The issue of mentoring patients with degenerative diseases of the spine for autotherapy for long-term kinezioterapeutical plan
KADERA, Jan
This bachelor thesis focuses on instructing the patients with a degenerative disease of the spine for autotherapy from the kinesiotherapeutic long-term plan point of view. The right instruction for autotherapy is one of the most fundamental elements not only due to its correct understanding, but most importantly for the right home exercising. Unless the exercises are done correctly, a patient's conditions may worsen, which we need to avoid. In the theoretical part individual anatomic structures of the axial system are described, including vertebrae and their interconnection and of course the spinal muscular components, greatly contributing to both the static and dynamic stability necessary for all forms of motion. Then, there are the most frequently occurring degenerative diseases of the spine and their diagnosis possibilities mentioned here. In the next part different ways of treatment are described. The main treatment for the degenerative diseases of the spine is a conservative treatment involving variety of methods and concepts in terms of physiotherapy. In case this treatment is not successful, a surgical treatment comes into effect, which does not yet guarantee a 100% positive result beforehand. In that case we are talking about the so-called failed back surgery syndrom caused by a number of factors, and thus the treatment is not clearly defined. Apart from its therapy the main aim of the thesis is to evaluate the efficiency of the variety of ways of instructing patients because there are many approaches through which it is possible to instruct patients on dong exercises at home. For my thesis I chose three options, which are a verbal instruction, an instruction according to pictures and a video instruction. To achieve the aim, the qualitative method of data collection was used. The sample group consists of three patients with a generative disease of the spine. In the practical part based on the initial kinesiology examination I, besides the therapy, deal with the patients instructions on autotherapy and I assess their efficiency. During the therapy each patient was informed about a few possibilities of the instructions which they should subsequently follow when exercising at home. During the first visit an initial kinesiology analysis was conducted. The second visit included the therapy and an oral instruction on how to do the exercises. These instructions and exercises were the same at the third visit. The fourth as well as the fifth session was finished with the instructions for autotherapy through pictures. The sixth and the seventh session involved video instructions. At the end of each session every patient demonstrated the assigned exercises which were being corrected at the same time. During the final therapy session the output kinesiology analysis was performed and the success of the therapy was assessed. Simultaneously an uninstructed interview was conducted with each patient in which the patients expressed their subjective opinion on the executed therapy and the ways of the instructions. All the patients state the most appropriate instruction was the audio instruction because of its simplicity and clarity.

Problems of nursing care in endoscopic retrograde cholangiopancreatography.
RYVOLA, Lukáš
Theoretical groundwork Endoscopic Retrograde Cholangiopancreatography (ERCP from now on) is nowadays referred to as the golden standard in diagnostics and treatment of bile duct and pancreas diseases. At the same time it is regarded as the most reliable method of modern medicine in diagnostics of most common oncogenic diseases of pancreas. This examination combines endoscopic diagnostics with X-ray screening and it is unique thanks to the direct display of bile and pancreas ducts. Final picture is made by probing of Vater papillae filled by a contrastive liquid. Other merit consists of completing eventual therapy that follows the diagnostic ERCP. Disadvantages of this examination are complications that may arise already when probing or after a certain period of time. Therefore it is essential to consider each ERCP procedure carefully. Nursering ERCP at the endoscopy section as well as at the ward block of the gastrointestinal department bears its particularities. The priority at the endoscopic section is to employ manually and technically competent nurses skilled in ERCP so they can assist the gastrointestinalist properly. The nurses must be also able to establish contact with the patient to guide him through the examination, using professional skills. Exactly the nurses moderate possible concerns of the examined person. Communication at the ward block is crucial as well ? the nurses explain particularities of the treatment and instruct the patient in basic rules of hospitalisation. They also must be professionally skilled to care for patients after ERCP. Goals of the thesis The aim of this thesis is to monitor particularities of nursing ERCP at the endoscopic section of the gastrointestinal department and to learn how the general treatment of ERCP patients is carried out at the ward block.Qualitative methodology was used for the empirical part of the thesis. Data were collected by interviewing and hidden observations. Results The research survey indicated that ERCP examination nursing treatment bears its particularities that are well known and reflected by nurses at the endoscopy section. Patients confirm that the skills of nurses minimize their stress when undergoing the treatment. The survey indicated that an ERCP examination should be assisted by a nurse with not only technical and manual skills but also empathic and anticipating qualities in order to react appropriately without confusion. Nurses should be able to explain the treatment to the patient properly and to create a comfortable ambience. The survey also indicates that nurses actively seek for courses to keep their knowledge up to date. Particular complications are managed very well by the nurses, they are familiar with specific complications and are always prepared and able to provide the patients adequate nursing. Nursing at the ward block is also at high level in the field of knowledgeableness when informing patients or preparing arrangements and regimes. The patients themselves are satisfied with the overall care after the ERCP examination. The hidden observations show that the nursing after ERCP examination follows the literature properly. Pleasant surprise was that nurses at the ward block of the gastrointestinal department fully implement the five parts of nursing process ? from reviewing the health status of the patient in the beginning to their final evaluation, and that they concurrently create up-to-date nursing diagnoses.
