emergency Department for Children of University Hospital in the Motol. Empirical part is aimed on the survey focused on a quality of communication between the nurses of The Emergency Department for Children and the parents of children who are transported to Emergency. The goal of this bachelor thesis is to describe the satisfaction of parents with a communication skills of nurses with them and their children and to analyse deficiencies of this type of communication. Readiness of the urgent reception to mass disasters This thesis is dedicated to the readiness of the urgent reception to mass disasters. It deals with the history, evolution and the current state of the urgent reception, both in the Czech Republic in the first place and in the world. Within the scope of this thesis I focus mainly on the work scope of the urgent receptions in dependence to their structure, equipment and their actual preparedness to deal with mass accidents. I focus besides other things on technical equipment of the urgent receptions, which is in frames of the Czech Republic very heterogeneous, but totally crucial to handle possible mass accident. A very import aspect is the matter of education and further training of the medical employees on all levels. It concerns not only medical positions, but also nurses and auxiliary staff. The question of the conditions severity of the admitted patients, their classification according to these conditions and transparent processing of the information about them cannot be passed over. Furthermore the thesis discusses emergency medicine as a discipline and the direction it should, in my opinion, further be heading in order to improve the functioning of the present system of pre-hospital and hospital emergency care. Urgent receptions in the Czech Republic represent a relatively new thing, which results into certain ambivalence of their concept and structure. Each urgent reception is somehow original and adapted to the requirements of the founder and the local situation. There is one thing the urgent receptions have in common, overload with patients who fall into the competence of practitioners. This is associated with the financial loss-making of these departments within the medical facilities. This, especially in the case of the private hospitals, leads to a reluctance of the founder to invest to the urgent reception. In this thesis I mention legal framework, contingency planning in the Czech Republic, contingency plans, their content purpose and type plan with its importance for solving crisis events. Another plan that is resolved in my thesis and that has crucial importance especially, for urgent receptions is the trauma plan of medical facility whose application into practice in the Czech Republic in many health care facilities is very problematic. And the practice of the trauma plan activation itself is, according to my research, the prerogative of only a very small amount of medical facilities.
Managerial and economic analysis of the Emergency department of the Jindřichův Hradec Hospital, public limited company

The focus of the diploma thesis is to evaluate the implementation of Jindřichův Hradec Hospital's Emergency department taking into consideration mainly fulfilling the aims targeted when decision about foundation of this department had been made. There are determined four goals that are crucial for the management of the healthcare institution. Based on that, the hypotheses are built. This thesis provides information which could be used by the Jindřichův Hradec Hospital's management in the terms of deciding about quality monitoring or patients satisfaction assessment.
Stressful Aspects of Work in the Emergency from the Perspective of Paramedics

Till the present-day the problems of the professional pressure have been explored above all in the field of the EMS nevertheless the inquiry in the field of urgent reception was and still is the field much less explored. In the thesis three aims were specified: To evaluate the most stressing situations of the urgent reception. To compare the stressful aspects of the paramedics at the places of urgent reception with EMS. To suggest the ways of managing stress at the urgent reception. Two inquiry questions were asked to fulfill the aims of the work: Are the paramedics working at the urgent reception able to describe stressing factors? Are the stressing situations same when working at the urgent reception and at the EMS? The inquiry was realized by the collection and analyses of the data while the collection of the data was realized by the qualitative form, by the method of semi-structured dialogues. The dialogues were led with 14 middle medical workers who worked at the position of paramedics. The choice of the workers was random, some of them worked at the urgent reception in Prague, in the Motol Teaching Hospital, in the Střešovice Central Military Hospital and in Královské Vinohrady Teaching Hospital. The questions were ranked into four areas. The first area dealt with the stressing factors and their comparison with the EMS. The respondents were asked why they find their work more difficult and in which it is easier then working at the EMS. Futher which situations they perceive as the most difficult for their psychical balance, if they face some fear in their service or if they are afraid of anything. In the second area we tried to go back with the respondents to the subsequences of the situations they experienced during their service. We traced the situations with which they feel to be influenced at the urgent reception and what kind of situations they were. The third area regarded the professional intervention. Here we found out if the respondents needed the help of the professional psychologist. Consequently if they have a psychologist at their disposal within the framework of their workplace. Lastly the respondents should express their opinion about the professional psychologist in the context of their profession. In the last forth area we dealt with the managing of stress. In this area we found out if the respondents try to cope with the stress load during their service or if they go back to such situations. Then the respondents were to answer if they have some kind of active relaxation and how often they practise this kind of relaxation. Then there was time to answer questions regarding the problems of smoking, alcohol and drugs. Thanks to the determination supported by the inquiry that the most demanding situations at the urgent reception regard death or serious health condition of a young man or a child, it was possible to state the proposion that the paramedics working at the urgent reception were able to describe the stressing factors they deal with. From the results it was also possible to state that the stressing situations at the urgent reception and EMS vary. Also it is necessary to remark that although the stressing situations and their perception vary in specific details, the perception of the demandingness of the work at the urgent reception can be subjective.The results also showed that some respondents found their work easier than working at the EMS. But the paramedics of these two subjects agreed in perceiving the most stressing situation the serious heath condition and death of young people and children. This thesis could be used in praxis to create a conception of a preventive and intervention program as an instrument of managing stress coming out of these results and the mentioned points. It would be only necessary to modify them in the area of urgent reception and in the situations which the respondents find demanding.
Difficulties in nurses work at the emergency department.

At performing high-quality nursing care for clients in the urgent reception ward, the nurse must have sufficient expert knowledge and experience. Nurses of the urgent reception ward meet clients whose health condition has abruptly changed. The health condition of such clients develops sometimes very dramatically and not only the nurses{\crq} expert knowledge and skills but also the communication and organizational capabilities matter. Communication in nursing process is perceived not only as a means to create interpersonal relations but it constitutes also an important element improving the quality of nursing care. That is why the treatment of the client and the client{\crq}s family members must be adapted so that efficient and effective cooperation is achieved. When providing nursing care to such clients, high psychical and physical demands are put on the nurse and they can even lead to the burnout syndrome. The goal of this Thesis was to process the issue and particularities of work of the nurses working in the urgent reception ward. Before starting the study, we set the following research question: ``What particularities does the work of general nurses in urgent reception wards have?{\crqq} The question was answered in the research part of the Bachelor Thesis, with the help of qualitative form of research investigation by the method of semi-standardized interview. The interview consists of 31 questions. The interviews were anonymous and they were conducted in the urgent reception ward in the Teaching Hospitals of Motol and Plzeň with eight nurses who work there. Case studies were processed on the base of the collected data and subsequently, categorization charts with the most important data obtained by the research were created on the base of the case studies. We confirmed, by our research investigation and by meeting the goal of our research, that the work of nurses in the urgent reception ward is highly demanding both professionally and psychically. The nurse of the urgent reception ward is required to assess and evaluate quickly the client{\crq}s condition and to respond to it adequately within a very short time. It was found out by the research that the nurses working in the urgent reception ward perceive particularly the overcoming of communication barriers in communication with the client and with the client's family and the reduced prestige of nursing profession as negative aspects with impact on their work. The basic particularities of the work of nurses in the urgent reception ward include quick decision-making, demanding character of urgent situations, psychical and physical burden and communication with the client and the client{\crq}s family.
