Approach towards women in birthing and postnatal care

This thesis focuses on the approach of obstetric and postnatal care providers towards women in the Czech Republic. To identify what experiences the mothers deemed essential and to explore the impact of physicians' approach towards them, qualitative research based on semi-structured interviews was conducted. Those interviews were later analyzed using mainly the sociological framework originating from Talcott Parson's role-based theory of the doctor-patient relationship, namely the paternalistic and partnership models of the doctor-patient relationship and the concept of person-oriented care. The main findings of the research are that interviewed women who experienced rather paternalistic model tend to imply overall dissatisfaction with the obstetric and postnatal care, meanwhile those women who experienced rather partnership model described their experiences as rather pleasant and stated they would choose the same hospital for labor again. The research identified six key topics, that interviewed women referred to as crucial for them in care providers' approach. Evaluation of satisfaction with maternal and child care in a perinatology centre - cultural differences

The bachelor thesis focuses on the evaluation of the quality of care provided in a perinatology centre from the perspective of women after childbirth. The aim of this study is to find out how satisfaction with the care provided varies according to the culture of the woman. The thesis consists of theoretical and practical parts. The theoretical part deals with the concept of perinatology centres, the quality and evaluation of the care provided, followed by the satisfaction of expectant mothers. Finally, the theoretical part includes a chapter on multicultural care, cultural minorities and multicultural communication. In my practical part I have processed the results of standardized questionnaires of the perinatology centre of the Gynaecology and Obstetrics Clinic of the 1st Faculty of Medicine of the Charles University in Prague. The survey highlights the main indicators of satisfaction with the care provided and cross-cultural differences in the perception of the quality of care. Women of other nationalities were also included in the survey and were given questionnaires in their native language to highlight cross-cultural differences in satisfaction with the care provided. Birth Plan at Birth

The bachelor thesis deals with the birth plan and its use during childbirth. It is divided into the theoretical and practical parts. The theoretical part is focuses on childbirth, birth periods, postpartum care, treatment of the newborn and caesarean section. Furthermore, the theoretical part deals with the birth plan, its writing and the various points that may occur in the birth plan. The practical part of the bachelor's thesis was carried out using a qualitative research investigation in the form of a semi-structured interview. The interview consisted of twelve questions. The interviews were conducted with eight women who used a birth plan during labor. For this bachelor thesis were set three main goals. The first goal was to find out the reasons for writing a birth plan by mothers. The informants wanted the birth to go according to their wishes. During the writing process, they sorted out what was important to them. Reasons for this included a negative experience from a previous birth. The second goal was to point out compliance with the birth plan at birth. The research investigation revealed that the birth plan is usually followed. Three of the eight informants indicated that not all points of the birth plan had been followed. Five of the eight informants said that their birth plan had been followed. The third goal was to determine changes in the birth plan during labor. During childbirth, the informants from the research surevy changed their attitude towards the repture of the bladder pouch and the intake of painkillers. Informants gave birth in a different position than they originally intended. Informants also changed their birth plan because of birth complications, namely that bonding could not take place or they were not allowed to move freely during labor.
Midwife's care of a woman at risk of imminent preterm birth

