The Work of an Operator of Emergency Medical Service

This bachelor thesis mostly dealt with the activity of medical controllers as well as the issues of medical operative center in general. Many people don´t know what it is all in all. It is only undemanding sitting at the table and answering the phone for most people. I would like to point out the facts related to this profession and refuted this claim of the majority. This thesis mentions the medical operation center, its history in the Czech Republic, and it is interested in its tasks, layout and equipment. There are also mentioned the activities of emergency medical service-controllers themselves, including their education or criminal responsibility. Emphasis is also placed on the stress on medical service-controllers in the performance of this profession. A large part of the work is devoted to taking the emergency call, telephone-assisted emergency resuscitation and telephone-assisted first aid. Mapping the theoretical knowledge of medical service-controllers and finding out the individual procedures for providing the telephone-assisted resuscitation of both, adults and children, was one of the aims of this bachelor thesis. Another goal was to determine the satisfaction of service-controllers within the performance of their profession. There was performed a qualitative research using the interview technique. The interview included knowledge and satisfaction with this profession as well as the issues of telephone-assisted resuscitation of adults and children. According to the results of this survey there was found that the medical service-controllers are not sufficiently prepared to perform this profession immediately after the graduating their studies. They use the same or very similar procedures within their telephone-assisted resuscitation of an adult or child. The survey also shows they are satisfied with their job.
Indications of Emergency Medical Services Responding

The bachelor's thesis is focused on a closer understanding of the issue of indications of emergency medical service (EMS) departures through the perspective of medical emergency dispatch centers. The issue of indications of emergency medical service departures is one of the still addressed and current topics in the pre-hospital emergency care. The activities of outreach crews as well as the activities of the dispatch centers are governed by Act 374/2011 Coll., however non-emergent, calls to the emergency line are becoming more and more frequent. The thesis maps the knowledge of the indication list of individual dispatchers and the attitude of dispatchers towards the indications itself. At the same time, it deals with the solution of unindicated calls and also the abuse of emergency services. The aim of the work is to compare the knowledge and procedures of dispatchers of both regions. The research took place at the Medical Emergency Dispatch Center of the South Bohemian Region in České Budějovice and at the Medical Emergency Dispatch Center of the Pilsen Region in Pilsen. The aim of the research was, among other things, to compare approach to indication issue between both regions. The research method chosen was a qualitative type of research that allows deeper examination of the topic. Research was executed by semi-structured interviews. The sample of respondents consisted of 8 dispatchers of the medical operations center i.e., people directly responsible for receiving calls and coordinating crews. The interview questions are focused on both the reception of the emergency call and the operational management. Above all, they focus on the urgencies N3 and N4, ones where we most often encounter not entirely unambiguous indications. It also addresses the relationship between the use of formalized and intuitive procedures in operators' decisions. Last but not least operators are confronted with the notion of EMS abuse and the solution of unindicated calls. The interviews provided information on a wide range of approaches to dispatching work but there was often agreement on key points. The results show that dispatchers are familiar with emergency procedures and legislation, however, despite the application of a largely formalized approach to classifications, it is necessary to act partly intuitively, as no call is ever the same as a regulatory case. There is a greater effort to define the terms unindicated calling and abuse of emergency medical services, which, however, are perceived individually by each of the dispatchers, as well as perceived by every rescuer who comes into contact with them. These concepts often evoke great emotions among dispatchers and rescuers. However, as the results show, even this problem does not have only one point of view, but it has several of them and all of them must be taken into account when solving it.
Receiving an emergency call on the line 155

Receiving an emergency call on the line 155 is the topic of this bachelor's thesis. The theoretical part of the thesis deals with the description of the medici operating center, the requirements and workload of dispatchers who are currently working at the medici operating center. It also describes the processing of an emergency call, the method of communication with the celer and, last but not least, telephone-assisted first aid and telephone-assisted cardiopulmonary resuscitation (CPR). In the practical part, two goals were set. The first goal was to map the knowledge of dispatchers in providing first aid for telephone-assisted cardiopulmonary resuscitation of adults. The sekond goal was to map the knowledge of dispatchers in providing first aid for telephone-assisted cardiopulmonary resuscitation of children. Qualitative research was performed using the semi-structured interview technique. These interview were conducted at the Medical Operations Center of the South Bohemian Region. The respondents were the emergency dispatchers .Ten respondents were randomly selected. The results were dividend into twelve categories, for better orientation at bachelor's thesis. Both set goals were achieved in this way. It was found that the dispatchers of the medical operating center responded very carefully and with a clear dose of knowledge to the issue of telephone-assisted CPR of children and adults. Dispatchers have knowledge and can describe the procedure of individual steps efore resuscitation. Knowledge and experience with AED was also at a good level. Respondent's answers regarding the need for resuscitation for more disabled people did not agree, some had no answer. Dispatchers could not imagine this situation and would not know how some would make decisions. When communicating with someone who refuses cardiopolmonary resuscitation, dispatchers disagreed. The results of this work showed the need to include in training programs by the employer methods of communication during urgent resuscitation, preferably using model situations.
Design and implementation of an IMS network with extended functions using Open IMS Core.

