Management of pre-hospital care in patients with refractory ventricular fibrillation.
ZEMAN, Denis
Ventricular fibrillation is a life-threatening arrhythmia that manifests as rapid and irregular contraction of the heart chambers. If not treated immediately, it can lead to circulatory arrest and death of the patient. Treatment of ventricular fibrillation consists of immediate cardiopulmonary resuscitation and defibrillation, which restores normal heart rhythm with an electrical shock. However, some patients may be refractory, that is, resistant to repeated defibrillation bursts. The first aim of my bachelor's thesis was to find out what awareness paramedics have of the options for managing refractory ventricular fibrillation. The second aim was to find out what methods paramedics use for refractory ventricular fibrillation in practice. The research investigation was carried out by means of semi-structured interviews with paramedics working at the South Bohemia Region ambulance service. The results show that paramedics are sufficiently familiar with the treatment options for refractory ventricular fibrillation. A negative finding was that not every rescuer was able to mention all the options for changing electrode positions and the correct pharmacological management. Another negative finding was that rescuers could not agree on the number of times that ventricular fibrillation is refractory. On the contrary, the investigated method of double sequential external defibrillation was found to be very successful and thus more beneficial than conventional methods in the treatment of refractory ventricular fibrillation. The results of the bachelor thesis may offer a way for paramedics to approach refractory ventricular fibrillation. This bachelor thesis may improve the management of refractory ventricular fibrillation, which should be approached differently than classical ventricular fibrillation.
New trends in cardiopulmonary resuscitation and their possible benefits for the treatment of patients in pre-hospital care
ŠKOLAUDY, Vojtěch
Cardiopulmonary resuscitation is a set of interrelated logical procedures that is essential for the survival of patients with sudden circulatory arrest. In recent decades, there have been significant advances in cardiopulmonary resuscitation methods and techniques, which have led to the development of new trends to potentially improve the outcomes of resuscitation efforts. The first aim of the bachelor thesis was to map new trends in cardiopulmonary resuscitation from the perspective of paramedics and the benefits of the trends for patients in pre-hospital emergency care. The second aim was to evaluate the differences between new and old trends in cardiopulmonary resuscitation. The practical part was developed using two qualitative investigations. The first research investigation was carried out by means of semi-structured interviews with paramedics working at the South Bohemian Region Ambulance Service. The second research investigation was conducted using document analysis, which focused on the Guidelines documents from 2000, 2005, 2010 and 2015. In these documents, we focused on the development of trends and practices within cardiopulmonary resuscitation. The results of the first survey show that paramedics are sufficiently familiar with new trends in cardiopulmonary resuscitation. A negative finding was that the general public does not use an automated external defibrillator in emergency resuscitation. Another negative finding was that although paramedics are professionally educated and trained, they have to consult a physician for administration of drugs during cardiopulmonary resuscitation. The second research investigation shows that trends and practices in cardiopulmonary resuscitation now focus on early defibrillation and well-performed cardiac massage. Based on the results, the bachelor thesis can provide the differences between new and old trends and the perspective of paramedics on current trends.
Modern technology in prehospital emergency care
BRŮŽKOVÁ, Nikola
This bachelor thesis deals with the topic of modern technologies in prehospital emergency care. The bachelor thesis is divided into theoretical and practical part. The theoretical part is focused on individual technologies, which are, according to available sources, considered modern. The set goal, which the bachelor thesis should fulfil, is to find out about applicability of modern technologies in prehospital emergency care. Two research questions were also established: which modern technologies are used in prehospital emergency care and what impact do modern technologies have on the providing of prehospital emergency care. To fulfil set goal, research study was conducted in a qualitative form via the semi-structured interview technique. These interviews were processed using open coding and after that, the analysed data were sorted into individual categories. The research sample consisted of paramedics of the South Bohemia region. The practical part of the bachelor thesis contains results of the research study, in which the answers to the previously mentioned research questions were found and in addition to that also the opinion on modern technologies in prehospital emergency care from the point of view of a paramedic. In the answers of the paramedics was found, among other things, that they all agree on the fact, that the app Záchranka benefits prehospital emergency care. On the contrary, a drone, as an unmanned aerial vehicle, does not have that much of a significance for the prehospital emergency care of the Czech Republic, actually exactly half of the respondents did not have any knowledge about this medium. It was also found, that, according to the respondents, modern technologies overall significantly influence prehospital emergency care, however the experience was not always a positive one. This bachelor thesis can be beneficial due to the processed information in the theoretical part and following results in the practical part.
Needs for Electrocardiogram Evaluation by a Paramedic
PROKOP, Jakub
This bachelor's thesis deals with the issue of the needs of a paramedic in the evaluation of an electrocardiogram. In the theoretical part, the evaluation of the electrocardiogram was described, starting with the basic anatomy and physiology of the vascular supply of the heart and the conduction system of the heart. Furthermore, a description of the electrocardiograph was given, and at the end, arrhythmias with ischemic changes were evaluated. The aim was to map the abilities of paramedics in evaluating the electrocardiogram and their need for education with possible suggestions for changes in education before and after starting work. Quantitative research was used in the empirical part. Data collection was carried out using a self-structured questionnaire, in which 24 items were listed. The questions were of both open-ended and closed-ended type, with responses rated using a modified Likert scale. The questionnaire was fully anonymous and intended for paramedics from the entire Czech Republic. The basic descriptive characteristics of the research group were processed. Quantitative data were expressed by mean and standard deviation and optionally supplemented with histograms according to predefined limits. For qualitative data, frequency characteristics (absolute and relative frequencies) were processed. The results of our research investigation point to the unpreparedness of paramedics in the evaluation of pathological changes on the ECG. Respondents were also found to have a need for further training in ECG assessment and would welcome periodic training on the latest in cardiology, assessment practice at their field bases, or specialist hospital internships. To improve undergraduate education, they would include an internship in a cardiac center, practical training in a cardiology department, or professional seminars focused on EKG evaluation.