This bachelor thesis deals with the topic of care for a pregnant woman with impending preterm birth. The bachelor thesis is divided into theoretical and practical parts. In the theoretical part, the is described preterm birth, its incidence in the Czech Republic, causes of preterm birth, diagnostic methods, therapeutic procedures in preterm birth, the needs of pregnant women with this diagnosis, and the care of midwives for women at risk of preterm birth. Qualitative survey research was used to develop the practical part. The first research group consisted of 10 women with a diagnosis of preterm birth, who answered questions in a semi-structured interview. The second research group consisted of 7 midwives at the Risk Pregnancy Station. The research was conducted during April and July 2021. Participation in the research was entirely voluntary. Before the start of the interview, the participants of the interview were informed about the topic of the bachelor's thesis and about the protection of personal data. The interviews were conducted anonymously and, after approval by the informants, recorded on a mobile phone. To maintain anonymity, the informants were marked with numbers I1 to I10 and P1 to P7. After that, the interviews were literally transcribed, analyzed, and processed using coding, the paper and pencil method. For the research of this bachelor thesis, two research questions were determined to find out how women perceive the care of a midwife in case of imminent preterm birth and how midwives perceive the care of a woman with a diagnosis of preterm birth. Based on the research survey, it was found that all informants were satisfied with the care of midwives. Research has shown that there were several factors for women's satisfaction with the care of midwives during hospitalization. These factors included the approach of midwives to their willingness and ability to communicate with women. Even though all informants were satisfied with the care and communication from the midwives, the research showed the dissatisfaction of women with the communication with the attending physicians. Dissatisfaction was expressed by informants I1, I3, and I6 when information from doctors seemed to be insufficient to them. Research has further shown that midwives are aware of the importance of their empathic approach in the care of women hospitalized with a diagnosis of preterm birth and also perceive the changing needs of women during hospitalization, which need to be taken into account during care. This work points out the importance of a kind and empathic approach of a midwife to a woman with a diagnosis of preterm birth because that approach of a midwife is one of the most important aspects for a woman to overcome hospitalization at the Risk Pregnancy Station of the gynecology and obstetrics department. The work also shows the influence of the communication of doctors to patients on the female psyche and the experience of hospitalization. The results within this bachelor's thesis can be used as feedback on the work of midwives at the Risk Pregnancy Station of the Gynecology and Obstetrics Department to improve the care of midwives for women with imminent premature birth and increase client satisfaction in the medical facility.
Satisfaction of women with the midwife's care during childbirth

This bachelor thesis focuses on the satisfaction of mothers with the midwifery care during childbirth. Two goals were set for the elaboration of the bachelor thesis. The first goal was to find out how mothers perceive the care of midwives during childbirth. The second goal was to record what factors are important for the satisfaction of mothers during childbirth. The bachelor thesis is divided into theoretical and practical part. The theoretical part is divided into four main chapters. The first chapter deals with the description of childbirth and the care of the midwife during the various periods of childbirth. In this section we will also find a section on preparation for childbirth. The second chapter deals with the issue of pain during childbirth, discusses the pharmacological and non-pharmacological control of labour pain. The third chapter describes the issue of the presence of a close person in childbirth and the fourth chapter is devoted to the characteristics of a midwife. In the practical part of the bachelor thesis, a qualitative research survey was used. The research group consisted of ten women after childbirth. Data collection was performed through semi-structured interviews. The interview was completely voluntary. All informants were acquainted with the topic of the bachelor's thesis before the start of the individual interviews and were also informed about the protection of personal data during the processing of interviews (GDPR). The informants were marked with numbers I1 to I10. The informants expressed their verbal consent to the processing of the obtained information, and at the same time they agreed to the recording of the interview on a dictaphone or a mobile phone, if the interview was conducted via a mobile phone, due to easier elaboration of interviews. During the data processing, the interviews from the Dictaphone or mobile phone were transcribed in detail. The research survey lasted from March to April 2020. The research was divided into two main categories and further divided into individual subcategories. In the research part of the work, two research questions were chosen. The first research question asked how mothers evaluate the care of midwives during childbirth. The results of the research showed that all informants were satisfied and evaluated the care of midwives during childbirth very positively. Only informant I1 stated that her midwife was great, but during the reception she was cared for by another midwife whose behaviour she did not like. She was reportedly noisy, forceful and uncomfortable. The second research question dealt with the satisfaction of mothers during childbirth. Research has shown that maternal satisfaction during childbirth has several factors. As the most important factor of satisfaction during childbirth, the informants mentioned enough information and the overall approach not only of the midwife but also of the entire medical staff in the maternity hospital. Other important factors of satisfaction during the birth, the informants mentioned the presence of a close person at the birth and the pleasant environment of the delivery rooms. From the results of the research survey, it was found that mothers are satisfied with the care of midwives. All the interviewees informed that they were satisfied with their midwife during the birth. This work points out that it is important for the midwife to be friendly, kind and empathetic to the mother, because the midwife's approach is one of the most important aspects of satisfaction for the mother during childbirth. The results of the work can be used to improve and enhance the care of midwives, as well as for future midwives who acquire professional qualifications and are preparing for this profession. The results will be published at professional conferences for midwives and the results will also be presented to the maternity hospital in which the informants gave birth.
Anthropological perception of current home births without medical assistance.

