The hyperactive urinary bladder in female patients with multiple sclerosis as seen by a physiotherapist

The aim of the thesis " A Physiotherapist's View of Overactive Bladder function in Multiple Sclerosis Patients" and the influence of physiotherapy on the symptoms of the overactive bladder. This thesis deals with exert influence on symptoms of an overactive bladder by physiotherapy. Therapy is based on the fact that guarding reflexes that maintains continence at the level of spine and subcortical (micturition centre in pons) are activated by the contraction of the pelvic floor muscles. Physiotherapy contains pelvic floor muscle training with biofeedback, behavioral training and training of stabilizing function of pelvic floor muscles. Powered by TCPDF (www.tcpdf.org)
Nurse?s role in the care of patiens suffering from disfunction of lower urinary tract.

The role of a nurse in the care of patients with dysfunction of lower urinary tract is one of the most urgent topics of modern urology, gynaecology and neurology. With regard to the latest results of epidemiological studies pointing out high prevalence in the population this problem has been in the centre of attention recently. Dysfunction of lower urinary tract is defined as a complex of symptoms. A quantitative research was performed to obtain the necessary information, employing the method of questioning with the technique of non-standardized interview. 8 patients with dysfunction of lower urinary tract and 8 nurses working at urological wards and outpatient departments in the South Bohemian Region were invited to participate. The following goals were set to obtain the necessary information: 1. To find out how patients with dysfunction of lower urinary tract gain information. 2. To find out how well patients with dysfunction of lower urinary tract are informed. 3. To point out the role of a nurse in the care of patients with dysfunction of lower urinary tract. The following research questions were set to reach the goals: 1. How do patients with dysfunction of lower urinary tract gain information? 2. What information is available to patients with dysfunction of lower urinary tract? 3. In what activities do nurses participate in the care about patients with dysfunction of lower urinary tract? The research has shown that the population is not well informed about this dysfunction. If the complex of symptoms occurs the affected persons seek help with general practitioners, gynaecologists or paediatricians and if the problems endure they are sent to a check up at a urological department. However there are lots of patients who procrastinate to go to a check up for months or even years. The role of a nurse in the care of these patients was also examined among experienced staff. The results may help nurses improve the quality of the care provided to patients with dysfunction of lower urinary tract. The obtained information may also point out the mistakes that should be avoided in cooperation of a nurse with a patient; the information from the research may serve for preparation of an educational plan.
The Nurse´s Role in Care of a Patient with Hyperactive Bladder Syndrome

The overactive bladder syndrome is a common disease that does not occur only in women, but it is almost equally common in men. The incidence is 17% in women and 16% in men in Europe and the United States. Despite its high incidence, only a small proportion of patients suffering from this problem visit their physicians. They try to cope with the symptoms of the disease alone. Embarrassment may be the reason of this. It is also caused by worries to inform other people about one?s intimate problems. Therefore, the overactive bladder has a negative impact on the quality of life of individuals and, in some cases, it can lead to social isolation. This thesis consists of two parts. It has a theoretical part and a practical part. The following objectives were set in the practical part. The first objective was to map the specific aspects of nursing care for patients with the overactive bladder syndrome. The second objective was to define the most frequent problems in the nursing care for such patients. The third objective was directed at mapping the knowledge of the nurses at urology departments of the overactive bladder syndrome. Research questions and hypotheses were set for these objectives. The first research question was: What are the specific aspects of nursing care for patients with the overactive bladder syndrome? The second one was: Do patients have enough information about the overactive bladder syndrome? The third research question focused on whether patients are afraid to talk with their physicians about their problems. The hypotheses set were as follows. H1: Nurses at urology departments have sufficient knowledge of the overactive bladder syndrome and H2: Nurses provide information on the regime measures to patients with the overactive bladder syndrome. Both the hypotheses were confirmed by the research carried out. To achieve the set objectives, a semi-structured interview was prepared for patients with the overactive bladder syndrome and a questionnaire was compiled for nurses at urology departments. The responses received through the interviews have been processed in the form of case reports. The results of the questionnaire survey were recorded in charts. The output from this thesis is an information leaflet created for patients with the overactive bladder syndrome and a system of nursing diagnoses. This will serve the nurses at urology departments.