"Subjectively perceived professional burden for social workers."
WEISE, Michaela
This diploma thesis is focused on the issue of subjectively perceived professional burden among workers in social services. The diploma thesis is related to the situation of the coronavirus pandemic and a government regulation that announced emergency measures to prevent the transmission and spread of infection in health and social services. The theoretical part focuses on workload, stress, burnout syndrome and prevention of mental hygiene. The practical part is a presentation of one's own qualitative research, which was carried out through in-depth interviews.
Rescuer's communication skills with foreigner
HLÁVKOVÁ, Petra
The subject matter of this undergraduate thesis is the ability of a paramedic to communicate with a foreigner. The theoretical part describes communication itself and it´s forms. It further deals with foreigners to be found in the Czech Republic and the very end includes information about the Emergency Medical Servis and paramedics. The practical part sets two subjectives. The first one is to determine the ability of a paramedic to communicate with a foreigner. The other to detect problems that might occur within such communication. The thesis uses the method of a semi-structured interview. The interviews were led with the Medical Rescue Servis of the South Bohemian Region´s paramedics and foreigners with no knowledge of the Czech language. The results were put in 5 and 9 categories, which helped to meet the objective. The results show that the interviewed paramedics are able to communicate with the foreigners by different means, sometimes with the help of various aids, their colleagues, witnesses of family members present at the place. The research also showed that the most common problem in communication with a foreigner is the language barrier. The main benefit of the thesis the awareness of problems that a paramedic can face in communication with foreigner and possible use various aids to make such communication easier.
Emotional Intelligence and Coping Strategies for Paramedics
Sarisová, Kristina ; Niederlová, Markéta (advisor) ; Hrachovinová, Tamara (referee)
The thesis deals with emotional intelligence and coping strategies for paramedics. The theoretical part provides information on the emotional intelligence of its models and measurement capabilities. It is then discussed strategies of coping with stress on its historical and theoretical approaches and the types of strategies and also about the methods by which we discover the use of a specific coping style. It is also described the characteristics of paramedic profession, qualification requirements for the exercise of the profession, as well as competence and personal characteristics of rescuers. The aim of the empirical part is quantitatively map a degree of emotional intelligence of paramedics, preference coping styles in this population and mutual respect between the two variables.
Selected legislation in the field of medical rescuers
MAREŠOVÁ, Ivana
In the theoretical part, the problematics of work legislation of paramedics in the Czech Republic is described. The thesis summarises legislation concerning the acquisition and recognition of the qualifications of paramedics, paramedics for urgent medicine, and ambulance drivers. Further, the thesis summarises legislation concerning the health rescue services, the rescue system, the medical operation centre, the emergencies, and the medical documentation. This thesis aimed at mapping the awareness of paramedics about selected legislation connected to their profession. To achieve the aim, three questions were set. The first question. 'How do paramedics evaluate the conditions of acquiring and recognizing the qualifications for performing their profession?'. The second question: 'How do paramedics evaluate their legally established competences?' The third question: 'What knowledge do paramedics have about the legislation relating to their profession?'. Qualitative research was used to collect data in the form of semi-structured interviews. The interviews were carried out with paramedics of South Bohemia. The result is the awareness of paramedics in the field of legislation and an evaluation of selected legislation from the paramedics' point of view. The knowledge of legislation describing the paramedics' profession isn't complex. The thesis points out the lack of knowledge of legislation and different opinions on selected topics among paramedics. The thesis could be used by students of the field of Medical Rescuer to gain an overview of the topic.
Application paramedic in a medical facility
ŠTĚPÁNEK, Marek
The title of the presented bachelor's thesis is: The application of paramedics in a medical facility. The theoretical part focuses on the possible employment of paramedics, i.e. in pre-medical and medical care. It then deals with the education, competences and lifelong learning of paramedics and nurses together with a mention of their motivation. The practical part employs the qualitative method of research. Data collection was carried out by means of a semi-structured interview. The researched group consists of 10 paramedic graduates from selected hospitals. The interviews were focused on the motivation for entering the medical facility, the possibility of their employment and what problems they face upon arrival. The aim of this study was to ascertain what problems the newly-employed paramedics encounter, and how they can be utilized in the facilities. Another goal was to find out what motivated them to start working in a medical facility. It was discovered that the main motivation was the lack of job vacancies or it concerned less specialised paramedic services care. It emerged that the difficulty with the employment of paramedics is the preference of licensed practical nurses with a specialisation, even though a paramedic possesses more competences. However, this is understandable because of the greater versatility of nurses. Nevertheless, the paramedics concurred that, after their integration, the situation on their wards met with their expectations. The main problem, which this study attempts to point out, is connected with the lack of job vacancies in EMS. This was the reason for many paramedics to start working in a medical facility instead of EMS, just to find a job position.
