The needs of the ICU patient after myocardial infarction requiring V-A ECMO support

Introduction: The coronary heart disease is still an actual topic. The mortality has been in recent years declining, but prevalence in the population stagnates, so involve a large group of patients. The incidence of acute myocardial infarction is still high. One of the methods of reperfusion therapy is the cardiac surgery by coronary artery bypass graft, which may be postoperatively complicated by the development of low cardiac output syndrome with the need of the extracorporeal membrane oxygenation. These patients require very specialized not only during the critical phase, but also during the long-term convalescence including rehabilitation, weaning from the artificial lung ventilation, self-sufficiency training and help with return to normal life. Literature search: The theoretical part is based on the research of National Medical Library (NLK) and Study and Scientific Library of the Pilsen Region (SVKPL). The PubMed, Scopus, Web of science and Science direct databases were used to search articles in professional publications, including the latest cohort studies. Accessibility and comprehensibility of information given to families of patients in intensive care

The thesis solves the quality of the process of passing information to the relatives / close patient in intensive care. A deficiency of important information or misunderstanding may cause anxiety and depression relatives and close patients. Communication with the patient's family must therefore be done methodically and systemically. The first part of the thesis brings orientation in the main terminology and introduces the already implemented strategies of solution of the problem in our country and in the world. The main objective is to create a methodical instruction for the OARIM (Department of Anesthesiology, Resuscitation and Intensive Medicine), which lists the priorities for informing relatives and close critically ill patients. The research maps the general view of the relatives of a critically ill patient hospitalized at OARIM on the process of communication and mutual co-operation with the staff. It also investigates how the OARIM patient's relatives assess the level of communication and interaction between them and the treating staff. Nursing care of the multiple trauma patient with communication barriers

Trauma is one of the most common causes of death in the population under the age of 40. Trauma causes irreversible and permanent disabilities and economically incalculable losses. In the Czech Republic, it ranks fifth among the most common causes of death. Trauma represents an extreme stressful situation for the human body. It burdens it and manifests itself with the disability of physical and mental functions. The man finds himself unexpectedly outside his social background and life security with an uncertain prognosis for a return to the lifestyle he had before the accident. In the theoretical part, this case study focuses on trauma and its division, specifically it deals with polytrauma and its pathophysiology and states the probability of survival. It points to the routinely used trauma protocol ATLS. The diagnostics and therapies in pre-hospital and hospital care are mentioned in the thesis as well as the most common complications of polytrauma. The conclusion of the theoretical part is devoted to communication, communication barriers and the definition of the nursing model according to V. Henderson. Role of nurse in organ donation in pediatric patiens

The topic of my diploma thesis is the issue of nursing care in pediatric pati- ents before consumption of organs at the resuscitation department. I consider this topic socially very topical because the number of transplant candidates on transplantation lists is constantly increasing. Since child transplant can- didates are the most vulnerable group, I have focused on child organ donors. The theoretical part criticizes the concept of death and threat, including examination methods that lead to brain death. I present the activities of the Coordination Center of the Transplant and Coordinated Coordinator in a donor program that seeks to proliferate the transplant program. An important component is legal legislation and ethical aspects, which are an integral part of the donor system in the Czech Republic. Study materials I searched in the databases NLK, SVKKL, Medvik, Embase, PubMed, Wiley Online Library, Medscape. The influence of nursing procedures on the incidence of vascular access complications during home parenteral nutrition

(EN) Home parenteral nutrition (DPV) has become a routine part of patients care whose oral intake is reduced or completely eliminated. An integral part of the application of DPV is also the permanent or temporary need for functioning vascular access. Catheter complications negatively affect the quality of life, morbidity and in case of catheter sepsis the patient mortality. In addition it also brings increased costs for therapy and the need for patients re-hospitalization. The main objective of our study is to analyze catheter-related complications and to determine whether use of catheter plugs like TauroLock and TauroLock Hep and Tegaderm CHG antimicrobial dressing affect the incidence of catheter complications with DPV, and whether the incidence of complications is affected by a person who treated the catheter. The secondary objective is to characterize patients with served DPV and types of catheter used for this purpose.The method chosen is a retrospective observational cohort study. The study contains 52 patients with 72 catheters (39 Broviac, 18 PICC and 15 ports). The monitored number of catheter- days in total was 33,875. Nursing Care of the Patient with Ischemic Heart Disease After Combined Cardiac Surgical Procedure with Following Complications

Bachelor thesis on topic Nursing care of a patient with ischemic heart disease after combined cardiac surgical operation with following complications, dealt with the specificity of nursing care of the patient, firstly after combined cardiac surgical operation and subsequently also after rehospitalisation for mediastinitis and infectious endocarditis on the heart valves. In theoretical part I analyze drawn knowledge, related to mentioned issue. Used information were obtained from professional literature, Czech and foreign articles according to recommended practices of the European Cardiology Society. The main objective of the thesis is summary and elaboration of patient's data in longer period of time into two case studies. In Case study I., I focus mainly on detailed description of nursing care after the cardiac surgical operation during three days, when the patient was transfered to intensive care unit. Verticalization of patients with peripheral ECMO in terms of nursing care

INTRODUCTION: ECMO is an increasingly popular method of support used in cases with severe respiratory or circulatory failure. Treatment is often associated with significant immobilization of the patient, which may take up to several weeks, resulting in deterioration of physical fitness, extending lenght of hospitalization, and a return back to normal life is very difficult. In recent years, however, efforts have been made to enable mobilization even to those patients who may be conscious, in order to maintain their muscle power. OBJECTIVES AND METHODOLOGY: The aim of this thesis was to obtain information about nurses' experience with mobilisation of patients treated by extracorporal membrane oxygenation, to evaluate an occurrence of the most frequent barriers that prevent them from mobilizing paticents and to evaluate differences in nursing attitude among the addressed workplaces. The research was carried out in the form of a questionnaire of my own creation. The research sample consisted of nurses working in Intensive Care Units under cardiocenters in the Czech Republic where the ECMO method is used.
