Possibilities of spa treatment in patients with Bechterew's disease

The bachelor thesis deals with possibilities and influence of spa treatment on patients with Bechterew's disease. The work consists of the theoretical and research part. Slatinné lázně Třeboň s.r.o. was selected to find out all the possibilities of spa treatment and a more detailed description of individual procedures, in which the research part was performed. The theoretical part describes the anatomical structures, kinesiology of the axial system, knowledge about Bechterew's disease, definition of the disease, clinical symptoms, examination and diagnosis itself. Subsequently, all treatment options for patients with Bechterew's disease are listed. The research part contains the description and effects of all spa procedures that are prescribed to patients with Bechterew's disease. There are also case reports of specific patients with the above mentioned disease and their course of physiotherapy. The aim of the work was to conduct a survey of the possibilities of spa treatment, to find out the effect of treatment. A qualitative form of research was used to achieve the set goals. All the important information have been obtained in cooperation with spa health professionals as well as due to a kinesiological analysis and detailed interviews with specific patients.
The effect of spa treatment on changes of anthropometric and biochemical parameters in obese pediatric patients

Childhood obesity is currently considered a serious pediatric problem. The main risk of this metabolic disease lies in the shift of comorbidities associated with obesity to ever younger age categories. This is also related to the increasing overall cardiometabolic risk in these obese individuals. Early detection and treatment of childhood obesity is an essential task of pediatrics. The possibility of non-pharmacological treatment of childhood obesity is spa treatment. It consists of intensive reduction therapy, which includes nutritional, physical and educational interventions. The aim of the study was to monitor changes in anthropometric and biochemical parameters after monthly treatment in pediatric patients. Furthermore, to identify and specify selected potential markers of cardiometabolic risk that significantly correlate with BMI and could be useful in children and adolescents. The thesis also focused in detail on two adipocyte hormones - leptin and adiponectin, which already correlate with the amount of adipose tissue in the pediatric population. These hormones could become significant predictive parameters of metabolic syndrome, subclinical atherosclerosis and other comorbidities associated with obesity and cardiovascular risk, already in children. Spa treatment of patients with metabolic syndrome

This diploma thesis deals mainly with patients with diagnosed metabolic syndrome disease in connection with the importance of spa treatment in Mariánské Lázně. The objectives of this work are: to find the most commonly used spa method for patients with metabolic syndrome, to find out the current lifestyle of patients with metabolic syndrome and to find out whether the health condition of patients who have undergone spa treatement was improved during their spa stay. A qualitative survey method was used to achieve the objectives. Data collection was conducted through interviews of patients with metabolic syndrome and visitors to Parkhotel Golf a.s., Mariánské Lázně. I consider the increased consumption of white bread, alcohol and cigarettes in some respondents to be the fundamental findings of my research. Most respondents do not consume vegetables in sufficient quantity, and deficiencies are also evident in dedicating to sporting activities. Information about the metabolic syndrome itself or the complications of the individual components of this syndrome is sufficient. In connection with metabolic syndrome, it was found out that not all patients who underwent spa treatment want to change their lifestyle after returning from the spa facility. Considering the findings from spa patients, I would recommend to increase the education of patients with metabolic syndrome, to emohasize to them the importance of healthy nutrition, smoking abstinence, or enough movement as the basis for preventing this disease. A lecture which has been created on the basis of interview information and is part of the supplement to this work can be used as means for patients with metabolic syndrome.
The effect of spa treatment in obese school-aged kids

