The Specifics of Individual Planning of Social Services with People with Dementia

The diploma thesis deals with the possibilities and limitations of the planning care and support to people with dementia. Diseases that lead to the dementia syndrome causing difficulties in almost all areas of life of the person who suffering from the syndrome. A person with dementia then requires specific approaches by others. The aim of this thesis is to identify the specifics of individual planning of social services with people with dementia and find out how is the individual planning with these persons done in selected homes with the special regime in the Capital City Prague and Central Bohemia.The first part provides an overview of current knowledge in the field of related topics. The second part contains the research which was carried in selected nine special regime homes in the Prague and Central Bohemia. In the research were used methods of semi- structured interviews and analysis of documents. In the research was found that the planning with people with dementia is in some aspects different from the planning with people without cognitive impairment. Adaptation of Senior to Placement in a Home for the Eldery

The topic of the bachelor thesis is an adaptation of seniors to location in a home for the elderly with the help of a key worker. The thesis deals with the many terms related to this issue. It mentions the concepts of aging, age, gerontology, adaptation, social services for the elderly and many others. It talks about reasons for entering into a home for the elderly. It explains who is a candidate, applicant and user of social services. It presents the possibilities of adaptation to the elderly home which the seniors have and describes the individual planning with key worker. The thesis also mentions the Home for the elderly Eliška Purkyňová, which has a seat in Prague 6. It describes its mission, target group, registered services and the whole procedure process from candidates to the users of services. Quality Improvement Process Support at Social Service

This diploma thesis deals with support of quality increasing process at social service provider in Domov pro seniory, Zahradní Město in Prague. The theoretical part intercepts a development of social services in context of social and legislative changes, theoretically deals with spheres which relate with change management process i.e. quality of social services, organizational culture and conflicts. With use of qualitative and quantitative methods was followed process of main aim filling of thesis i.e. quality increasing of provided social service. By focus group method was verified successfulness of planned process in praxis of social service provider. Final part of thesis contains my reflection of process from process participant view and from managerial practice view. I also give out my vision of further organization progress shortly. Considering to results of research evaluative phase it is possible to say that main aim of thesis i.e. quality increasing process of provided social service was fulfilled. It is possible to note that partial aims of diploma thesis were successfully fulfilled too i.e. describe action research process, define process pillars and barriers. Individual planning with people addicted to drugs in the contact center

The topic of this bachelorthesis is individual planning with people who are addicted to addictive substances. The main objective is to compare process of individual planning in different contact centers in the Vysočina region. The partial aim is to find out which theoretical starting points are being used in the contact centers for individual planning with addicted clients. This thesis could help widen public awareness of the contact centers and services they offer. It could also be interesting for social workers themselves, who might find some inspiration by the concept of individual planning in other contact centers. The theoretical part of the thesis is devoted, among other topics, to a description of a social worker, low-threshold facilities, contact centers and individual planning. This part also pays attention to addictive substances. The practical part of the thesis is elaborated through semi-structured interviews with six contact center workers, while these workers were selected by the method of deliberate selection. Questionnaires are structured in order to help answer the main research questions. This part also includes a comparison of individual planning with addicted clients amongworkers from different contact centers in the Vysočina Region. Based on the interviews that were processed, it was found that the number of clients per one worker differs significantly (sometimes up to twice). This is an important factor in terms of administration, which is described by almost all communication partners as an obstacle to individual planning. The research also showed that none of the communication partners uses a specific approach when working with clients. From my point of view, the surprising finding was mainly the evaluation of individual planning (mostly communication partners) as a burden while working with clients.
The adaptation process in the Home for the eldery in the street Rybniční Strakonice.

