Author: Adriana Ondrášková Supervisor: PhDr. Ivana Vláčilová, Ph.D. Title: A case report of physiotherapy care of a patient diagnosed with congestive heart failure and implantation of HeartMate 3 LVAD Objectives: The aim of this thesis is to theoretically process the information related to the diagnosis of heart failure and implantation of HeartMate III and to record the course of physiotherapeutic care of a patient with this diagnosis. Methods: The case report of physiotherapeutic care of the selected patient was prepared during the concurrent professional practice at the Institute of Clinical and Experimental Medicine in Prague. The theoretical part is devoted to information and knowledge about the diagnosis of heart failure and implantation of HeartMate III. To write the special section, a anamnesis was first taken and an initial kinesiology examination was performed. Based on the examination, a short-term and long-term therapeutic plan was written. A total of 10 therapeutic units were performed with the patient. After comparing the initial and final examinations, the effect of therapy was evaluated. Lenka Chvojková Title of bachelor thesis: Physiotherapy in patients with long-term heart assist devices Abstract: This thesis deals with the issue of physiotherapy in patients after the implantation of long-term heart assist devices (in this case of the type HeartMate 3). The goal of this thesis is to find the possibilities of physiotherapy within cardiovascular rehabilitation after surgical implantation of heart assist device and subsequent application of this knowledge in practice. The thesis is divided into theoretical and practical parts. The theoretical part consists of basic information about the heart muscle and heart failure as the most common cause of implantation of the long-term heart assist device. Next, both heart assist devices in general and concretely HeartMate 3 are presented. Finally, there is a chapter devoted to cardiovascular rehabilitation. The practical part was created based on professional practice in the IKEM hospital in Prague at the turn of July and August 2020. There is an elaboration of case studies of two probands using functional testing such as the 2 Minute Walk Test and Barthel Index. European research policy and its impact on financial management of universities

The thesis focuses on theoretical and empirical analysis of the EU research policy and its impacts. The aim of the thesis is to evaluate effects of this policy on the financial management of universities, or more precisely the impact of financial rules defined by the European Commission (EC) in the 7th Framework Programme (FP7) on the development of full cost methodologies at universities in the EU27. The thesis also analyzes the process of the preparation of the new FP8 HORIZON 2020 financial rules and asses if feedback and needs of participating universities are reflected and thus principles of European good governance are applied. In order to meet its goals, the thesis states three hypotheses: Hypothesis 1: Universities from the EU27 that participate in FP7 develop full cost methodologies. Hypothesis 2: EC requirements in FP7 are one of the main drivers to implement full cost methodologies for universities form EU27. Hypothesis 3: Universities from the EU27 communicate development of their full cost methodologies with the EC. The thesis is organized into four chapters. The first one characterizes the concept of European good governance and one of its instruments, i.e. policy evaluation and its use in the thesis (including the analysis of performed questionnaire research). Second chapter defines the term of financial management and its use in the thesis. Third chapter describes the EU research policy, its historical development, main instruments (FPs), role of European institutions and attitude to full costing. The last forth chapter analyzes in details experience of universities form EU27 with development of full cost methodologies. Within its conclusions, the thesis proves that the EU research policy significantly influences the financial management of universities from EU27 and implementation of full cost methodologies by them. It is inter alia proved, that 70 % of universities from the EU27 that do not implement full costing in FP7 yet plan to introduce such methodologies in the future; that FP7 financial rules are drivers to implement full cost methodologies for almost 2/3 universities from the EU27 participating in FP7 and that 3/4 of universities form EU27 applying full costing in FP7 consulted on their methodologies with the EC. All three hypotheses thus were verified. Nevertheless, it was concluded that principles of European good governance are not satisfactory applied by the EC, because it does not support the use of full costing in HORIZON 2020 anymore.
Vital capacity of lungs after operation hearts IKEM Prague

Cardiovascular diseases in the Czech Republic represent the main cause of death and significantly contribute to the sickness rate and premature disability. Possibilities of treatment of cardiovascular diseases keep developing increasingly these days. An important part is the follow up spa treatment as well as sufficiently performed effective cardio rehabilitation. The theoretical part characterizes functional examination of the lungs and stress tests in cardiology. Simultaneously it describes early spa treatment and defines quality of life. The aim of the diploma thesis, which deals with the vital capacity of the lungs, was to prove positive effect of early spa treatment on the vital capacity of the lungs and on improvement of quality of life, specifically with respect to positive perception of one´s overall physical health. Hypotheses - H1: An early spa therapy positively affects spirometry values. H2: Patients with an early spa therapy better perceive their overall physical health. A form of quantitative research was chosen, in order to verify the determined aims and hypotheses of the diploma thesis. A standardized international Short Form SF - 36 questionnaires on quality of life was used for the data collection. Spirometry was utilized for ascertaining objective functional parameters. Values of spirometry examination were used for comparison. 32 patients after cardiovascular surgery were included in the research, who were transferred to Lázně Poděbrady on the 6th - 8th day after the surgery, 22 of them being men (69%) and 10 women (31 %). The average age of patients under research was 66,06 + 11,48 years. The check group consisted of 10 healthy volunteers. 8 women (80 %) and 2 men (20 %) at the average age of 37,1 + 13,3 years were included in the research. It follows from the spirometry results measured before the heart surgery at IKEM cardio center that a difference in results between the second and third spirometry occurred. The second and third spirometrical examination is the period of time, when the respondent undergoes the early spa treatment. After the heart surgery spirometry values worsen and improve in the course of the spa therapy. The research showed that the perception of overall physical health after the heart surgery is subject to perception of pain, which negatively affected sense of overall perception of physical health. H1 was confirmed and H2 was not confirmed. In order to improve current situation, it would be suitable to devote not only to cardio rehabilitation but also to improvement of perception of pain, e.g. by means of psychotherapy (art therapy, music therapy?). It is also important to widen a possibility of outpatient cardio rehabilitation in each cardio center for patients after heart surgery.
Possibilities pregnancy after transplantation of the heart

The thesis deals with the possibilities of pregnancy of women in fertile age after the heart transplant. The aim was to find out, whether there are women after the heart transplant in the Czech Republic, who got pregnant, delivered a baby, or are trying to get pregnant, or are interested in pregnancy, at least. The qualitative research utilizes a survey method, which was aimed at physicians, who are looking after patients after the heart transplant. Furthermore a non standardized interview was held with the physician of the Outpatient{\crq}s department for the Transplants at IKEM and with her patient, who is currently trying to get pregnant. Personal documents of the patient trying to get pregnant were used, in order to make the research more specific. It is apparent from the results that there is a small group of women after heart transplant in fertile age. Thus, for ascertaining the result, the foreign literature was used, which describes already successful pregnancies after the heart transplants in such women. Pregnancy risks must be divided into risks for a mother and for a fetus. The pregnant woman after the heart transplant is most endangered by hypertension and further to that there is a danger of implant rejection, infection, as well as premature delivery. The fetus is most endangered by infection and premature delivery. A majority of newborns are delivered with a low delivery weight. The abortion rate is relatively low compared to a number of successful deliveries. In the event that a woman desires to get pregnant, she is always informed of the risks associated with the pregnancy after transplant. The risks, which endanger a mother and a fetus, can be decreased to a minimum through a well monitored pregnancy. The thesis can be used for increasing awareness of women, who want to get pregnant after heart transplant.
