Psoriasis and Comorbities, their Prevalence and Impact on the Treatment

Patients with severe psoriasis suffer frequently from associated comorbidities and therefore might have reduced life expectancy, which explains the necessity of corresponding screening and treatment recommendations in cooperation with general practitioners and other specialists. The aim of this study was to evaluate patients with moderate to severe psoriasis regarding associated comorbidities and compare the obtained information with nationwide data. In a subgroup of patients with concomitant cancer we tried to prove the safety of biologic treatment. Our study involved 3 groups of patients. First group included 1961 patients with psoriasis treated at our Department within last 5 years. Second group consisted of 268 patients with psoriasis, who voluntarily filled out our questionnaire during a 2-year observation period. The third group contained all 141 patients treated to date in our Centre of biologic treatment. Based on our real-world clinical data, we proved and confirmed the known and lately discussed cardiometabolic comorbidities associated with psoriasis. The questionnaire survey demonstrated overweight and/or obesity in 74% of all male respondents and in 60% of females, with average BMI 28,7. Sexually transmitted infections (STIs) are a major public health problem. Gonorrhoea is the second most common STI and if untreated can lead to serious complications. Early detection and treatment is commonly recommended as an important means of HIV prevention. The Czech Republic (CR) has had a long-established STI surveillance program since 1959. Although the Institute of Health Information and Statistics provides important data for STI surveillance, it contains no further characterisation of patients, particularly with regard to risky sexual behaviour. At the same time there are very few such data from the CR in the literature. This work presents view of STIs, especially of gonorrhoea, syphilis, chlamydia and HIV infection; examines their correlations and influences of their transmission in different groups of venereological patients, informs about incidence and epidemiological factors. The objective of this study was to evaluate clinical, epidemiological and behavioural characteristics of patients with diagnosed gonorrhoea with respect to risky sexual behaviour, and to compare these parameters by gender and sexual orientation. Monitoring of long-term efficacy and safety of the biological therapy in psoriasis

Psoriasis is a chronic inflammatory skin disease affecting about 2-4% Central-European population. Treatment options include topical or systemic, often in combination. Among conventional systemic therapies we include phototherapy (UVB, PUVA), methotrexate, cyclosporine and acitretin. The newest drugs are so-called biologics: drugs blocking tumor necrosis factor alpha (TNF alpha) - adalimumab, etanercept, infliximab, drugs blocking interleukin-12 and 23 (IL-12/23) - ustekinumab and newly drugs blocking interleukin 17 - secukinumab, ixekizumab, brodalumab and recently as well IL-23 blockers - guselkumab, risankizumab.. The aim of this paper is to describe the long-term efficacy and safety of treatment of patients with moderate to severe psoriasis with adalimumab, a fully human monoclonal antibody against TNF-alpha. Retrospectively we analyzed all patients with moderate to severe psoriasis who between 2008 and 2016 were treated with adalimumab in the center of biological therapy in Dermatovenereology clinic of Faculty hospital of Kralovske Vinohrady. Although the average PASI patients assigned to treatment was 22.5, the results shows a rapid onset of adalimumab, where after 3 months of treatment PASI75 was observed in 85.6% of patients.