The dissertation is focused on the adaptation process in a certain Retirement home in Strakonice. The old age is the last evolution period of one´s life and the society should do its best to enable the people to spend this time in harmony, with sense of peace and joy. Nowadays, a pensioner is forced to think of the options provided by social services/social care in case that the health status doesn´t allow him to take care of himself sufficiently and independently or is not able to assure such care through his family. Then a long stay service in a retirement home seems to be an appropriate possibility. Joining such facility is usually considered as one of the most difficult situations that a retiree can meet with. It brings various risks as environment change with no knowledge of the area or people; it represents the loss or limitation of one´s autonomy, habits and privacy etc. A pensioner should properly consider this situation with his family, get ready for it and voluntarily decide to join an retirement home. The objective of my dissertation is to analyse the factors that influence the adaptation of retired people in the Retirement Home Rybniční, Strakonice. In the empirical part, there has been used the method of qualitative research and technics of demi structured dialogue for data collection. The research file has been chosen by the method of contingent defined selection. The own survey has been performed on a selected sample of retirees who complied with the given contingent conditions. The research showed that the arrival to the home for the elderly people is subject to poor health and inadequate housing situation, which represents a barrier to the environment for seniors.To answer the main research questions: What factors affect the adaptation process in the Retirement Home Rybniční, Strakonice? I can state, that the factors such as the home, the saturation of the needs, social contacts with family, friends and other users and active leisure more or less influence the seniors. The research results also pointed out the importance of self-sufficiency, which is affected by the health condition on one hand. On the other hand seniors are driven not only by the mission of Retirement Home Rybniční, but also by their own initiative to maintain the greatest possible degree of autonomy and self-sufficiency. The health status and the rate of self-sufficiency have a great impact on the process of adaptation and affect the individual factors of adaptation. This work should highlight the importance of the adaptation process of the seniors in the home for the elderly, which is not possible to underestimate. Furthermore, it should highlight the need for cooperation of the staff with the family, the importance of communication, environment, needs saturation and activities that can help to overcome this tough life step. Findings and conclusions of the thesis will be used as a feedback for the institution, where the research was carried out.
The Contribution of the Individual Planning for a Client of Home for the Elderly in Domažlice, Baldovská 583

This bachelor´s thesis deals with the contribution of the individual planning for a client who uses social services in a Home for the elderly in Domažlice. The theoretical part deals with the description of the Home for the elderly in Domažlice. It also describes what it means to work with a focus on the person, what is the support, assistance and care. The last chapter concerns the description of the process of the individual planning. In the research section the contribution of the individual planning is determined through the interview process for the client of a home for the elderly in Domažlice and validates my experience acquired knowledge. My findings are verified by my practice. In short we can say that the greatest contribution of individual planning for a client in a Home for the elderly in Domažlice is in the maintenance of human dignity and respect for privacy, satisfaction with the services provided, active involvement in the course of providing services, the development of self-sufficiency, a sense of individuality. In addition, individual planning for clients, safety, security, performance wishes, understanding, skills development.
Individual Planning in House for Elderly People in Blatna

Old years, in the case everything is good and people are given to be alive, are the last period of the life. Unfortunatelly, it often becomes complicated for physical and psychic difficulties. From this reason it is necessary to take a cautions approach to the life, especially in the case that the person has lost a partner who could provide him or her at least basic care and support. In view of the fact that an average time of the survival and the numer of old people and their families to look forward.They also should carry on preventivelly some things, for example to submit an application to gain a long-stay accomodation in a senior houses which can late help to solve unpleasant life?s situation. During interview with old people I found out that most of them had problems to fill in application forms to get a long-stay accomodation in a senior house. Some of them have difficulties to have an appointment with a doctor and to let him or her to fill in a form. Not due to they are handicapped but only because they are shy and are affraid of being placed in senior houses. A lot of people think that these ?houses? are only very last places before they are going to die very soon. In the first chapter I dealt with the present state of old peoples? problems which is above all typify with biological and psychological needs and their satisfying. Ageism /age discrimination/ is a very unpleasant phenomenon. A lot of old people have to be confrontated with it every day. It means for example during doing shopping, in the offices and so on. Very interesting for me was the part about how Czech people understood the old age.Their opinions are worth to have a think about them. In the second chapter I tried to highlight the characteristic of the care od old people which firstly start when we speak to them. The other very important part is the part about forms of the system of the care of old people. The next part offer the help to the families or the old people to choose the available place for a long-stay social accomodation. The third chapter deals with the listing and strict specification each, for old people affordable service around the town of Blatná and it?s surroudings which should help to choose the most available place. The fourth chapter deals with the history and the present days of the Senior House near the town of Blatná. It?s history started in 1952 when the first people were placed in the old castle in Lažany, the village within 10 kilometres from the town of Blatná. The fifth chapter deals with individual planning which is the main theme of my bachelor thesis and forms the core of research questions, concerning of the clients? and workers? opinions. The replies to researching questions I gained from the interview with some clients and some workers and supervisors. The information which I had got I sorted out logically and I even made some charts to get clear arrangement. This thesis, focused on regional features, could be useful for senior houses, head workers, supervisors and all workers in social services, I intended the thesis as an impulse to improve working environement in the team and also to improve the support and help for workers through the agency of supervising.
The Role of Key Worker in the Home for the Elderly