The study discusses the current home births in developed countries. Introduction deals with the emergence of a model of human birth in the background of evolutionary mechanisms and the resulting biological risks, which are illustrated by data on maternal and neonatal mortality. Home births are understood in the complex scale, and so formation of current model of obstetrics in selected countries is shown in historical background. It brings most common image of women and the circumstances that lead to the choice of home birth. The study focuses on the comparison of home and hospital births due to the occurrence of undesirable health complications. As this comparison showed the results that births are largely influenced by medical interventions carried out, frequency of these procedures is also compared. Higher occurrence of interventions in hospital births can be partly explained by changes in the demographic profile of mother, which increases the risks associated with childbirth at home. Data processed in the study suggest that today's home births require extensive implementation of system changes, and therefore the home birth as a safe alternative is not recommended. The theme of my thesis is to look at the role of the father in the expectant mothers during pregnancy and in childbirth. The work is divided into a theoretical part and a practical part. The theoretical section discusses briefly the history of this issue, the preparations during pregnancy on a totally different life, what is the importance of psychoprofylaktické courses, the positives and negatives of the father's presence at birth and what are the roles of the partner for the birth. The main aim of the practical part was to determine whether active or passive help partner during pregnancy affect the mental state of women, whether partners visit antenatal courses, how to evaluate the presence of partner expectant mothers at delivery, what roles meet partner at birth and that the joint participation at birth should affect their relationship. To achieve the aims of quantitative research method has been selected in the form of a questionnaire survey. The questionnaires were handed out to women who have had a partner at birth. Research has found that most of the surveyed women cannot complain about the lack of attention on the part of the partners, whether it's about pregnancy is wanted or unwanted. A satisfaction of expectant mothers with care in the perinatology center

This thesis is split into two parts, theory, where I primarily define terminology of perinatology, medical staff, perinatology itself in Czech republic, neonatology, which the Apolinář centre is very well known for. And also research theory. Second part is so called practical part of the bachelor thesis, which concerns the area of research into satisfaction of mothers in care in the perinatology centre of Apolinář in Prague. It is primarily focused on satisfaction of mothers with intimacy, privacy, care, staff behaviour, quality of food, cleanliness of the facility and the impression of the environment, in which the woman experiences the most important moments of her life. Research has been conducted by surveying mothers in care of the facility, as the simplest most effective, anonymous and least time consuming method. This research has pointed out the strong ant he weak points of perinatology centre. The weak points are also addressed in more detail in chapter ,,Recommendation and prevention", where I try to present viable solutions for those issues. Doula as a psychological support women during childbirth

This thesis deals with the question why women use doula services for the birth of their child or children. The findings of the theoretical part describe the history of assisting women in childbirth and the current practice of people accompanying mothers to be and their role in childbirth. At the same time, it deals with the mental state of women during pregnancy and childbirth and circumstances that affect this state. For the practical part qualitative research was used based on interviews with women who have had the experience of childbirth with a doula and without, in the first phase, and based on questionnaires to respondents who have not used a doula during their delivery, in the second phase. Research findings have identified factors that act on childbirth and affect the psychological state of mothers. The research also indicated areas in which the services of a doula positively contribute to a favourable childbirth. These areas are the main reasons why women use doula services during delivery.
