The impact of metabolic parameters on cognitive functions in bipolar patients treated with mood stabilizers
Hubeňák, Jan ; Tůma, Ivan (advisor) ; Látalová, Klára (referee) ; Bankovská Motlová, Lucie (referee)
TITLE: The impact of metabolic parameters on cognitive functions in bipolar patients treated with mood stabilizers. OBJECTIVE: Cognitive impairment in euthymic phase of bipolar disorder has been documented in many studies. There are several contributing factors exerting detrimental effect to such an impairment, e.g. sedative medication, thyreopathy. Cardiometabolic rick factors constitute another frequent condition found in bipolar disorder and exerting probably adverse impact on cognition. Since this condition is treatable and current literature suggests possible connection with cognitive dysfunction, this study aimed to explore such associations to identify promising targets of complex treatment. METHODS: Forty euthymic bipolar patients were enrolled into a prospective one-year study. Patients underwent the examination in the beginning of the study and one year after. Their body and metabolic parameters were measured. Medical history data were collected. Cognition was evaluated using battery of tests. Neuropsychological performance was transformed into neurocognitive composite score. Cognition of subjects was compared dichotomously according to presence or absence of pathological body and metabolic parameters. Correlations of selected parameters and composite score were done. RESULTS: Low...
Victimization of persons with serious mental illness.
Černý, Martin ; Vevera, Jan (advisor) ; Harsa, Pavel (referee) ; Látalová, Klára (referee)
Purpose: To estimate a) the state of the research of violent behavior and victimization of patients suffering with schizophrenia; b) to identify the prevalence of violent behavior and victimization in patients with schizophrenia and to study predictors of violent behavior. Method: a) detailed analysis of relevant studies in the PubMed database; b) data on violent behavior and victimization in the last six months, physical and sexual abuse in childhood and adolescence, substance abuse and post traumatic stress disorder were gathered in 158 patients with psychotic illness and matched comparison group. Additional information was collected from collateral informants. Several regression analysis was used to identify factors independently associated with recent violent behavior. Results: a) From relevant studies of patients suffering with schizophrenia 72% (n=176) exemined aggressive behavior, 23% (n=55) victimization, and 5% (n=13) both. b) In the sample, the presence of a psychotic disorder was associated with an increased risk of assaults (OR=3.80; 95% CI 2.060-7.014). Additional risk factors in persons with and without psychosis included recent physical victimization (OR=7.09; 95% CI 3.922-12.819), childhood maltreatment (OR=3.15; 95% CI 1.877-5.271), the level of drug use (OR=1.13; 95% CI 1.063-...
Predikce jaterní encefalopatie po TIPS
Routhu, Michaela ; Hosák, Ladislav (advisor) ; Češková, Eva (referee) ; Látalová, Klára (referee)
Prediction of Hepatic Encephalopathy after Transjugular Intrahepatic Portosystemic Shunt One of the common complications of Transjugular Intrahepatic Portosystemic Shunt (TIPS) is the development of Hepatic Encephalopathy (HE). Several studies attempted to find predictors of HE after TIPS in the past. Unfortunately, many findings were contradicted by other studies. We have completed two studies of HE: The first study focused on finding predictors of development of overt HE after TIPS. Data of 895 patients' cohort were retrospectively analysed. Patients' age, pre-TIPS portal venous pressure, serum creatinine, aspartate transaminase, albumin, presence of diabetes mellitus and aetiology of portal hypertension were associated with the occurrence of HE after TIPS (p<0.01). The knowledge of the risk factors could be helpful in aftercare planning for patients undergoing TIPS. The second study looked at HE in detail and studied cognitive changes prospectively. The patients were tested before TIPS, and 1 month and 6 months after TIPS. We used standardised hepatic encephalopathy test, PHES. The cognitive performance after TIPS worsened in the overall sample, which is consistent with world literature (median PHES score = -9). However, there was a significant subgroup of 8 patients in the sample, who showed a...