The problem of old age and aging is a present topic. The bachelor thesis is divided into a theoretical and an empirical part. The theoretical part is focused on the old age and aging, individual needs and their particularities, on the related legislation, care of the elderly, the Chvalkov retirement home, individual planning and the role of a key worker. The empirical part of the thesis deals with the research methods, the collection of data techniques, the research schedule, the characteristics and the method of choosing a research file, the methods of data processing and the results of the research. The first chapter of the theoretical part describes old age and aging and it specifies individual needs of old age. The second chapter then deals with the care of senior citizens in general, its division, its rules and regulations. The most important laws are presented with emphasis mainly on law no. 108/2006 on social services and on the related public notice no. 205/2006. The third chapter is focused on the Chvalkov retirement home, where the research took place. The next chapters deal with the key workers and individual planning. The empirical part of the thesis deals with searching for answers to the research questions. The aim of the thesis is to find out how individual planning and a key worker can influence the quality of social services from the point of view of the elderly in the Chvalkov retirement home. The main research question is: ?What importance do the elderly in the retirement home attach to the role of a key worker?? And three intermediate research questions are: ?What is the attitude of the elderly in the retirement home towards individual planning? What is the attitude of the elderly in the retirement home towards the role of a key worker? And what is the attitude of the elderly in the retirement home towards the importance of a key worker in connection with individual planning?? The empirical part of the thesis is based on qualitative research methods and on the questioning method (semi-structured interviews with open questions). The research itself was done in two stages. First a preliminary research was done, and then the main research followed. The senior citizens in the Chvalkov retirement home represented the researched group. The respondents were chosen by means of a goal-directed selection. The clients needed to meet some requirements ? to be residents of the retirement home for at least one year and to be willing to cooperate. Four respondents took part in the research. The method of creating clusters was used to analyse the obtained data using the qualitative research methods. It was found out through the research that thanks to individual planning the senior citizens in the Chvalkov retirement home are not only passive addressees of social services, but they can also decide on the quality of their life through the agency of a key worker. Individual planning encourages them to take an active role and responsibility of their own life, the key worker being a part of it. Individual planning and the role of a key worker are evaluated positively by the senior citizens and they find it of high importance for the quality of the social services.
The Importance of in-home nursing care for seniors and their families

Domiciliary care as a field or ambulatory service is provided to people with reduced self-sufficiency due to age, chronic illness or disability and is also used by families with children, whose situation requires assistance of another person. The aim of the service is to individually support a person's life in their natural environment. The theoretical part is divided into five chapters. The first chapter deals with old age and aging, as it is the time when changes affecting the overall function of human body occur. These are then reflected in increased vulnerability, limited performance and decline of an individualas abilities. The second chapter deals with changes that come with age, which mainly relate to the physical, psychological and social area. In the third chapter I describe the current situation in the provision of social services. These services are defined by the Act no.108/2006 Coll. primarily as a means to ensure the greatest mental and physical self-sufficiency of the individual obtainable. The services seek to enable people to participate in daily life, live in dignified environment and be treated with respect. The fourth chapter deals with the concept of providing domiciliary care services in the Czech Republic. The last, fifth, chapter deals with individual planning, which is one of the most important tools to ensure and improve the quality of social services provided and is also enshrined in the Act no.108/2006Coll. describing social services. The practical part of my thesis was elaborated by the qualitative research technique, specifically the questioning method using a semi-standardized interview with predetermined open-ended questions and by analysis of personal documentation. Basic research sample consisted of selected users and staff of the care services provider Ledax (charitable trust). The aim of this research was to determine factors that are perceived by health care users as major. The investigation showed that these are mainly health deterioration and old age. My research has shown that users perceive their key worker rather as a confidant. They are not afraid to confide their feelings or problems to them and report that the worker always has enough time to hear them. On the contrary, the workers themselves see their role only as a helper's and also due to the large workload lack sufficient time for the user. From my own experience I can confirm that this problem is often encountered because caregivers are responsible for more users, and the care is not concentrated in one place. The solution for this situation would be to reduce the number of users assigned to one caregiver, but the provider would be forced to expand their staff. However providers are currently limited by operational and financial limits of the given organization. It is important to emphasize that if the care and support provided by the user be at a professional level, the key worker should coordinate the care for the user and be their confidant and guide in a difficult season of life. Although there are some differences in the view of the factor of caregiver's personality, I think that the fact that users perceive their caregiver as a confidant is also a guarantee of efficient implementation of individual planning in the organization of the provider.
Activation activities in homes for the elderly in the Tábor region

Bachelor?s Thesis is focused on activation in homes for the elderly in the Tábor region. Theoretical solutions are introducing concepts of old age and aging, changes of old persons life, specification of communication with old person and last not least, meaning of activation in one?s old age. It?s describing also individual planning in residence social installations, role of a key worker and individual planning importance. Finally, possible forms of old persons activation are introduced. The practical part describes planned and implemented activities in selected homes for elderly, which are determined to their occupants. Next, it?s also focused on expert authorities of workers, who are participating on the leisure time coordination and realization.
