Roztočilová, Dominika ; Fulín, Petr (advisor) ; Plecháčová, Marie (referee)
BACHELOR THESIS ABSTRACT Author: Dominika Roztočilová Supervisor: doc. MUDr. Petr Fulín, Ph.D., MBA Consultant: Ing. Milan Šebek Title: Results evaluation of reverse shoulder arthroplasty during physiotherapeutic intervention Abstract: This bachelor thesis deals with the topic of reverse shoulder arthroplasties and functional outcomes of the shoulder joint after the implantation. The aim of this thesis is to apply described physiotherapeutic approach to a selected group of patients and to evaluate the outcomes of shoulder joint function at given intervals during the postoperative physiotherapeutic intervention. The aim of the thesis is also to analyse current studies about this topic, compare the results and compare the effectiveness of different physiotherapeutic approaches. The theoretical part summarizes knowledge about the anatomy, kinesiology of the shoulder girdle and biomechanics of the shoulder joint. It also discusses shoulder replacements, especially the reverse implants. Furthremore, principles of physiotherapeutic postoperative intervention are described and physiotherapeutic approaches are outlined. The practical part of the thesis is procesed in the form of a research. It includes 8 patients who underwent implantation of a reverse arthroplasty and subsequent physiotherapeutic intervention. For...
The importance of high-intensity laser therapy in the treatment of the shoulder impingement syndrome
Pokorná, Pavlína ; Fulín, Petr (advisor) ; Plevová, Kristýna (referee)
BACHELOR THESIS ABSTRACT Author: Pavlína Pokorná Supervisor: doc. MUDr. Petr Fulín, Ph.D. Consultant: Mgr. Lucie Benešová Title: The importance of high-intensity laser therapy in the treatment of the shoulder impingement syndrome Abstract: This bachelor thesis examines the importance of high-intensity laser therapy (HILT) in the treatment of the shoulder impingement syndrome. The objective of this study is to verify the effectiveness of the HILT on clinical results and on alleviating difficulties of the patients with shoulder impingement syndrome. The thesis is theoretical-practical. The theoretical part deals with the anatomy and kinesiology of the shoulder girdle, diagnostics and treatment methods. The final component of the theoretical part is the laser therapy. Based on the inclusion criteria, a set of 7 patients was selected for the practical part. The HILT was performed on a total of 10 of their shoulder joints. The total amount of therapies was 6, administered 2 times per week for 3 weeks. The effectiveness of the treatment was evaluated with the clinical examination, which consisted of shoulder range of motion (ROM) measurement, clinical tests, Constant shoulder score, The Disabilities of the Arm, Shoulder and Hand Score (QuickDASH) and pain evaluation. These examinations were held at the baseline,...
Scapular fractures associated with acromioclavicular and sternoclavicular luxation
Strnad, Tomáš ; Tuček, Michal (advisor) ; Fulín, Petr (referee) ; Pilný, Jaroslav (referee)
Fractures of the scapula used to be among rare injuries, but in the last two decades their number has increased. Fractures of the scapula can be isolated, but in 67-92% they are combined with injuries to other structures of the shoulder girdle. The dissertation was divided into two parts: experimental and clinical. In the experimental part, the aim was to describe the occurrence and variants of sulcus coracoglenoidalis (CGN) and sulcus arteriae circumflexae scapulae (SCF). These two little-known structures play an important role in the occurrence of some types of scapula fractures and in their surgical treatment. Combined scapular injuries with AC and SC dislocation are rare. Therefore, in the clinical part, we decided to identify and describe in detail combined injuries of the scapula with AC and SC dislocation, including a detailed description of fractures of coracoid process. Anatomical studies were performed on 330 dry bone specimens of adult scapulas. Clinical studies were conducted on a set of 519 scapula fractures in adult patients occurring in the period 2002-2020. In the anatomical part of the dissertation, two anatomical structures were described for the first time: CGN and SCF. Our studies pointed to considerable variability in the occurrence and degree of formation of both of these...
The use and the effect of High Intensity Laser in treatment of rehabilitation patients
Benešová, Lucie ; Plevová, Kristýna (advisor) ; Fulín, Petr (referee)
This thesis deals with the effect of High Intensity Laser on soft tissues and postsurgical scars in the ankle joint area. The thesis is divided into two parts, theoretical and practical. The theoretical part presents a summary about ankle joint traumatology and its surgical management. Further there is also information about wound healing, scar formation, their examination, assessment scales and treatments. Last but not least there is a description about the laser mechanism, effect and treatment use. The main focus point of the practical part is to discover the effect of the High Intensity Laser used on soft tissues and postsurgical scars in the ankle joint area. A sub-objective is to evaluate the high power laser effect on pain reduction in the treated area. Methods: There were 20 probands tested in the practical part (average age 42,3, SD ± 13). The intervation group (n = 10) has applied High Intensity Laser Therapy (HILT) on postsurgical scars. The control group (n = 10) haven't HILT. The Observer Scar Assessment Scale (POSAS) was used for scar evaluation and pressure algometer was used for pain evaluation. Results: There was statistically proven that High Intensity Laser Therapy has an effect on pain reduction on level of significance α = 0,05, (p = 0,0047). According to the POSAS, results...
The influence of the quality of articulation UHMWPE inserts on the lifetime of joint replacements
Fulín, Petr ; Pokorný, David (advisor) ; Koudela, Karel (referee) ; Gallo, Jiří (referee)
This thesis focuses on the introduction with the problems of high molecular weight polyethylene (UHMWPE) in orthopedics in the first part. It discusses the history, properties and processes that lead to the failure of UHMWPE joint replacement components. The experimental part validates the hypotheses expressed. It acquaints readers with the experimental verification of the effect of different types of sterilization on the oxidative stability of laboratory prepared samples and explanted UHMWPE joint replacement components using methods of infrared microscopy, electron spin resonance, mechanical tests and tests of microhardness. Other experimental measurements on a large set of explanted components of hip and knee total joint replacements validates the hypothesis that the amount of oxidative damage affects the lifespan of joint replacements. The third part of the experimental study maps the degree of oxidative damage in different places of the hip and knee joint replacements. In the final part we experimentally demonstrate the fact that the rate of oxidative damage has an effect on supramolecular structure of the UHMWPE. From the above experiments are concluded clearly defined recommendations for clinical practice, which should lead to extend the lifespan of total joints replacements in orthopaedics....

Survey of patients treated for fracture of the ankle in the year 2005
Fulín, Petr ; Svatoš, Filip (advisor)
The topic of his thesis, A survey of patients treated for fractures in the ankle in 2005 I chose on the basis of their interest in the subject orthopedics and traumatology. The issue of fracture-dislocations of the ankle is quite complicated and frequent occurrence. Orthopedic - Traumatology Clinic of the Faculty Hospital in Vinohrady in Prague in 2005, treated 261 patients with fractures of the ankle. This means that on each working day is more than one fracture hocks (2005 had 253 working days). Each case was among others recorded in the questionnaire, with whom I then worked. We were interested in what circumstances, at what age and what type of fracture occurs and how to treat. Results for the year 2005, I had the opportunity to compare the results with certain categories from previous years.
