The care for physiological newborns in the period of postpartum adaptation from the point of view of parents and pediatric nurses.

The bachelor thesis deals with the care of physiological newborns in the period of postpartum adaptation from the perspective of parents and nurses. The work is divided into theoretical and practical part. The theoretical part describes the care of physiological newborns in postpartum adaptation, the physiology of postpartum adaptation of the newborn, the role of nurses and birth plans. The practical part includes the processing and evaluation of the results obtained in the research survey. Two goals were set for the bachelor thesis. The first goal was to describe the key and most frequently discussed points of care for newborns in the delivery room and partly in the area of physiological newborns. The second goal was to identify those parts of care that are most often a source of discrepancies between the perception of parents and health professionals. A semi-structured interview was chosen to meet these objectives. The interview contained 42 questions and was conducted with ten participants. Five pediatric nurses working in the neonatology ward and 5 mothers hospitalized after giving birth at the sextuplet station were selected. Based on the results, it was found that the most common points discussed in newborn care are mainly communication about overall care and procedures. Mothers sometimes have specific wishes regarding decontamination of the conjunctival sac and the form of vitamin K administration to the newborn. The most common discrepancy was found in these points, and despite these the most common points, the examination of the newborn on the mother's chest is mentioned. The research found that the most common source of disagreement between the idea of parents and health professionals is the lack of valid information about the overall care of the newborn.
Birth Plan at Birth

The bachelor thesis deals with the birth plan and its use during childbirth. It is divided into the theoretical and practical parts. The theoretical part is focuses on childbirth, birth periods, postpartum care, treatment of the newborn and caesarean section. Furthermore, the theoretical part deals with the birth plan, its writing and the various points that may occur in the birth plan. The practical part of the bachelor's thesis was carried out using a qualitative research investigation in the form of a semi-structured interview. The interview consisted of twelve questions. The interviews were conducted with eight women who used a birth plan during labor. For this bachelor thesis were set three main goals. The first goal was to find out the reasons for writing a birth plan by mothers. The informants wanted the birth to go according to their wishes. During the writing process, they sorted out what was important to them. Reasons for this included a negative experience from a previous birth. The second goal was to point out compliance with the birth plan at birth. The research investigation revealed that the birth plan is usually followed. Three of the eight informants indicated that not all points of the birth plan had been followed. Five of the eight informants said that their birth plan had been followed. The third goal was to determine changes in the birth plan during labor. During childbirth, the informants from the research surevy changed their attitude towards the repture of the bladder pouch and the intake of painkillers. Informants gave birth in a different position than they originally intended. Informants also changed their birth plan because of birth complications, namely that bonding could not take place or they were not allowed to move freely during labor.
Birth plan from a midwife' s perspective

The bachelor's thesis deals with the birth plan from a midwife's perspective. Two goals were established advance. The first goal was to find out the attitudes od midwives to the birth plan and the second goal was to find out what is the approach of midwives to mothers with a birth plan. The bachelor thesis is divided into two parts. The theoretical part and the practical part. The theoretical part introduces the profession of midwife and profession competencies. Furthermore, the theoretical part presents the importance of the birth plan, which women usually create a birth plan and the wishes that women can state in their birth plan. One of the chapters describes a childbirth and all his periods of birth (first, second, third and fourth). The end of the theoretical part is described the characteristic of the puerperium. The practical part of the bachelor thesis was based of a qualitative research survey. One research group was created with the help of semi-structured interviews, which was carried out with midwives working in the delivery room at Český Krumlov Hospital, a.s. and at the Hospital of České Budějovice, a.s. There were total an eleven midwives. The main criterion for the research survey was requirement for midwives to work in the delivery room and have experience with the birth plan. Before the interviews began, the topic of the bachelor's thesis was explained to all midwives and it was pointed out that the information obtained would remain anonymous. Interviews were conducted with the consent of the midwives. The individual interviews were recorded on a dictaphone and then literally transcribed. This was followed by data analysis and processing. To evaluate the data, two main categories and ten subcategories were identified.
Birthplan and its influence of the course of childbirth.

