Comparative analysis of the demographic situation in the Czech-German borderland in the period 1992-2007

The presented diploma thesis aims to analyse and evaluate the development of demographic situation in the Czech-German borderland in the transformational period (1992-2007). We define the Czech- German borderland as the area of 14 Czech districts bordering on Germany, which we further devide according to the borders with the neighbouring federal country to the Bavarian and the Saxon section. 11 indicators in total enter the comparative analysis, which characterize the age structure and the aging of the population, fertility and mortality, natural reproduction, migration and the total population development. The comparison is carried out in three dimensions: a) between the Czech-German borderland and the averadge level of the Czech Republic; b) between the Bavarian and Saxon section; c) among the districts in the Czech-German borderland. Analysis verifies specific position of the surveyed area from the point of the population development in the Czech Republic. The Czech-German borderland features younger age structure, higher level of fertility and natural reproduction, worse mortality and lower rate of net migration. Analysis of work incapacity

The aim of this thesis is to analyse the trends of incapacity for work in the Czech Republic since 1990. These trends are related to the legislative and socioeconomic changes, which are the results of transformation of socialist society towards capitalist society. In this thesis, there are followed three main indicators of incapacity for work. The number of cases of incapacity for work per 100 sickness insured persons, which has dropped since the beginning 1990; the number of calendar days of incapacity for work per 1 case, which has on the contrary increased and the average percentage of incapacity for work, which has oscillated. The performed analyses have proved the crucial effect of sickness intensity (quantum) on changes of the values of mentioned indicators. The effect of intensity has explicitly outweighed the effect of age structure of population. Regression analysis has confirmed the hypothesis of causation of other factors, than only health factors, on the global level of incapacity for work. With respect to obtained information and level of sickness intensity by age and social groups, there is an issue of possibility and manner of reduction of their level, which exceeds the European average. Most discussed aspects are primary health care, while this debate is rather on the pages of daily newspapers than among the interested institutions. This illustrates the fact that currently there are no studies or conceptual material for the issues of demographic aging of doctors examined. And this was also the reason which became one of the impulses that led to the elaboration of this theme. Primary health care has had throughout the whole health system a very crucial position, despite the fact that its definition goes back to the end of the seventies of the last century (WHO conference in Alma-Ata in 1978).
