Midwife in the role of lactation consultant for women

This bachelor thesis explores the topic of the midwife in the role of lactation consultant for women. The thesis is divided into two parts - a theoretical part and a practical part. The theoretical part describes the postpartum period, the production of breast milk, the physiology of lactation, including the composition of breast milk and the benefits that breastfeeding brings to the woman and the baby. Furthermore, the complications associated with breastfeeding, their management and the role of the midwife in lactation counselling are described. The bachelor thesis also discusses lactation counselling in antenatal preparation and the education of lactation counsellors. Last but not least, the theoretical part describes breastfeeding technique, breastfeeding positions and the most commonly used breastfeeding aids. In the practical part of the bachelor thesis, the aim was to find out whether women who have a problem with breastfeeding seek care from a midwife involved in lactation counselling. The second aim was to find out how midwives provide lactation counselling to women. The qualitative research was conducted through semi-structured interviews. The research was conducted from February to March 2024 through telephone calls, video interviews or face-to-face meetings. The research sets were two. The first research set consisted of midwives involved in lactation counseling and the second research set consisted of women who were breastfeeding. The interviews with the informants were conducted on the basis of their consent and were completely anonymous. The collected data were transcribed and then analyzed using text coloring. The main categories and subcategories were established for data analysis and are described in the practical part of the bachelor thesis. For midwives, two main categories (Lactation Counsellor Profession, Lactation Counselling) were established based on the responses obtained, which include six sub-categories: (Lactation Counselling Education, Service Promotion, Most Frequently Addressed Breastfeeding Problems, Form of care provided, Access to Lactation Counselling and Resolution of Emergency Problems). For women, there are three main categories (Breastfeeding information, Starting breastfeeding and Breastfeeding complications). These categories are divided into eight subcategories (Women's information about breastfeeding before delivery, Interest in breastfeeding, First attachment, Education at the postpartum department, Lactation support, Lactation problems, Use of lactation consultant care, and Breastfeeding termination). The first research question looked at the most common issues that lead women to seek care from a midwife for lactation counselling. According to the responses, the most common reasons were feeding the baby with artificial milk and breast problems such as soreness, raga and difficulty attaching the baby to the breast. Informants also reported that women contact lactation counsellors because they need reassurance and a check on breastfeeding technique. The second research question explored how midwives provide lactation counselling. All informants visit women in their home environment to help them with breastfeeding, check correct breastfeeding technique and help women with any complications. Most of the informants noted that they also discuss breastfeeding issues with women in the antenatal or breastfeeding course, where they try to prepare women for breastfeeding.
Education of first-time mothers about breastfeeding

Specifically, it focuses on the education of first-time mothers about breastfeeding, which plays an irreplaceable role in the subsequent course of breastfeeding and the quality of nutrition of the breastfed child. The work is conceived as theoretical - practical. The aim of the theoretical part is to introduce the issue of breastfeeding. It introduces the anatomy of the female breast, the physiology of lactation and the composition of breast milk. It also describes the importance and benefits of breastfeeding, the correct breastfeeding technique and mentions common complications associated with breastfeeding and their solutions. Last but not least, it describes the role of the midwife in caring for a breastfeeding woman, the importance and forms of educating women about breastfeeding and points to the breastfeeding support system in the world. In the practical part, the aim is to map the interest of first-time mothers in lactation counseling services and to find out their satisfaction with the services provided. Meaning of communication in the Lactation Counseling

The topic of this thesis is the importance of communication in lactation counseling. The thesis consists of a theoretical and a research section. At the beginning of the theoretical section, we describe breastfeeding and its importance for both mother and newborn, while other chapters describe lactation physiology, breast milk composition and lactation counseling. The most important part of the theoretical section is definitely the chapter named communication, which is discussed in greater detail. We cannot omit elements of communication, verbal and non-verbal communication and, last but not least, the chapter elements of communication between a healthcare professional and a patient, which emphasizes how to conduct a conversation with a patient to be successful. In the theoretical section, great emphasis is placed on education in lactation counseling and on complications that may occur during breastfeeding. Of course, we cannot leave out proper breastfeeding positions and correct techniques that are necessary for the next stage of breastfeeding, that every lactation advisor should know. Two goals have been set for the research section. The first aim of the work was to map the satisfaction of mothers of sucklings up to one year with lactation counseling. The second goal was to map the level of knowledge and communication lactation counselors have in the field of lactation counseling. According to the first goal the hypothesis was constructed so that there are differences in the satisfaction of mothers with lactation counseling depending on the level of their communication skills. A research question was assigned to the second goal and it focuses on the level of knowledge and communication lactation counselors have in the field of lactation counseling. The research was carried out using qualitative and quantitative research, whereby individual semi-structured and in-depth interviews were used in the qualitative survey. These were conducted with five lactation advisors. The lactation counselors were acquainted with the topic of the bachelor thesis and were assured of maintaining anonymity. Based on data analysis, three main categories with subcategories were set. The results of qualitative research have shown that lactation consultanst have 7 communication skills in lactation counseling at a very good level. The responses of the lactation counselors were matching, independent of each other. Quantitative research was conducted using an anonymous questionnaire whereby we were most interested in the satisfaction of mothers with the lactation counselor. A total of 126 questionnaires were filled in, which were completed electronically between February 2019 and March 2019. A zero hypothesis was chosen and later confirmed by reaserch. It is clear from the results of the quantitative research that more than half of the interviewed mothers would use the services of a lactation advisor in the future. Lactation counselors were also evaluated by mothers who used lactation counseling during pregnancy or after birth. The evaluation of lactation counselors was rather positive than negative, as more than half of the mothers evaluated the lactation consultants positively. The results of the research can be used as material for a better awarness among lactation counselors, as well as for the general public, especially for expectant mothers, but also as a study material for schools.

