Operational management in stroke patients

The bachelor's thesis is concerned with the operation management and management in patients with a cerebrovascular accident (CVA) (stroke). It deals with stroke diagnosing and the subsequent stroke patients routing to a target medical facility. The theoretical part covers the general anatomy of arterial blood supply to the brain and the stroke origin pathophysiology, it also includes the assessment protocol concerning the pre - hospital diagnosis and emergent medical care of the patients. Moreover, this part covers specific guidelines for patients' routing to specialized medical centres in the South Bohemian Region. The investigation part is based on the qualitative survey using the method of semi-structured interviews with two target groups of respondents asked eight identical questions. The research sample is constituted by the employees of the South Bohemian Emergency Medical Service, the first group enrolling the operators of the Emergency Medical Centre, the second one the paramedics. The obtained data are analyzed and subsequently evaluated. The first goal is to map the operation management and management in stroke patients, and the second goal is to find out what is the paramedics' experience with stroke-diagnosed patients transport by the Emergency Medical Service and the subsequent routing the patients to a medical facility.
Specific tasks of Emergency Medical Dispatch centre when dealing with mass casualty.

The goal of this thesis was to analyze processes of the Emergency Medical Dispatch Center of the Emergency Medical Service of the Pilsen Region (ZOS ZZS Pk) when dealing with a mass casualty incident. The theoretical part of the thesis provides basic information relevant to the Pilsen Region, the Emergency Medical Service of the Pilsen Region, basic information on operations and general work processes of an emergency medical dispatch center during a normal work regime, and specific activities performed when dealing with a mass casualty incident. The research part of this thesis analyses activities and processes of an emergency medical dispatch center as described in the Mutual Mission Types Manual for Integrated Rescue System forces while at a mutual incident - Activities of Integrated Rescue System Forces at a Mass Casualty Incident (STČ 09/IZS). Further on the thesis evaluates activities of the ZOS ZZS Pk taken while dealing with two mass casualty incidents. These were a bus accident nearby Rokycany town in 2013 and a two trains collision nearby Horažďovice town in 2015. In order to make the analysis of readiness of the ZOS ZZS Pk to deal with mass casualty incidents more complex, SWOT analysis was added to the results. Based on the results of the STČ 09/IZS-manual analysis and of the SWOT analysis, suggestions for new manuals were made, that should make all the actions taken by employees of the ZOS ZZS Pk while dealing with a mass casualty incident more efficient and more effective. The goal of this thesis: to analyze activities of the ZOS ZZS Pk while dealing with a mass casualty incident, and to create suggestions of which inadequacies to remove and how to make operations of the ZOS ZZS Pk more efficient and more effective when dealing with mass casualty incidents; has therefore been fulfilled. The main outcome of the thesis are the final recommendations for making operations of the ZOS ZZS Pk more efficient and more effective when dealing with a mass casualty incident.
Comparing the activity of basic IRS operating procedures in Pardubice and Hradec Králové Region in 2008 - 2012

In this thesis, is compared to the basic operation of the integrated rescue system operating procedures in Pardubice and Hradec Králové region in the years 2008-2012 for the thesis were two objectives, the first objective is to assess the activities of the essential components of the integrated rescue system operating procedures, the second goal a proposal on measures to improve the activities that facilitate the work of the basic components of the integrated rescue system with mutual cooperation. It was determined one hypothesis, the basic operation of the integrated rescue system operating procedures in Pardubice and Hradec Králové is the same. The results will be presented with the basic elements of the integrated rescue system both regions to implement their own analysis, and streamline their operations. In the theoretical part of the thesis presents the basic components of the integrated rescue system, their brief description, basic tasks and historical development in the regions. Counties are also briefly described. In this work we discuss the operational management, Performance Operations and Information Centre of the Integrated Rescue System, Operations and Information Centre Fire Brigade, Integrated Operations Centre of the Czech Police and Health Operations Centre. Subsequently, the individual hotlines operating in the Czech Republic. The last section describes the basic types of intervention. The practical part deals with the comparison of the activities of the essential components of the integrated rescue system in Pardubice and Hradec Králové region in the years 2008 2012. Since the failure to obtain materials from emergency medical services in Pardubice and Hradec Králové, comparisons focused on the evaluation of cooperation Fire and Rescue choirs Pardubice and Hradec Králové region and the Police in both counties. On the basis of data obtained from statistical yearbooks affect activities of fire protection from the years 2008-2012 is compared cooperation Fire and Rescue Service and the Police regions of the Czech Republic in Pardubice and Hradec Králové, and cooperation Fire and Rescue Service regions and emergency medical services in Pardubice and Hradec Králové. The next section is compared cooperation Fire and Rescue Service Pardubice and Hradec Králové region and cooperation of the Police of the Czech Republic in Pardubice and Hradec Králové on the basis of information obtained from structured interviews with officials of both institutions. Planned statistical analysis, using bivariate methods has been carried out with regard to the lack of basic materials provided by the integrated rescue system. The research data collected from statistical yearbooks Pardubice and Hradec Králové region, and based on structured interviews with members of the Fire and Rescue Service Pardubice and Hradec Králové region as members of the Police of the Czech Republic in Pardubice and Hradec Králové, given hypothesis is confirmed. The activities of the essential components of the integrated rescue system is in operational management in Pardubice and Hradec Králové almost identical, with small variations.
Problems of Indications Rides Emergency Medical Services