The bachelor thesis focuses on the architecture of the IP Multimedia Subsystem, its main elements and extension functions. It also deals with the implementation of the IMS network in the simulation environment Open IMS Core, which is also briefly described. The first chapter describes the IMS architecture divided into layers and it also defines the basic elements such as CSCF and HSS. In addition, it briefly characterizes the most commonly used communication protocols and the structure of identity in IMS. The second chapter deals with the open source environment Open IMS Core, it briefly defines the individual parts and contains a list of configuration files. The third chapter introduces the additional services offered by the IMS architecture. It characterizes their basic functions and their way of working. The fourth chapter focuses on the practical part of the bachelor thesis. It describes configuration procedure of the Open IMS Core and testing of its functionality. It is divided into four sections according to the individual configuration steps.
Risk of Loss of Infomation Transmission in IRS Operation Centres

This thesis focuses on the evaluation of the risks related to the work of the operators of emergency call centres 112. It is divided into two parts, the first part deals with the theoretical basis of the analysis. It describes the legislation, the integrated rescue system, the operational centres of its basic components and their functioning. The second part of the thesis presents a risk analysis which reveals serious risks. It also includes measures leading to the elimination of these risks.

Dispatcher of medical operation center casts an important and significant role in the chain of survival for emergency resuscitation. Within a short time must quickly and accurately provide information from the caller, delegate a professional help to the place of an incident. Timely launch telephone-assisted resuscitation, which this thesis is about. The first goal was set to chart providing phone-assisted cardiopulmonary resuscitation dispatchers at the regional operational center of emergency medical services. Analyze the process by which the dispatchers manage and provide instructions for conducting cardiopulmonary resuscitation, whether these guidelines actually manage and provide proper instruction to all rescuers. The second goal was to explore the motivational factors MUDr. Ondřej Fraňěk, the author of a publication "The manual of medical dispatcher in operations center " who was deeply engaged in telephone-assisted resuscitation. This goal should be focused on the analysis of the story of the author and the origin of interest to the issue. The research was conducted by qualitative method of data processing. To the research part of the thesis I used to analyze six audio recordings of emergency calls from the archives of the Regional medical operation center of South Bohemia, where dispatchers recognized by the affected persons malfunction of vital signs and started the phone-assisted resuscitation. The recordings were transcribed into electronic form and analyzed. I also realized interviews with MUDr. Franěk, with MUDr. Kratochvil, head doctor of Air Rescue Service of South Bohemia and Mrs. Hornychová, deputy head of the paramedical staff of the Regional Medical Operations Centre South Region. The results of the analysis showed that most of the dispatchers is not progressing adequately in providing telephone-assisted emergency resuscitation. The second objective was not achieved because it has not been possible to obtain the data needed to evaluate the objectives. Dispatchers, after reading my thesis, you can think over this issue. Work can be an incentive for self-improvement and improvement of the results. Prospective students of paramedical staff in health service can help as a source of information.
THE MANUAL NOT ONLY FOR YOUNG FIREFIGHTER - What to do, when...?

This manual is based on the laws, promulgation and tools of the Generální ředitelství hasičského záchranného sboru ČR and The Ministry of Education, Youth and Sports and with the information from Záchranný kruh. It´s goal is to create practical manual including information about protecting inhabitants for the youngest firefighters. In the theoretical part of the bachelor´s work, I analyzed specific laws, other regulations, literature and other sources of information from the field of inhabitants protection. Certain areas of inhabitants protection are analyzed, for example informing and warning, evacuations or sheltering the citizens, their decontamination and providing first aid. Furthermore, I explained certain rare cases and its problematic. In the practical part I realized interviews with the chiefs of the young firefighters, who could spread this manual and use it during the training and other shared events of young firefighters. Interviews were done with six chiefs and their answers were compared. The results are, that the groups of voluntary firefighters doesn´t have such or different manual that they like it and would use it, however it needs to have an attractive concept and tasks made so that even the smallest firefighters can do them. Research question: Do the documents include enough information to create a manual for a higher quality of education of the smallest firefighters to protect inhabitants? There was an effort to gather as much information for this case and create a manual from the gathered knowledge and the interviews result. By creating the manual, it is proofed that the documents have enough information to create an manual, that will help educating the smallest young firefighters.

