Kordarna - the first large case of reorganization under the new insolvency act

The long awaited insolvency act in force - Act 182/2006 Coll., changed the whole insolvency process by enabling new solutions to handle corporate insolvency issues. Among the most revolutionary features is the facilitation of a reorganization process. The thesis focuses on legal and economic aspects of the reorganization framework and compares it with the possibility solution of bankruptcy liquidation. The thesis analyzes the reorganizations of company called Kordarna, a.s. and also three subsidiary firms belonging to the KORD Group companies, which have been the first major test of the reorganization framework under the current Czech Insolvency Act. After the analysis of the procedural steps throughout the insolvency proceedings, the thesis arrives at a conclusion that given the circumstances, the chosen reorganization procedure was the best possible solution to Kordarna's insolvency for all classes of creditors and also other involved stakeholders. Moreover, the slow pace of the process is also discussed in detail. An important conclusion is reached in the field of bankruptcy estate valuation, where the thesis claims that the values of bankruptcy estates valuations were unrealistically high, because the adverse impact of the current extraordinary asset market conditions was never fully considered.

Comparison between flow in the left internal mammary artery harvested via sternotomy and via a left anterior small thoracotomy with clinical impact for the patient Introduction: In the treatment of coronary artery disease (CAD), in addition to conservative therapy and percutaneous interventions, is irreplacebly belong coronary artery bypass grafting (CABG). The gold standard in the surgical treatment of CAD is CABG from a median sternotomy access using cardiopulmonary bypass (on-pump CABG). A less invasive and economically advantageous is CABG from a median sternotomy access without cardiopulmonary bypass (off-pump CABG) and last but not least CABG through a left anterior small thoracotomy (LAST) approach (MIDCAB - "minimally invasive direct coronary artery bypass"). Although the final technical implementation anastomoses is essentially identical in off-pump CABG and MIDCAB, the difference is very important during left internal mammary artery (LIMA) harvesting, especially through a LAST approach, which can be difficult for the surgeon. 1 Summary Elective MIDCAB: Shunt or Tournique Occlusion? Assessment of a Protective Role of Perioperative Intraluminal Shunting on Myocardial Damage. Background: To determine impact of intraluminal-left anterior descending (LAD) shunt to prevent myocardial damage in minimally invasive coronary artery bypass (MIDCAB). Methods: 38 patients were randomly assigned to external tournique occlusion (TO, n = 19) or intraluminal-LAD shunt group (ILS, n = 19). Blood samples for cardiac troponin T (cTnT), Creatine Kinase (CK), CK-MB, myoglobin and aspartate aminotransferase (AST) were collected at 30 min. prior to, 6 and 24 hours after surgery. Results: 1 patient in TO and 2 in ILS group were excluded from futher analysis due to preoperative cTnT level above the 99th-percentile (≥ 0,01 μg/l). The anastomotic time in TO group was significantly shorter than in ILS group (13.44 ± 5.06 vs 18.9 ± 6.56 min., p = 0.0094). Postoperatively, each six patients in TO (33.3%) and ILS (35.3 %) group were above the 99th-percentile. 2 patients from each group (TO 11.1 % and ILS 11.8 %) had peak values above 10-% CV cutoff (p = 1). The differences in postoperative plasma concentrations of CK, CK-MB, myoglobin and AST between groups were similar. The clinical relevance of implementation of new technologies in extracorporeal circulation Summary Background: An organism's inflammatory response related to cardiopulmonary bypass (CPB) use is caused by activation of immune system cells with the production of a whole spectrum of mediators, whose activity can result in organ dysfunction. Current research, in trying to eliminate the negative effects of CPB, is engaging in innovating technology of systems for CPB. One of the last complex innovations is the so-called miniinvasive cardiopulmonary bypass. Till now though, the clinical benefits of using this system has not been clearly proven. Methods: A group of 54 patients, who were indicated for elective coronary surgery were randomised into two groups - group cCPB (patients operated on using classic CPB in the open modification) and group mCPB (patients operated on using minicircuit). We monitored and compared concentration of interleukin-6, polymorphonuclear elastase, monocyte chemoattractant protein-1, interleukin-10 and soluble receptors for tumour necrosis factor-α in both groups during operation and postoperatively. The incidence of heparin resistance in patients operated with the use of extracorporeal circulation

Heparin resistance occurs in about 20% of population. In that case, after application of a usual dose of heparin (2-3 mg/kg) ACT will not be sufficiently extended. Aim: To determine the real incidence of heparin resistance in patients scheduled for a cardiac surgical procedure. To determine whether there is an association between preoperative treatment with heparin and the incidence of heparin resistance. Also, to establish whether there is an association between platelet count, patients' age, and heparin resistance. Patients and methods: A total of 624 patients scheduled for on-pump surgery were included into a prospective study over a period of three years. Preoperative and intraoperative activated clotting time (ACT) values were recorded. Additionally, four factors referred to in the relevant literature as potential causes of developing heparin resistance were monitored in all patients: age 65 years, preoperative platelet count ≥ 300 × 109/l, preoperative administration of various types of heparin, antithrombin concentration ≤ 60%, and a combination of all. Postoperative pain after cardiac and thoracic surgery

POSIPÍŠILOVÁ, Lenka Postoperative pain after cardiac and thoracic surgery The bachelor dissertation is divided into theoretical and empirical part. The pain is defined generally in theoretical part, description of the cardiosurgery and the thoracic surgery is in the next part and finally the separate chapter about postoperative pain. This part is finished by demonstration of improving the postoperative analgesia. The level of present postoperative analgesia on the ICU is evaluated through the use of the anonymous questionary and in next part results and suggestions for improving postoperative pain are presented. Results: average value of intensity of pain, which was evaluated by use of the VAS (visual analog scale), is 5,1 in category A, 6,8 in category B and 5,8 in category C. These values are high. On the contrary the patients are almost satisfied with postoperative analgesia care on our ward.