The bachelor thesis deals with the issues addressed at the dispatching center of the emergency medical service. The indication of actions by emergency medical service has been a topical issue in the debate on provision of pre-hospitalization emergency care. In 2012 the dispatching center for the South Bohemian region was centralized in České Budějovice. The center managing the operations has acquired a more comprehensive overview of positions and activities of all emergency medical crews. This has also changed the approach to crew deployment coordination. The dispatcher now coordinates crews in action in the entire region, being the only person responsible for the correct procedure. The most important aid used by the dispatcher is a list of indications which describes various situations and procedures to deal with them. However, the list includes only situations when the action by the emergency medical service is required. The list of indications is issued by the provider of the emergency medical service based on the Act No 374/2011 Coll., on emergency medical services, and the Decree 240/2012 Coll., implementing the act. The research was conducted at the emergency medical dispatching center in České Budějovice. A qualitative method of research was selected as the most appropriate. The respondents were employees of the emergency medical dispatching center who are in charge of coordination. A part of the respondents performed both the function of emergency calls receiving and the function of operative management of crews. Semi-structured interviews with 15 questions were conducted with the respondents. The questions focused both on receiving of emergency calls and operative management of crews and they concerned primarily the urgency levels N3 and N4. A substantial part of the interview concerned the list of indications and confrontation of formalized and intuitive procedures in decision.-making by the operators. During the interviews the respondents were asked additional questions to ensure maximum accuracy of the answers. Tables or diagrams were used for several questions to obtain some quantitative results in order to determine prevailing opinions or facts among the operators. It should be noted that the data are approximate and have low validity due to the low number of the respondents. The surveyed group consisted nine of operators of both genders, different ages and different lengths of experience in the health sector. The research has shown that the operators use alternative options in ca. 10% of cases. They are very well familiar with the issues of indication of actions by the emergency medical service. Differences in decision-making result from a greater or smaller willingness to be exposed to risks of forensic consequences. The issue of indication of actions by the emergency medical service keeps developing. The general efforts head towards maximum formalization of the procedures to relieve the operators from their legal liability. The thesis maps procedures used in the emergency medical dispatching center and it may be used as a study material for medical workers who need more information about the issue.
Integration of operations centres in the South Regional Police Headquarters Region

As the topic of my diploma work I chose the issue of integration of operation centers in the jurisdiction of the Directory of Czech Police of the South Bohemian Region. This topic was chosen with regard to my job in the police of the Czech Republic and also with the assumption to assess the current state of the operation management in the jurisdiction of the Directory of Czech Police of the South Bohemian Region and compare it with the newly proposed integrated centre which should be created within the framework of integration of existing regional operation centres, evaluation of communication of operations centres within the framework of the integrated rescue system and within the framework of the police of the Czech Republic itself at different levels of operation management. The aim of my thesis is to describe the current state of workplaces of operation centers of the police of the Czech Republic and to assess their integration into a single operation center in the teritory of the South Bohemian region. I analyze the situation from the viewpoint of conduct, management and personal security and I assess material-technical security. Last but not least it's about confirming or refuting the hypothesis that was formulated before i started the thesis. This thesis stems particularly from internal act of conduct of the police of the Czech Republic and from the current legislation. Aims of my work are summarized in the capter Discussion which was preceded by description of the current state of operation centers and their function in the framework of operation conduct. Comparing the current and the suggested solution from the viewpoint of conduct, management, personal security, economic and material-technical security and physical integration. The formulation given by the hypothesis was confirmed based on the comparison carried out in the study of the current state and the prepared state, and seems to be effective. Because of the introduced measures that are realized by integration of the current operation centers into one, the effectivity will be increased.
Operational and Crisis Management within the Police of the Czech Republic

