Early mobilization in critically ill patient
Veberová, Marta ; Pucholtová, Romana (advisor) ; Hocková, Jana (referee)
Introduction: The most of critical ill patients is immobilised for short or longer period of time. This depends on the character of their illness, medication or another organ support. As it has been proved during critical illness and forced immobilisation there is an significant reducing in muscle strength, exactly in 3 to 11 percent with every another day of immobilisation (46.) It is necessary to say that early mobilisation is not standardised approach in Czech medical environment. Aim of tthe work: The main target of this diploma is to figure out whether early mobilisation in critical ill patients means any significant difference in their functional status in comparison to those given standardised physiotherapy. Another target of this research is to set up safe and structured protocol for leading early mobilisation and to determine whether exercising with cyclo ergometer is linked to higher risk of side effects and to state the most probable reasons for delayed or not performed rehabilitation. Research methods: This diploma thesis is based on randomised research. Participants in the control group are undergoing standardised rehabilitation, intervention group is undergoing extended rehabilitation with cycle ergometer. The cyclo ergometer exercise will take place at least six times a week with...

Karasová, Veronika ; Klokočková, Šárka (advisor) ; Pucholtová, Romana (referee)
Introduction: This thesis deals with the time management of nursing care for patients in the ICU after bronchogenic carcinoma surgery. The treatment of patients with this diagnosis requires a multidisciplinary approach by all members of the medical team. Aims and methodology: The aim of this thesis was to map the time demands of nursing care for a patient who is the first day after lung cancer surgery, about current recommended procedures of nursing care. Furthermore, the aim was to find out whether a methodical guideline or a standard defining postoperative care of a patient after lung cancer surgery is available for the nurses. The way nurses perceive their working conditions and relationships in the workplace was also investigated. Finally, the aim of the diploma thesis was to map time management in nursing care for patients with lung cancer, all in the intensive care unit from nine medical facilities that participated in the research survey. Results: The research group consisted of nurses working in surgical intensive care units of the 1st, 2nd and 3rd degree and cardiac surgical intensive care unit from four university hospitals, 3 regional hospitals and 2 other hospitals. A total of 330 questionnaires were distributed. The return rate was 209 questionnaires (63.3%). The research results were...

