Nursing care of clients around the shoulder joint prosthesis
HÖFEROVÁ, Eva
This bachelor´s work tries to specify and determine differences in the care of a client after a shoulder joint replacement. The shoulder joint is a complicated joint of the shoulder girdle which can be affected by many disorders and worsen the whole self-care of the client. If a damage or a disorder of the shoulder joint appears, the client may become non-self sufficient and dependent on the help of others. The bachelor´s work is divided into two parts. The first part is a theoretical part and deals with the history ? how replacements were performed and looked like if compared with the present times and the present possibilities of treatment. The anatomy has to be followed by another important information concerning indications and reasons for performing the replacement. The reasons are different. As far as we mentioned indications, we also have to mention contraindications, i.e. reasons for which the replacement must not be performed. After going through the contraindications, we get to a chapter about examining and here we mention the complete examination of the shoulder joint and how the shoulder joint is examined before the operation. An important part of this bachelor´s work is a preoperative care which relates to a complete preparation of the client for the operation. The preoperative care is followed by a postoperative one. The postoperative care starts at an intensive care unit (ICU) where the client is transfered from the operation theatre. At the ICU, the client is monitored and all undesirable situations that might appear are observed. The second part relates to carrying out the research and to setting the aims of the bachelor´s work. The research was carried out on the basis of a semi-structured interview with nurses in the Hospital České Budějovice on the The interviews were carried out since January to March 2013. All the written interviews were later transferred into an electronic record. The questions related to the nursing care of the client before and after the operation of the shoulder joint, to a position of the upper limb after the operation, and to the complete care of the client. The answers result in the fact that all the respondents know and manage the care of the clients after the operation of the shoulder limb. The major follow-up care includes putting the client into the right position and supporting his upper arm, checking the blood samples, an X-ray image and applying the Gilchrist- bandage. The second part of the interview was focused on providing the clients with information before and after the operation, who, when and what type of information is given to the client and what extent is the client informed to. All the interviews were evaluated and divided into cathegories. Two research questions, relating to this work, were determined. The research question about the principles in nursing the clients with the shoulder joint replacement, and the one about a way in which runs passing the information to the clients with the shoulder joint replacement. Both of the questions were answered. The answer for the first question was, that the major principles include the position of the operated limb, the Fowler´s position of the client, the X-ray image and applying the Gilchrist-bandage, carrying out the blood samples, checking the client´s complete condition and proper and careful rehabilitation. The second question, concerning the information, was also answered by the respondents. The client is told the information orally at the moment of his transfer from the operation theatre to the ICU department. But first of all, the client is told about what is going to happen on the Department of Orthopaedics by the nurses.