This thesis deals with the issue of operational and crisis management within the Police of the Czech Republic. The organizational structure and the staffing of operational centers and emergency management centers is described as closely related to police activities, with emphasis on the responsibility for executing tasks resulting in particular from internal acts of the Police management as the essential component of the Integrated Rescue System (IRS). The issue of emergency calls handling and the related communications between the basic components of the integrated rescue system, particularly between the Police and the Fire and Rescue Service, considering the technological and material and technical equipment of operational centers of the police of the CR is dealt with. The empirical section is focused directly on the "modern" Integrated Operational Centre of Regional Police Headquarters of the Vysočina region where questionnaires are distributed to workers (policemen) of this organizational element of the Police, the object of study is divided into three parts. The first part is focused on the IRS issues, the level of knowledge, orientation in this area and views on cooperation of the IRS components are identified by the questionnaire. The second part is focused on crisis management, the knowledge level of respondents in this area is determined. The third part of the questionnaire is directed entirely to obtain information and knowledge from practice of the staff members involved in special activities. The discussion and conclusion gives information about the progressive development and integration of operational centers, including the possible and most convenient way (system) of operational and crisis management within the Police of the Czech Republic.
Operational directing entity SDH micro-region Trebonsko and their exercise at solving extraordinary situation

In this thesis I am trying to document the activities of voluntary fire brigades op-erating in the micro region of Třeboň with its administrative centre in the town of Tře-boň. This is a flatland area in the southeast part of South Bohemia. There is a vast lake system, with a lot of culture spots, and diverse density of populated areas. There is no serious polluter of industrial origin, thus the region is a very attractive location for tour-ism. The population security is safeguarded by fire brigades of all categories according to the plan of the insurance coverage against fire. Emergency units consist of members of voluntary fire brigades. These civic associations are mainstays of culture, sports and social life, and a local fire brigade is a part of the integrated rescue system in compli-ance with the Act No. 239/2000 of the Code. It is an absolute matter-of-course for most people in this part of the region to help other people in emergency, thus involvement in various civic activities and associations is relatively extensive, and some of them belong among other organs of the integrated rescue system. In order to co-ordinate and unite the process of alert to remove the consequences of emergencies, laws, notices and regu-lations were issued and they are more or less followed. This thesis highlights some im-perfections and suggests some possible solutions.
Emergency Operators´ Mental Load

This bachelor thesis deals with mental strain in dispatchers of medical emergency services. I focused on covering these questions, the follow-up analysis and description, on the basis of research. The first objective was to survey opinions of operations centre dispatchers of the mental strain in their work. The next objective was their opinion of the question whether the number of dispatchers on duty in one shift influences their mental strain. My research is based on questionnaire surveying, and the research object covered 40 employees in operational centres all over the Region of South Bohemia. (Note: a nationwide research would be technically hardly realisable, as it is improbable that the questionnaires sent in would be returned). The introductory chapters contain a brief description of the operations centre and activities that the dispatchers perform every day, and I try to formulate the principles of the correct communication that should be led in the conversation with the callers on the distress line. I also describe general features of stress, the factors that lead to it, and the states breaking out as a result of its long-term effect on people. Specifically in this thesis, I describe post-traumatic stress disorder and the burn-out syndrome, and I also mention prevention to defend oneself against these risk factors. The second part of the thesis focuses on the results of the research conducted by quantitative technique, by handing out 45 questionnaires, 40 of which were processed after they were sent back. The research and results make up an essential part of the thesis and bring proofs of the fact that employees of operations centres undergo mental strain influenced by a number of factors. Some of these factors can be influenced, and becoming aware of these factors, this strain may be at least partially reduced. The thesis also uses the comparative method with the possibility of comparison of the mental strain between individual dispatchers. Last but not least, it contains inspiration for considering these questions, and personal proposals for a positive contribution to lowering this strain and a further development of this problem in general.