Problems of nursing care for klient with senile dementia
KLOUDOVÁ, Martina
Dementia, also called old age disease is no more the problem of developed countries only, it has become a global problem. The population in the Czech Republic is growing old, which is caused by decreasing birth rate and decreasing mortality in all age groups. Dementia belongs to more frequent diseases than brain stroke and diabetes mellitus among seniors. In 65 year-old people the risk of dementia development is 5 %, at 75 it grows to 10 %. More than 30-50 % of seniors over 85 suffer from the disease (Jirák, Holmerová et al., 2009). Most nurses meet clients with dementia at their wards, which is why they should be educated in the field of specific needs in caring about these clients. The goals of the thesis were: 1. To indicate the specific problems of nursing care about a client with old age dementia. 2. To map the possibilities of educating hospital nurses in the problems of clients with old age dementia. 3. To find the negative factors affecting the psyche of nurses caring about clients with old age dementia. Four research questions were set to reach these goals. 1. What are the most frequent problems dealt with by healthcare staff caring about clients with old age dementia? 2. In what way a nurse communicates with a client with old age dementia? 3. What are the possibilities to educate nurses in the issues of old age dementia? 4. What are the negative impacts on the psyche of healthcare staff members working with clients suffering from old age dementia? The method of quantitative research by means of interviews was applied to elaboration of the thesis on the topic Problems of Nursing Care of a Client with Old Age Dementia. The interviews were semi-structured and were prepared for nurses from various standard departments of Strakonice Hospital. The interviews consisted of seventeen questions and were recorded with the consent of the nurses and consequently literally transcribed. The research was performed from 22 February to 1 March 2013.The research results have shown that all nurses often meet clients with dementia and they know the specific problems related to nursing care of these clients. The results also show that although they are aware of various forms of education in the issues of old age dementia they do not educate themselves and they consider other nurses uninformed. We found during the research that no forms of education are provided in this field at the departments. Despite the lack of information nurses try to solve problem situations with clients upon their experience or collecting information from other healthcare staff, which is usually insufficient. Because of these relations communication between a nurse and a client with dementia worsens, which leads to poor cooperation with both, a client and his/her family, and negative impacts in nurses? psyche like fatigue, stress and tiredness often occur, which is also reflected in their family lives. We think that nurses should be more strongly motivated to education in the issues of old age dementia. Education would bring nurses theoretical knowledge of dementia, it would help them create better cooperation with a client suffering from dementia and they would be able to prevent and duly solve possible problematic situations in the treatment and they would also learn how to communicate with a client with old age dementia efficiently. My bachelor thesis results in a draft of a seminar intended not only for nurses from consequent care departments, which contains the basic information on the issues of nursing care of a client with old age dementia (Annex 5).

Social dilemma of male homelessness
TICHÁ, Jana
Diploma thesis is focused on the issue of male homelessness in České Budějovice. In theoretical parts I described mostly general causes of homelessness and other specific problems associated with life of the homeless. I targeted the impediments in return to the common life, such as difficulties in searching the employment, health state of this population and also a barrier in communication with organizations and institutions, which would facilitate their return to the common life. In the next parts I described services available to the homeless persons to manage their difficult situation in living in the Czech Republic. In relation to the social services I described fundamental legislature relevant to rendition of these services. Statistical data from count homeless in Prague and Brno were marginally enclosed to the theoretical parts. In practical parts I concentrated on usage of alcohol and other habit forming substances by the homeless persons, and on their going through an execution of punishment in former times. Further I focused on the causes of their hard life situation. All information for this research I obtained from the clients, who use the services of sanctuary house of St. Pavel in České Budějovice.
