Some aspects of pathogenesis and management of valvular heart diseases that are commonly treated with surgical interventions: aortic stenosis and mitral regurgitation
Fojt, Richard ; Moťovská, Zuzana (advisor) ; Špaček, Rudolf (referee) ; Kuchynka, Petr (referee)
Aortic stenosis (AS) and mitral regurgitation (MR) are the most common valvular heart diseases in the adult population. Additionally, they are the most likely to need surgical intervention. Treatment preceding surgical or percutaneous correction of a valvular defect is targeted primarily on symptoms, since causal pharmacological treatment is lacking. A detailed understanding of the mechanisms behind valvular heart disease is necessary to effectively intervene in the pathogenesis. Calcified aortic valve stenosis, currently considered an active process consisting of a wide range of cells, signaling molecules, and transcription factors, is largely consistent with the pathogenesis of atherosclerosis; however, about 50% of patients with calcified AS do not exhibit significant atherosclerosis. Our work was focused on measuring osteoprotegerin (OPG) levels in the tissue of calcified aortic valves. OPG acts as decoy receptor for RANKL and affects calcification and differentiation of various cells during progression of AS. We found significantly higher tissue levels of OPG in patients with isolated AS compared to patients with concurrent AS and coronary atherosclerosis. These findings support the hypothesis that there is a unique pathogenesis for each clinical manifestation of calcified aortic valve disease. MR is...
New diagnostic and therapeutic aspects of inflammatiory cardiomyopathy
Kuchynka, Petr ; Šimek, Stanislav (advisor) ; Krejčí, Jan (referee) ; Gregor, Pavel (referee)
Introduction: Inflammatory cardiomyopathy (DCMi) represents a non-familial form of dilated cardiomyopathy (DCM) and endomyocardial biopsy (EMB) is crucial for its diagnosis. Aims: To assess the prevalence of DCMi in patients with DCM of unclear origin, to evaluate the significance of serological tests for antibodies against infectious cardiotrophic agents and to analyze the effect of specific therapy guided by EMB results. Methods: EMB was performed in 56 subjects (mean age 52 ± 10 years) with DCM of unclear etiology and left ventricular (LV) ejection fraction (EF) < 40% with a history of heart failure less than 1 year. EMB samples were analyzed by immunohistochemistry, polymerase chain reaction (PCR) and electron microscopy. Results: Immunohistochemical examination revealed myocardial inflammation in 26 patients (46%), the PCR method detected genome of microbial agents in 32 patients (57%). Electron microscopy showed the presence of particles of microbial agents in 41 patients (73%). Serological blood tests found no IgM antibody positivity against any of the investigated microbial agents. Targeted antibiotic therapy in patients with evidence of Borrelia burgdorferi (Bb) genome in the EMB led to a reduction in LV size, improvement of LV EF and alleviate symptoms of heart failure. Conclusion: DCMi...

The Use of Project Management Methodologies for Implementation of Information Systems; their Comparison and Use in Practice
Kuchyňka, Petr ; Kačín, Radovan (advisor) ; Křivánková, Petra (referee)
The diploma paper introduces and, consequently, compares the worldwide accepted project management standards PMBOK Guide and CzNCB and ASAP Methodology for Implementation. Project management standards were presented by PMI and IPMA organizations, ASAP Methodology for Implementation was developed by SAP organization. The theoretical background is further applied on real situations within projects of implementations of SAP solutions that are realized by the company NESS Czech s.r.o.. Possible difficulties, which may arise during every implementation of SAP solutions, are being identified due to knowledge and experience of project managers and solution architects of NESS Czech s.r.o., not only during each phase of ASAP Methodology for Implementation but also during pre-implementation project phase. After settling the role and responsibilities of the supplier's project manager, the solutions how to avoid and/or react on possible difficulties, in charge of supplier's project manager, are being indicated.
