Plan of Measures for Implementation in the Event of an Emergency Situation at a Swimming Pool

The thesis focuses on creation of precaution „Plan applicable in case of exceptional/unsafe incidents taking place at TJ Tesla swimming pool located in Brno”. The theoretical chapter(s) deal with legislative framework and related bills regarding leak of dangerous substances from buildings. It also deals with protection of population within the Czech Republic with regard to separate measures applicable according to the character of incident. The practical chapter(s) deal with analysis of risks within the premises and define the character of unsafe attributes of the risk sources. The various risks are therefore modelled in order to show the possible range of unsafe effects on population. These models are necessary to define the most effective protective measures applicable while exceptional/unsafe incidents occur. The aim of the thesis is to create the specific precaution plan as well as dispute the way the incidents of unsafe substances of below the legislative limit amount could be handled.

Circulating tumor cells in personalized cancer therapy

Circulating tumor cells (CTC) have showed great potential to become both prognostic and predictive biomarker in various types of oncological diseases. CTC can help detect patients in higher risk of shorter overall survival, progression-free survival or relapse. They can be also helpful in therapy selection as in current clinical practice treatment is chosen based on primary tumor characteristics. Regular CTC counts and features monitoring can be real-time indication of therapy response and can be used to guide-targeted treatment. This information can be implemented to personalized medicine and each cancer patient can be treated based on individual profile. However, patients' sample with CTC is easily accessible, their detection has remained challenge due to low CTC number in the circulation and heterogeneous nature. CTC can circulate in blood in the form of single cells or in clusters that usually represent minority in comparison with single CTC but their metastatic potential is significantly greater than of single CTC. Apart from CTC count, the molecular character showed dynamic development and heterogeneous nature not only between patients but also within the individual patient's tumor tissue itself. The phenomenology of experience in extreme situations based on the example of unplanned caesarean sections

Our qualitative study "The Phenomenology of Experience in Extreme Situations Based on the Example of Unplanned Caesarean Sections" tries to map and to describe the phenomenon of extreme situations mainly from the point of view of their dynamic aspects and puts the emphasis on the experience of such. As a typical example of extreme situation we have chosen unplanned Caesarean Section on the background of historical - economic and social changes. Such example is to serve as a prototype of successful ending to the extreme situation. Therefore on ly such cases when both mother and child survived the delivery were included in our research. ln our work we are laying a question what experience of this concrete extreme situation means to women, what types of feelings may be connected with such experience and above all what various ways there are to cope with such experience. We are not actually trying to generalize because every such experience is a unique one. We are more searching for suitable theoretical frameworks, research methods and safe ways to allow mothers to gain distance from this difficult and sensitive experience so they can reinterpret such in a positive way. Then they can integrate such important and often traumatic event into their present every day life and enrich it by another experience dimension.

Significance of detection of human papillomaviruses in draining lymph nodes in cervical cancer paients

Background: Metastatic involvement of pelvic nodes is the most important prognostic parameter in early-stage cervical cancer. Still, approximately 15% of patients with negative pelvic nodes experience recurrence, majority of them in the pelvis. The presence of HPV DNA in histology-negative pelvic nodes is considered a subclinical metastatic spread. Methods: Patients with early-stage cervical cancer referred for surgical treatment were enrolled in the study. Cytobrush technique was used for sample collection from the fresh tissue to avoid any loss of material for histology. Results: Altogether, 49 patients were enrolled in the study. High-risk (HR) HPV was identified in the tumor in 91.8% patients, and in the sentinel node or other pelvic nodes in 49.9% patients. Among the 10 HR HPV genotypes detected, HPV 16 was the most frequently represented in both the tumor and the lymph nodes (66.7% and 71.4%, respectively). All metastatic lymph nodes were HPV positive. Conclusion: The presence of HR HPV DNA in a sentinel node had a positive predictive value for metastatic involvement of pelvic lymph nodes in our study. This could be considered a sign of an early subclinical metastatic spread; however, the prognostic value has to be evaluated through a longer follow-up.

A Plan of Measures in Case of Exceptional Incidents on Manufactury Premises

This Diploma thesis deals with creating the Plan of Measures in Case of Exceptional Incidents on manufacture Premises. The first part of thesis is dedicated to description of Exceptional Incidents, primarily accidents with leak of hazardous chemical substances, legislation background of Exceptional Incidents and Emergency planning, Civil Protection, as a complex within Czech Republic and then individual measures (the plan are assembled of) are descripted. The Plan of Measures, as such itself, is created on the basis of processed analysis of hazardous properties connected with chemical substances, used in McBride, a.s., mainly the Acids and mixtures of alcohol. The leak of this compounds was simulated in software ALOHA, which gives information about degree of threat to inhabitants of this area. Suggested Measures are created to be practicable in particular situation in surroundings of McBride, a.s. Aim of this Diploma thesis is not just to create a particular Plan, but integrally describe the issues of Emergency planning especially for smaller subjects within municipality, that do not come under the scope of law n. 59/2006 Sb., and to create a tool, that can help to other creators of similar Plans of Measures to understand to partial relations.
