Occurrence of HLA-B27 antigen in patients examined for suspected rheumatological diseases

Author: Zuzana Lipenská Title: Occurrence of HLA-B27 antigen in patients examined for suspected rheumatological diseases Bachelor's Thesis Charles University in Prague, Faculty of Pharmacy in Hradec Kralove Field of study: Medical Laboratory Technician Background: The aim of the study of HLA B27 antigen in patients investigated for suspected rheumatological disease in 2015. Methods: Patient samples examined for the presence of antigen HLA-B27 in the HLA laboratory of the Transfusion Department, University Hospital Hradec Kralove are processed using the serological method. First is used the isolation of peripheral blood lymphocytes using a density gradient, thereafter lymphotoxical test with peripheral lymphocytes. Results: In 2015 were examined 1,329 patients. 207 persons had the HLA-B27, 1122 persons did not. Of the total number of examined patients were examined 754 women and 575 men. Positive was found 108 women and 99 men. The highest incidence of HLA-B27 was observed in the group of 20-29 years old, another slight increase was in the group of 10-19 years old and 40-49 years old. The examined persons had mainly a group diagnosis of disease of the musculoskeletal system with the most frequently diagnosed dorsalgia (M54). Problems of giving blood and blood components in the Czech Republic

Blood is an irreplaceable kind of tissue in the human body, therefore I have decided to focus on the issue of its donation. Blood has an unique and extremely important function in medicine. Its transfusion is needed in case of polytrauma, surgeries and many of others chronical diseases. I am going to focus on the blood donation as a process and then to sum up basic legislative demands which form the blood donation. In summary I am going to specify some methods of processing blood to transfusion preparations and I am also going to mention promotion of blood donation. In practical part of my thesis I am comparing annual statistics of blood donation in the Czech Republic and at the blood transfusion department of The University Hospital in Pilsen. HLA typization of celiac disease in pediatric patients

This theses is a study of the relationship between certain HLA alleles and coeliac disease. I am concentrating on individuals with a predisposition to coeliac disease and the link between these and the presence of particular alleles. In the theoretical part of the theses I'm describing the structure of the HLA system and the association of HLA with the disease. Furthermore I am concentrating on characteristics of the disease and possibilities of its diagnoses. In the practical part I describe in detail the molecular-genetical examination of coeliac disease. I asses the presence of predisposed alleles for this disease in patients from The II. Internal Gastroenterology Clinic , The Children's Clinic of Faculty Hospital in Hradec Kralove and other gastroenterology clinics in the region.
Cryoconservation of erythrocytes and its use in civilian and military healthcare

The theme of my final paper is Cryoconservation of erythrocytes and its use in civilian and military healthcare. This work consist of theoretical part and practical part. The first part of the thesis introduces cryobiology basics, types of cryoprotectants and the cryopreservation of erythrocytes itself. The thesis describes individual procedural steps leading to the final transfusion product, i.e. methods of blood collection, blood's pre- glycerolisation processing, glycerolisation itself and freezing. Furthermore, it describes issues associated with storage of frozen blood products, as well as with their thawing and deglycerolisation. The second part, which is practical, deals with quality control of cryopreserved erythrocytes. It describes various quality parameters and acquaints readers with the results of such measured parameters.

This bachelor thesis summarizes the basic information about Rh system, the second most important system of blood groups. This work is split into two parts. Theoretical part is focused on the blood - its composition and formation. Next, it focuses on history, antigens and antibodies of Rh system, prenatal and postnatal serological examination, laboratory testing of irregular antierythrocytes antibodies and hemolytic disease of a newborn. Practical part describes procedure examination screening of antibodies on the Transfusion departments in Havlíčkův Brod and detection of irregular antierythrocytes antibodies. The other human red blood groups systems.

Immunohematology studies antigen-antibody reactions and different forms of antigens and antibodies which cause the immunologic reaction of the organism. One of the hazards of blood transfusion is the immunization of individuals. The reaction between antigens and antibodies leads to the binding of the antibodies on the red blood cells membranes and premature degradation. Development of the antibodies affects about 0.1 - 3 % of the human population. Currently 270 blood group systems are known. The National Reference Laboratory for Imunohematology determines the specificity of antibodies against the blood group systems Diego, Yt, Xg, Scianna, Dombrock, Colton, Landsteiner-Wiener, Gerbich, Cromer, Knops, Indian, Ok, John Milton Hagen, HFA and LFA. For the detection of antibodies against the mentioned blood group systems uses the laboratory proper assays and Scarf - an archive of serum with antibodies and red blood cells containing the special antigens. Over the time period since 1st January 2007 till 31st December 2011 102 antibodies against the named blood group systems were detected. Some of these antibodies are clinically significant and subsequent hemoteraphie is very complicated. The most detected antibody was anti-Wr(a), an antibody against the low frequent antigen Wr(a) Diego system.

Failure to comply with the correct blood practices may affect the safe transfer of blood products. To ensure the safe transfer are therefore important theoretical knowledge of health professionals, who are entitled to the blood products. The result of the test, which I form, clearly speaks for more training in this area. The correct blood practices relating to: before transfusion procedures performed outside of the blood bank (clinical Department), before transfusion examination carried out on blood bank, handling with transfusion preparations for clinical departments. Failure to comply with best practices we will post the patient at risk of post-transfusion reactions. Even in the case of suspicion of post-transfusion reaction are important theoretical knowledge of health professionals, they should know how to proceed. Testing of HLA antigens and antibodies in the laboratory of transfusion department.

Methods: Following examinations were performed on potential kidney recipients in the HLA laboratory of the Transfusion Department, University Hospital Hradec Králové: serological fenotyping HLA-A, B (CDC test), PCR HLA-DRB1* genotyping (SSO, SSP) and screening of antileukocyte antibodies (CDC test). Results: The total number of 154 potential kidney recipients were examined during the period of time from 1. 1. 2008 to 31. 12. 2010 (2008: 57, 2009: 46, 2010: 51). Screening of antileukocyte antibodies was performed on 1 140 samples, the most frequent result of antibody levels was between 0 - 30 %. Most patiens were at age of 41 - 61 years, male/female ratio was 2:1. The most frequent HLA antigens concerning the group of patients with kidney diseases were HLA-A2, HLA-B8 and HLA-DRB1*11.