The aim of bachelor thesis was to assess knowledge of Budweis district inhabitants of points of interest and rules of communication during emergency call. Teoretical part of thesis deals with issues of points of interest and emergency calls and its legal standards. In the introduction the attention is paid to operative and informational centre. Law no. 239/2000 coll. about integrated rescue system and about change of some regulations concerns its obligation and permission and deals with united system of warning and notification. Next is described different function of united european number of emergency call 112 in the Czech Republic and other European countries. The teoretical part continues with describtion of points of interest used in emergency call system of the emergency number 112 and 150, and is ended by emergency call issues. There is described how to communicate from dispatcher´s or celer point of view, which complications can set in or which rules should be observed. The aim of practical part of thesis was to answer formulated research question no. 1 "How are Budweis inhabitants informed about point sof interest?" and no. 2 "How are Budweis inhabitants informed about rules of communication during emergency call?". There were formulated hypothesis: Hypotesis no. 1: "Does knowledge of Budweis inhabitants of point of interest reach 80 %?", hypothesis no. 2: "Does knowledge of Budweis inhabitants of point of interest depend on gender?", hypothesis no. 3: "Does knowledge of Budweis inhabitants of point of interest depend on age?", hypothesis no. 4: "Does knowledge of Budweis inhabitants of point of interest depend on education?", hypothesis no. 5: "Does knowledge of Budweis inhabitants of point of interest depend on activity in driving motor vehicle?", hypothesis no. 6: "Does knowledge of Budweis inhabitants of rules of communication during emergency call reach 80 %?", hypothesis no. 7: "Does knowledge of Budweis inhabitants of rules of communication during emergency call depend on gender?", hypothesis no. 8: "Does knowledge of Budweis inhabitants of rules of communication during emergency call depend on age?", hypothesis no. 9: "Does knowledge of Budweis inhabitants of rules of communication during emergency call depend on education?" and hypothesis no. 10: "Does knowledge of Budweis inhabitants of rules of communication during emergency call depend on activity in driving motor vehicle?", There was used survey, for answering researched questions and hypothesis, among Budweis inhabitants. The survey contained 4 parts. The first part contained 5 questions, which characterized respondents. Second part, called "PART A general questions about communication with operativ and informational centre", contained 6 questions and was used for unify the problem. The last part, made of two, called "PART B knowledge of point sof interest", contained 10 questions, and "PART C knowledge of rules of communication during emergency call", contained 8 questions, were the more important for the survey and were used for assessed results of the survey. The survey was carried out in elctronic form of questionnair in February and March in 2018 among Budweis inhabitants. The questionnaire was opened by 1881 respondents, but finnished and sent was only by 372 respondent. For the research was used 307 questionnairs. All the results were evaluated in software program Microsoft Excel. For improving results, was used statistical method 2 - test of good conformity. The results show that the knowledge of points of interest is insufficient, the limit of 80 % was not exceeded. But the results show that the knowledge of ruled of emergency call communication is sufficient, the limit of 80 % was exceeded. The whole survey did not prove dependence on gender, age, education or activity in driving motor vehicle. The results of bachelor thesis will be used for instruction of dispetchers of emergency number 112 and 150.
Dispatcher-assisted cardiopulmonary resuscitation

The main task of providing dispatcher-assisted first aid is to guide bystanders to giving the first aid to the victiim directly in the situation where the incident happened according to the possibilities and conditions. It's one of the basic roles of medical dispatch centre and each dispatcher working on dispatch centre has to manage this role. These basic instructions should be provide in many different situations. One of this situation is pre-hospital cardiac arrest. When the dispatcher detects cardiac arrest during the emergency call, he/she should start providing dispatcher-assisted cardiopulmonary resuscitation (DA-CPR) immediately. It means telephone assistance and guidance of bystander to perform cardiopulmonary resuscitation on the place of incident. The main aim of the research is to find out, whether the dispatchers are able to detect cardiac arrest in time and respond to it appropriately. It means iniciate the bystander to begin cardiopulmonary resuscitation, motivate him/her, instruct him/her correctly and easily, calm the situation and guide the bystander till the ambulance arrives. There are aslo set three sectional goals. Their task is to apprise with the work of the medical dispatch centre, with the dispatcher's job description and with dispatcher-assisted cardiopulmonary...
