Michaela Zahrádka Köhlerová Title: Myofunctional therapy in patients with orofacial disorders Abstract: This bachelor thesis deals with the issue of myofunctional disorders in the orofacial area and the possibilities of therapy. The work is a case study character and consists of theoretical and practical parts. The theoretical part provides an overview of the current knowledge about myofunctional disorders and therapeutic approaches. It covers an anatomy and ontogenetic development of the orofacial area, physiology of speech development, etiology, symptomatology, diagnosis, and possibilities of prevention of myofunctional disorders. The theoretical part also includes an overview of selected therapeutic concepts and part about cooperation with pediatric patients. The practical part demonstrates the use of myofunctional therapeutic concept in the form of case studies of two pediatric patients. A partial goal of the work was create an educational brochure to facilitate self-therapy for parents and their children at home. This bachelor thesis deals with the problem of hypertrophied nasal tonsils known as adenoid vegetation. This is a very common condition amongst children and it is usually operated by surogical removal, in other words by adenotomy. An enlarged nasal tonsil often causes obstruction of the nasal passages that leads to mouth breathing. This incorrect way of breathing leads to changes in the orofacial area, as well as weakening of the respiratory muscles and poor posture. The theoretical part focuses on the correct breathing mechanics, symptoms and treatment of adenoid vegetation and its effect on the postural and respiratory system of children. The aim of this theses was to describe the symptoms and the effect of adenoid vegetation on the postural and respiratory system. The second aim was to map the possibilities of physiotherapy. After adenotomy, there is often no spontaneous recovery of nasal breathing. The defective stereotype needs to be modified and at the same time the defects of the poor posture need to be corrected. The practical part contains a description of the methodology and the course of the research. The research was conducted in a qualitative form and consists of two case studies of children who underwent adenotomy. Their medical history was collected and initial analysis was carried out. Subsequently, individual respiratory and postural therapy was performed. At the end of therapy, an exit kinesiological analysis was made. Finally, the input and output analyses were compared at the end of the practical part. The research showed that postural stabilization and respiratory mechanics improved after the therapy. Adenoid hypertrophy and adenodectomy from the viewpoint of the respirotory physiotherapy

The aim of the thesis is the examination of respiratory and postural changes on examined group of children after adenoidectomy and evaluation, whether it is recommended in post- operative phase to indicate respiratory physiotherapy for the examined children. The theoretical part of the thesis deals with the anatomy, etiology and symptomatology, which accompanies adenoid hypertrophy. Furthermore, there is a description of the kinesiology examination and complete methodology of respiratory physiotherapy and more. In the practical part of the thesis there is a statistic analysis of the examination of 20 children after adenoidectomy and control group investigation. Examination content is the thoracic respiratory excursion (breathing amplitude) and orientational examination of lungs function (PEF, FEV 0,5, FEV 0,75 and FEV1) using digital Peak Flow Meter. The part of the thesis i also kinesiology analysis and questionnaire filled up before adenoidectomy and 3 weeks after operation.

Role of Respiratory Physiotherapy for Patients suffering for adenoid hypertrophy

The bachelor's thesis deals with the adenoid hypertrophy problems. It is divided into a theoretical and a practical section. The theoretical one consists of a general and a special part. The theoretical section summarizes anatomical knowledge related to upper airway, describes some of the respiration physiology aspects and it also deals with the most frequent upper airway pathologies. The special part goes into details about the adenoid hypertrophy problems. It describes symptomatology, diagnostics and therapy possibilities. It widely investigates physiotherapy possibilities, breathing re-education in particular. The practical part contains outputs of a questionnaire research which took place at the ORL children clinic of the 2nd Faculty of Medicine, Charles University. It gathers information about each aspect of the problems. Based on the questionnaire research, an examination and a training of a selected group of patients suffering from adenoid hypertrophy, it compiles a possible training methodology and offers an information brochure to parents