Title: Impact of spa stay on obesity of older school-aged children. Purpose: Principal purpose of this thesis is to find out and assess the impact of spa stay on obesity of older school-aged children. Methodology: 100 children in the age of 11-15 participated in this inquiry (49 boys, 51 girls, average age 13 years). The children went through the 28-day long reduction treatment in spa facility. At the beginning and end of the therapy the following criteria were assessed: body weight, BMI, volume of body fat, volume of muscle mass, abdominal circumference and tests of agility, endurance and strength (Jacik's motoric test and test of walking for 2 km). During the 4-day treatment the children had the adjusted diet to 5000 KJ, respectively 8000 KJ (by age) and they participated every weekday in group exercise classes and guided walk. Results: The outcomes showed that the spa treatment favourably affected all observed criteria. The decrease of the following criteria of the respondents is statistically significant: body weight decrease (on average by 5,76 kg), decrease of volume of fat tissue (on average 3,36 %) and decrease of abdominal circumference (on average 5,62 cm). Basic theoretical background: Because this thesis mainly deals with the awareness of clients, it was necessary in the beginning to mention the importance of the communication in a medical environment. Communication takes place between two or more people. During communication, people communicate necessary information, feelings, attitudes, talking about relationships and moods. There are several communication rules that one should follow, however, it depends on the individual, whether follow it or not. Communication skills can be developed gradually for they are important to us and in our personal lives. If a person loses the ability to communicate, it can lead to danger to his relationships with others. For healthcare professionals, it is very important social skill ( Mikuláštík 2003). Main aim of the thesis: To verify the information rate of clients in preoperative phase of total hip arthroplasty and in postoperative care. Discover also whether patients after total endoprosthesis have possibility of aftercare (rehabilitation, spa treatment). From these targets were developed following questions : In what areas are the clients in preoperative stage informed? How are clients reporting in a post-operative care ? What options of aftercare patients after total endoprosthesis have? Methodology: For the research was used the qualitative survey data collection. A qualitative investigation was approved by head nurse of the hospital, where the research has been done. The interviews was realized at home of clients. Survey was conducted through semi-structured interviews, interviews were anonymous. The data were collected in March 2015 in the set of seven respondents. Despite the fact that interviews were realized in the clients' home, we needed approval for the research from the head nurse. Especially because the subsequent care for patients after total prosthesis was also investigated. It was therefore important to identify initially the applicable patients in the hospital and up to a certain time after the surgery to interview them.
Impact of the change in providing spa care according to the ordinance 267/2012 to the physiotherapy conception in the spa surrounding

My bachelor thesis deals with problems, that occurred after the cancellation of the Decree No. 58/1997 Coll. and the adoption of the Decree No. 267/2012 Coll. establishing a new indicator list for the Spa sanatorium rehabilitation care for adults, children and adolescents, when there have been significant changes in the area of Spa care. Significant changes occurred, for example in the length of stay in treatment, that was shortened to 14 days or 21 days and to changes in the overall indication of the spa treatment, when some disease it was not possible to draw a spa care at all. This resulted in serious problems for both the spa facilities, as well as for patiens. Physiotherapy as an important part of balneotherapy has a very close relationship to the overall concept of the Spa treatement and its effect depends to a large extent on the presence and frequency of other treatments. The first part of the theoretical work deals with a spa treatment, legislation of Spa care and impact which has occurred after the adoption of Decree No. 267/2012 Coll. The second theoretical part deals with the therapeutic methods in the Spa. The aim of the work was charting of physiotherapeutic procedures in the context of Spa care pursuant to decree 58/1997 Coll. and 267/2012 Coll.. For the processing of a qualitative research strategy was used the method of the personal case study. Research file consisted of three patients who have completed the three-week comprehensive spa treatment in Bertiny lázně in Třeboň. One patient was treated with rheumatoid arthritis and two patients with Bechterew's disease. All the listed patients visited the spa facilities regularly and repeatedly. For these respondents was conducted the subscription history, input and output kinesiological analysis. Input and output of kinesiological analysis contained the examination of aspects, palpation, examination of walk, antropometric examination, goniometric examination and examination of distance on the spine. Six individual therapies was done and a subjective evaluation of the results of therapy at the end. Physiotherapy treatment was evaluated according to the above regulations. For these three treated patients was noted a difference in the effect of the spa treatment in the three-week and the four-week stay. Extended stay brought the longer-term pain relief, improve mobility and overall had a long-term effect, thanks to the greater number of balneotherapy treatment and the greater number of physiotherapy. It also had a better effect on the psyche of the patient when the patient could rest during the longer stay and managed to learn exercises and consolidate new friendship. The good news is the adoption of the new law, when from 2015 the length of the Spa stay was set back to four weeks. In my bachelor thesis I'd like to point out the problems that caused the adoption of Decree No. 267/2012 Coll. Furthermore, public awareness on this issue could be improved in this way.
SPECIFICS OF PHYSIOTHERAPY OF PATIENTS TRANSFERRED AFTER ORTHOPEDIC OPERATIONS DIRECTLY ON THE SO-CALLED SPA BUNK

