Stress hyperglycemia during extreme cycling sports

Introduction: This work focuses on the differences in metabolism in patients with type 1 diabetes associated with the stress response and interaction of insulin with epinephrine and cortisol. The main topic is increased hyperglycaemia during and immediately after a stress situation and the possibilities of regulation in this disease. The practical part consists of a case study of a subject with type 1 diabetes mellitus and his state of hyperglycemia during races of extreme cycling disciplines. Objectives: The main aim of this work is to introduce and descript this phenomen in Czech language, to clarify the issues of this phenomen and to conduct a case study to verify results of theoretical part. Methods: Methods that were used in this work includes literature rewiew summerising available materials on this topic and in the practical part a qualitative research method, a case study, examining one subject. Data from platform Diasend were observed and compared and the subject provided an interview. Results: The results of the case study support the findings of the literature review. The highest glucose levels during sport related stress reached up to 20.0 mmol/l despite increased basal and bolus doses. The possibilities of physical exercise in relation to the metabolism after spinal cord injury

The goal of this bachelor thesis is to summarize the information about the possibilities of the physical exercise in individuals with the spinal cord injury. The work presents various types of the physical activity and describes their effect on metabolism and body composition. It also includes the information about the frequency, the duration and the intensity of exercise. The interruption of the somatomotor and somatosensory pathways in patients with spinal cord injury leads to the reduction of their physical activity. As a consequence of that, their metabolic profile and body composition get worse. Early after the injury, the patients suffer from the massive muscle atrophy and the rise of the amount of adipose tissue. These changes have a negative effect on metabolism of lipids and carbohydrates. It results in the development of dyslipidaemia and insulin resistance, sometimes even in type 2 diabetes mellitus. These secondary complications worsen the quality and the length of life of the patients with the spinal cord injury. Better understanding of these issues is necessary to improve the quality of health care for these people. The work includes a short questionnaire. The role adipose tissue in development of insulin resistance and other metabolic disorders in patients with pheochromocytoma

Pheochromocytoma and functional paraganglioma (PPGL) are rare neuroendocrine tumors characterized by catecholamines overproduction, which give a rise to disorders of glucose, lipid, and energy metabolism. The role of adipose tissue in these processes remains unclear. Our aim was to determine the gene expression profile in subcutaneous and visceral adipose tissue of patients with PPGL focusing on endocrine functions of adipose tissue, occurrence of brown (BAT) and beige adipose tissue (BeAT), all in connection with other measured metabolic and energy parameters and levels of circulating adipokines. We demonstrate signs of UCP1-dependent norepinephrine induced thermogenesis connected with overexpression of DIO2 in retroperitoneal VAT of PPGL and higher expression of key transcriptional factors of brown/beige adipogenesis, namely PPARGC1α, CEBPB and PRDM16. However, classic murine BAT or BeAT gene signature in VAT of PPGL was not detected. In subcutaneous adipose tissue (SAT) of PPGL we found signs of possible BeAT transformation, however without simultaneously undergoing UCP1-dependent thermogenesis.