Return to Life after a Post Intensive Care Syndrom Resulting from an Acute Kidney Injury Treatment
Sirmaiová, Anna ; Aboši, Alexander (advisor) ; Pucholtová, Romana (referee)
Introduction to the issue: Stay in an intensive care unit or anaesthesiology and resuscitation department has a demonstrable effect on the quality of life, whether mental, physical, or mental. A large percentage of patients experience post intensive care syndrome. The huge challenge for nursing care is to reduce this percentage and enable patients to recover in the best possible way and return to normal life of the same quality as before the hospitalization Methodology: The aim of this work is to find out how the quality of life of patients is affected after hospitalization in the intensive care unit or anaesthesiology and resuscitation department, with a proportion of acute renal failure and the need for continuous renal replacement. First, patients were evaluated with APACHE II score, SOFA and TISS 2. Next, questionnaires in which they responded to the period before hospitalization (SF 36, DEMMI, ADL, IADL) were filled, when released from ARO they went through the test of physical capability, (30s sit-up test, 6-minute walk test), further measurements when released from ICU took place (HADS, MAF, DEMMI, ADL, 30s sit-p test, 6-minute walk test), and after three months (SF 36, HADS, MAF, DEMMI, IADL, ADL 30s sit-up test, 6-minute walk test and a week of wearing a Garmin vivofit bracelet). Main...
Influence of bariatric surgery on the physical and mental state of the patient
Fořtová, Tereza ; Maffei Svobodová, Ludmila (advisor) ; Pucholtová, Romana (referee) ; Kulhavá, Miluše (referee)
1) INTRODUCTION: Obesity is a global problem and its the prevalence is constantly rising. Overweight and obesity are key risk factors for many diseases. Obese patients are a greater economic burden for the state, they often rely on the help of others. The goal of obesity treatment is to reduce weight and then keep it within the recommended range. In an obese patient, it is not always just about the actual weight loss, but also about treating the associated diseases caused by obesity. Bariatric surgery allows patients to lose weight in a relatively short period of time and has a very positive effect on associated diseases. 2) OBJECTIVE: The aim of this diploma thesis was to evaluate the physical and mental condition before and after gastric bypass and to determine whether this surgery has a positive or negative effect on the physical and mental condition of the patient. The objective was further divided into sub-goals and hypotheses. 3) METHODOLOGY: The research was carried out using a quantitative method. This is an observational study, in which the mental and physical condition and advancement of patients who were about to undergo bariatric surgery - gastric bypass - were monitored for 6 months. Mental status was monitored using four standardized questionnaires: MAF, HADS, iADL, SF-36 in four time...
Return to life after an PICS, based on acute respiratory failure
Šoltýsová, Veronika ; Hocková, Jana (advisor) ; Pucholtová, Romana (referee)
Introduction: Thanks to the progress of the today's medicine, in particular its therapeutic possibilities, the number of patients overcoming critical acute respiratory failure has increased, resulting in their exposure to stress and impact of intensive care. Thus, we can say that almost every patient suffers from at least one sign of post-intensive care syndrome. The theoretic part of the Diploma Thesis deals with acute respiratory failure and PICS. Materials used to prepare this work were obtained from a research made by the National Medical Library and from sources found in expert databases (PupMed, ResearchGate, ScienceDirect etc.). Aim of the work: The aim of this work is to examine and evaluate the physical and mental state of patients after overcoming the acute respiratory failure and their return to normal life with post-intensive care syndrome. We've divided this main goal into several partial objectives with the main research questions if the signs of PICS can be impacted by the length of artificial ventilation. Methodology: As a part of the research survey and data collection a qualitative research survey was performed on patients with acute respiratory failure by using standardized questionnaires: DEMMI, ADL, IADL, HADS, MAF, SF 36 and physical tests: 30-s SCT, 6 MWT and use of Garmin...
The concept of Snoezelen-MSE in the treatment of anorexia nervosa
Dvořáková, Pavlína ; Holanová, Petra (advisor) ; Pucholtová, Romana (referee)
The bachelor's thesis deals with the therapeutical effect of the Snoezelen multi-sensory room in the treatment of a subgroup of female patients suffering from anorexia nervosa. Because of their significant underweight and malnutrition, these patients exhibit significant impairment of cognitive functions, lower ability to solve problems, deformed interests and attention and memory problems as well. However, some of these can be renewed. This thesis explores the impact of Snoezelen-MSE concept-based therapy on the recovery of these patients. The thesis aims to find out how the patients themselves perceive the effects of their therapy in a Snoezelen-MSE room. This room was involved in the treatment programme of a specialised eating disorders treatment centre in 2017, as a modern method of brain neuroplasticity development. The research method is a semi-structured interview with female patients hospitalised in a ward specialised in eating disorders treatment, who suffer from anorexia nervosa with BMI (body mass index) lower than 16. The research shall be approved by the ethical commission. The participants shall be approached in the final stage of their treatment, their participation being completely voluntary. The patients shall have the right to refuse the interview without any consequences and they...

The influence of the burnout syndrome on the sexuality of general nurses in intensive care in university hospital
Bohuslavová, Monika ; Dynáková, Šárka (advisor) ; Pucholtová, Romana (referee)
Introduction: Burnout syndrome is a topical issue in the world and in the Czech Republic, not only in the healt sector. The diploma thesis is focused on finding the conection between burnout syndrome and sexuality of general nurses in emergency care. I chose the topic for my diploma thesis because it is close to me and I work in emergency care myself. In addition, the topic is not very well explored, and the research data will contribute to an international study examining the imapct od burnout syndrome on human health. This study in the Czech Republic is under the guidance of PhDr. Šárky Dynákové, Ph.D. and doc. PhDr. Jana Kožnara, CSc. Methodology: Data collection for research was conducted using two anonymous questionnaires. The first questionnaire focused on burnout syndrome using the Czech version of the Shirom-Melamed scale (Shirom, Melamed, 2006 In Ptáček, 2013). The second questionnaire examined the sexual function of a woman according to Mellan in Kratochvíl, S. (1999). The research sample consisted of general nurses working in emergency care at a teaching hospital. Results: The impact of burnout syndrome has been demonstrated by research in sexual desire (H1), self-assessment as a sexual partner (H2), and mood before sexual intercourse (H4). The results show that the area that achieved...