This bachelor thesis addresses the specificity of physiotherapy of patients, transferred to a so-called spa bed, after orthopedic surgery. Among the most performed orthopedic surgeries are the total hip replacement and the total knee replacement. Arthrosis is the most common reason for the implantation of joint replacements. It is a painful and life-aggravating disease which has a tremendous impact on the patient's quality of life. In order to lower the risk of the patient having to take a revision surgery of joints, a promptly initiated rehabilitation is necessary. It has to be intensive and professionally conducted. The present time-tested trend of rehabilitation treatment is a prompt comprehensive care, which takes place in a spa resort in the form of a direct transfer "from one bed to another", i.e. the patient is transported directly from the hospital to a spa resort. Often, patients would get to a spa resort with delay, while already having intercurrent problems like vertebrogenic and other problems. This thesis is divided into a theoretical part and a practical part. The theoretical part includes the anatomy of the hip joint and the knee joint, the description of the joint replacements including the indications and the contraindications, the specifications of particular types of hip and knee joint replacements and possible complications after surgery. The most important topics of the theoretical part are stated in the last chapters. The individual therapeutic rehabilitation methods, which have a positive impact on the health status of the patient after surgery, spa therapy and physical therapy included, are discussed and described in those chapters in detail. A qualitative research strategy and a personal case study technique (case history), with the use of a secondary data analysis, were used in the practical part. This strategy was carried out on three chosen patients, who after being discharged from a hospital after surgery had continued their treatment in a spa resort, i.e. a so-called direct transfer from a hospital bed to a spa. The research was conducted in Berta's spa resort in Třeboň, where the patients were transferred on the tenth day after surgery. On their first therapy session, all patients had their personal history taken and data from their medical documents was added. The practical part also includes a pre-therapy and a post-therapy kinesiologic analysis which included visual inspection, palpation, goniometric and anthropometric examination of the knee and hip joint area. Furthermore, a muscle test and an examination of shortened muscle groups related to the knees and hip joints, created by Janda, were conducted. The gait, stereotypes and neurologic functions were also examined. Following the evaluation of the pre-therapy data, the patients were introduced to the short-term therapeutic plan. The results were acquired by comparing the pre-therapy data and the data which was obtained during the last therapy session from the post-therapy kinesiologic analysis. The rehabilitation in the spa resort consisted of different treatment procedures each day and the patients were getting medical attention and care from doctors, nurses and physiotherapists. In each therapy, we focused on removing the problems at hand and only after that we moved to correcting other imperfections. After the completion of the therapy, the patients stated a decline of pain and an improved balance of the extremity and the whole body. The data of the chosen patients shows improvement of muscle strength, decreasing of swelling and widening of range of motion. These results were achieved thanks to a comprehensive rehabilitation and a positive attitude of the patients.
Effect of Spa Therapy on Quality of Life and Consumption of Drugs in Patients Aged 35-50 Years with Musculoskeletal Disease.

This bachelor thesis titled ?The Effects of Spa Treatment on the Quality of Life and the Consumption of Drugs in Patients Aged 35 to 50 Years with Musculoskeletal Disease? deals, in the first chapters of the theoretical part, with the history and current status of balneology, individual spa treatments, and the importance of spa treatments in treating musculoskeletal diseases. The other chapters deal with health, the quality of life, its measurement and evaluation. Within the thesis, two goals were set: (a) to find out the effects of spa treatment on the quality of life and (b) to monitor the effects of spa treatment on the consumption of drugs in patients aged 35 to 50 years with musculoskeletal disease who were clients of Bertiny lázně in Třeboň. For fulfil the objectives of the thesis, the method interviewing was applied using standardized questionnaire SF-36 about the health related quality of life, which is suitable for finding out about the influence of spa therapy on the quality of life and health state of spa treatment clients. The respondents were examined prior their spa treatment, i.e. immediately after coming to Bertiny lázně in Třeboň, and then 3 months after the spa treatment, using the method of ?postal survey?. The results of this thesis show that the health state of patients with musculoskeletal disease improved after completing spa treatment, most distinctly in the domain of pain relief (30%) and in the vitality domain (26%). The consumption of drugs saw only minimal changes. Based on the results obtained, we can clearly say that spa treatment contributes to the improvement of the quality of life of patients with musculoskeletal disease and it also clearly points to the effectiveness of finance invested in balneology.
Influence of spa treatment on a patient with Bechterev disease from a biopsychosocial point of view

Chronic disease brings some deterioration of the quality of life which is reflected in physical, emotional, as well as in social spheres. From this point of view is balneotherapy bio-psycho-social model of intervention. The goal of theses was to documentate and to prove positive bio-psycho-social effects of spa treatment on 20 patients suffering from Bechterew disease , while using objective and subjective measuring parameters.