Problems related to breastfeeding in cleft disorders

In my thesis I describe problems related to breastfeeding in children with cleft disordres. In the first part, I present general knowledge general about breastfeeding and cleft disorders and specific problems that occur in particular cleft defects, and their management. I describe alternative feeding methods, considering advantages and disadvantages, and the possible effect on breastfeeding. Second part deals about the effect of breastfeeding on specific complications in children with cleft disorder, such as otitis media, growth retardation and others. The importance of lactation counselling is mentioned in the last part. The goal of my work is to bring the topic to the awareness of health professionals, to provide relevant information, and make the challenge of breastfeeding children with cleft disorders successful.
Midwife as a lactation adviser

Breastfeeding is the best and, at the same time, the cheapest nutrition for babies. Current trends are in favour of breastfeeding and its support. Support of breastfeeding can be viewed as a preventative way of care, since a mother?s decision against breastfeeding her baby may have material adverse impact on the health of both the baby and the mother. Organisations devoted to support of breastfeeding can be found across the Czech Republic and the whole world. The theoretical part of this paper summarises findings concerning breastfeeding and lactation consultancy.The practical part of it is focused on gathering data from lactation consultants. For the purposes of this paper, I have set 3 objectives and 3 related questions. Objective 1: To map midwifes who provide lactation consultancy in the South Bohemian region. Objective 2: To establish the type of employment relationship under which lactation consultants provide their services. Objective 3: To define specific aspects of a lactation consultant?s job. I have chosen qualitative research as my method and conducted non-standardised interviews consisting of 28 open questions. My research group included 5 midwifes who provide lactation consultancy in the South Bohemian region. The research was performed through March and April 2011. Answers to the questions comprise the output of the research. Research Question 1: Are there enough midwives providing lactation consultancy in the South Bohemian region? Research Question 2: What type of employment relationship do the lactation consultants work under? Research Question 3: What are the specific aspects of a lactation consultant's job? The research questions have been answered. The research indicates that lactation consultants provide this service either as private midwifes or in their free time when off work, whilst employed in hospitals. Lactation consultancy is a specific type of job in that it's fieldwork. I have mapped lactation consultants in the South Bohemian region. 3 of my respondents stated that there was a lack of lactation consultants, however, that, for the time then being, the offer was meeting the demand. Students of midwifery, midwifes, but also nurses who work at hospital puerperium units can find this paper useful. It provides findings which may help to improve this type of work and may be useful for midwives who are interested in providing lactation consultancy.
Lactation consultants in the District of Jindřichův Hradec

The title of thesis is Lactational Consultancy in the district of Jindřichův Hradec and this is also the main topic of the thesis ? the meaning of the term and the general principles of becoming a good consultant in the field of brestfeeding. Lactational consultancy is supported by the lactational league and that is why this organization is mentioned in the thesis; it is an independent and non profit-making organization but it is the most important one in the field of this consultancy. The thesis contains also lactational physiology such as anatomy of breast glands and the influence of human hormones on the glans. It also contains the description of mother´s correct nutrition because it is of the same importance in the lactational period. Further chapters are about breastfeeding ? preparation, advantages both for the infant and the mother, the procedure of the first giving the breast to the infant. The following part of the thesis introduces the breastfeeding technique which describes general principles of giving the breast to the infant. The next chapter is about positions used while breastfeeding. But breastfeeding is not only the matter of advantages, principles and positions while breastfeeding. Therefore there is also chapter about breast care in the lactational period. The thesis describes the correct breast hygiene, care after each breastfeeding and also how to massage the breasts. In the course of breastfeeding many various problems may occur; the most important ones are mentioned in the last part but one of the theoretical section of the thesis. Two objectives were set up at the beginning of the thesis, and two hypotheses. One objective was focussed to the level of acquaintance with lactational consultancy among women, the other on the level interest in the service. Hypothesis No. 1 was that women know about the possibility of lactational consultancy and this hypothesis was well supported in the course of us research; thus the objective was completed successfully. The other hypothesis was that women are interested in using lactationaly consultancy. On the basis of the same principle this hypothesis was supported and the objective No. 2 completed. All the data were archieved by anonymous questionnaires which had been submitted to mothers in lactational period at local gynaecologists in policlinics and to pregnant women attending course preparing for birthgiving. The main objective was collect data for founidng a lactational centre or non-stop telephone line in the district of Jindřichův Hradec where such a service is badly missing.