This bachelor 's thesis deals with a recent trend - birth plans. The beginning of my work focuses on anatomical proportions of woman and the changes that a body goes through during pregnancy. Following the psychological changes, psychoprofylactical methods from the rudimentary to the modern ones are analysed. The second chapter describes evolution of midwifery in the area of Czech lands and abroad untill present days. The last chapter of the theoretical part is focused on characterization of birth plans and what approach can a nursing staff adopt to the plans. The relationship of the birth plans and advance healthcare directive is also defined. The aim of the practical part is to investigate the influence of birth plans on the parturition. A research was conducted through an analysis of collected data. Subsequently, the acquired information was visualised in graphs and charts to achieve clarity. Delivery plann, its formulation and testing at The Gynaecological and Obstetrical Clinic in Plzen (GPK FN Plzen).

The subject of this bachelor thesis is delivery plan, its formulation and testing at the Gynaecological and Obstetrical Clinic in Plzen (GPK FN Plzen). Most women presently show active interest in pregnancy and delivery. They often imagine in advance the progress of childbirth, the behaviour of the maternity hospital staff towards them and the options offered by the hospital to delivering mothers. The delivery plan will provide to this type of women the progress of delivery most convenient for them. They may note their requests and desires in it concerning the progress of childbirth, its individual parts and potential alternatives. Thus the pregnant woman gets control over its coming delivery and may think the individual delivery parts over and prepare for them. The GPK FN clinic in Plzen does not dispose with its own delivery plan to offer to mothers-to-be within its offer of services to the pregnant. That is why this bachelor thesis will represent an asset both for the Plzen clinic of gynaecology and obstetrics GPK FN and for the mothers who decide to give birth to their child here. The theoretical part of this thesis includes chapters explaining the notion of the delivery plan and its individual parts. Further chapters briefly describe the progress of childbirth and the options of care of the delivering mothers in the course of the individual sages of delivery. The purpose of the present thesis was to find out which areas of the delivery plan are considered relevant by the pregnant women who have decided to give birth to their baby at the GPK FN clinic in Plzen and which parts of the plan are considered most important by the healthcare staff working at the GPK FN clinic in Plzen. Another objective of the thesis was to formulate the optimum delivery plan meeting the demands of both the delivering mothers and the staff of the GPK FN in Plzen and to find out the response of the healthcare staff of the GPK FN Plzen and the pregnant women to the use of the formulated delivery plan in practice. The theme was processed on the basis of a combination of qualitative and quantitative research. The needed information was obtained by means of post-structured interviews with open questions and a questionnaire with open questions, yes-no questions and semi-open questions. The subjects of the qualitative research included male and female obstetricians working at the delivery ward of the GPK FN Plzen, midwives assisting deliveries at the GPK FN Plzen, pregnant women hospitalised at the Risk Pregnancy Department of the GPK FN Plzen and mothers after delivery (in their puerperium) hospitalised at the Puerperium Department of the GPK FN Plzen. The research further included private gynaecologists of the Plzen {--} city district and gynaecologists of the IVF centre, and midwives working in the advisory centres of the private gynaecological surgeries in Plzen {--} city and in the IVF centre in Plzen. On the basis of the first part of the research the following research questions were formulated: ``What areas of the delivery plan are considered relevant by the pregnant women who have decided to give birth to their baby at the GPK FN clinic in Plzen{\crqq}, ``What areas of the delivery plan are considered relevant by the staff of the GPK FN in Plzen``, ``What should the optimum delivery plan look like to be usable for deliveries at the GPK FN in Plzen``. The second part of the research included evaluation of the following hypotheses: ``Pregnant women consider important for the delivery plan to include criteria assuring progress of their childbirth according to their desires``, ``The healthcare staff of the GPK FN Plzen takes a positive approach to the formulated delivery plan``, ``Pregnant women are interested in application of the delivery plan to their childbirth at the GPK FN in Plzen``.
