Haemolysis and other biochemical evaluations of vacuum-assisted venous drainage in cardiac surgery Aims of the study: Vacuum-assisted venous drainage (VAVD) improves the quality of venous return in procedures using extracorporeal circulation systems (ECC). Nevertheless, there is not an evidence that such high negative pressure applied to ECC in combination with selective bicaval cannulation due to open heart surgery cause a trauma to blood elements and deteriorates organ function. A prospective randomised study was designed to demonstrate that negative pressure of -20 mm Hg to -80 mm Hg does not cause a significant haemolysis and organ deterioration in such procedures. Materials and methods: 85 consecutive patients undergoing combined cardiac surgery procedure with two separate venous cannulas were randomised in three groups A, B and C. VAVD with negative pressure of -20 to -45 mm Hg was applied to 28 patients in group A and negative pressure of -45 mm Hg to -80 mmHg was applied to 28 patients in group B. There was zero negative pressure applied to 29 patients in group C. Using the questionnaire survey and in-depth interviews, the attitude of recruiters to recruiting new employees through social networks, decision-making based on social network information, and employee control through these net-works was investigated. The thesis also examined which social networks are given the greatest weight and what information the HR officers are looking for.

The clinical relevance of implementation of new technologies in extracorporeal circulation Summary Background: An organism's inflammatory response related to cardiopulmonary bypass (CPB) use is caused by activation of immune system cells with the production of a whole spectrum of mediators, whose activity can result in organ dysfunction. Current research, in trying to eliminate the negative effects of CPB, is engaging in innovating technology of systems for CPB. One of the last complex innovations is the so-called miniinvasive cardiopulmonary bypass. Till now though, the clinical benefits of using this system has not been clearly proven. Methods: A group of 54 patients, who were indicated for elective coronary surgery were randomised into two groups - group cCPB (patients operated on using classic CPB in the open modification) and group mCPB (patients operated on using minicircuit). We monitored and compared concentration of interleukin-6, polymorphonuclear elastase, monocyte chemoattractant protein-1, interleukin-10 and soluble receptors for tumour necrosis factor-α in both groups during operation and postoperatively. An increase in a mean age of the candidates for coronary artery bypass surgery is observed due to universal trend of the whole mankind to grow old. Standard methods using cardiopulmonary bypass and cardiac arrest have achieved undisputable reputation in cardiac surgery and in low-risk procedures they present excellent results. In comparison with younger groups of patients, the elderly showed higher risk of mortality, stroke and morbidity dependent on using cardiopulmonary bypass, general anesthesia and prolonged in-hospital stay. New trends in cardiac surgery seem to be the alternative to accomplish better outcome of this high-risk group of cardiac patients. Those are generally the procedures in surgery, anesthesia and postoperative care called "minimally invasive". An effort to minimize the surgical trauma, prolonged ventilation time and length of stay in postoperative ward as well as to offer prompt verticalisation, rehabilitation and shorter in-hospital stay are the key characters of the abovementioned procedure. The "Aortic No-touch Technique" is an up-to-date method with potential to fulfill the criteria of "minimal invasiveness". The incidence of heparin resistance in patients operated with the use of extracorporeal circulation

Heparin resistance occurs in about 20% of population. In that case, after application of a usual dose of heparin (2-3 mg/kg) ACT will not be sufficiently extended. Aim: To determine the real incidence of heparin resistance in patients scheduled for a cardiac surgical procedure. To determine whether there is an association between preoperative treatment with heparin and the incidence of heparin resistance. Also, to establish whether there is an association between platelet count, patients' age, and heparin resistance. Patients and methods: A total of 624 patients scheduled for on-pump surgery were included into a prospective study over a period of three years. Preoperative and intraoperative activated clotting time (ACT) values were recorded. Additionally, four factors referred to in the relevant literature as potential causes of developing heparin resistance were monitored in all patients: age 65 years, preoperative platelet count ≥ 300 × 109/l, preoperative administration of various types of heparin, antithrombin concentration ≤ 60%, and a combination of all. Effect of perinatal hypoxia on cardiac tolerance to acute ischaemia in adult male and female rats.

Background: The number of adult patients with history of hypoxaemic insult in childhood has increased significantly. Cardiac interventions performed on these individuals in adulthood have become frequent. The evidence of their possibly specific response to ischaemia while undergoing these procedures is scarce and not sufficiently conclusive. Aim: The aims can be summarized as follows: 1. to develop a method of appropriate measurement and assessement of infarct size induced by regional ischaemia on isolated perfused rat heart; 2. to examine the effect of different types of perinatal hypoxia to the tolerance of the adult myocardium to acute ischaemia/reperfusion injury with regard to sex. Conclusions: The results of the present study support the hypothesis that perinatal hypoxia is a primary programming stimulus in the heart that may lead to sex-dependent changes in cardiac tolerance to acute ischaemia in later adult life. This would have important implications for patients who have experienced prolonged hypoxaemia in early life.

This work named When actor writes a play is mostly dedicated to the Authorial theatre and the Authorial acting. I tried to describe the creation and the production of graduate and other authorial performances that I personally contributed to. I also described the creation of the play Siegfried´s Idyll, which I wrote and staged. The original text of the play is included in my dissertation work. By writing the paper I recapitulated my present theatre life and hopefully I learnt my lesson for the future career.
