Role of healthcare assistants and nurses in the nursing process.
VODIČKOVÁ, Petra
This thesis treats the issues regarding the role of healthcare assistants in the nursing process and their mutual cooperation with general nurses. Four goals were set. The first goal was to ascertain in which way the general nurses allow the healthcare assistants to engage in the nursing process. The second goal was to ascertain what are the healthcare assistants? opportunities to participate in the nursing process. The third goal was to determine what problems are seen by managers regarding the integration of the healthcare assistants in the nursing care system. The last, fourth goal, tried to ascertain whether the system of integration of general nurses and healthcare assistants in the nursing team contributes to improvement of the patient?s care. The thesis is divided into two parts: theoretical and practical one. The theoretical part deals with the nursing process as a method of work of a nurse, qualification training of general nurses and healthcare assistants and possibilities of finding vacancies in the nursing practice. The practical part consists of quantitative-qualitative research survey. For the qualitative research a method of individually conducted, pre-prepared interviews with representatives of the above mentioned professions. Four research questions were determined, i.e.: 1. To what extent are the healthcare assistants, general nurses and managers in positions of ward sisters aware of the nursing process? 2. Have the healthcare assistants a possibility to use their knowledge of the nursing process pursuant to Act No. 96/2004 Coll.? 3. Is the cooperation of healthcare assistants and general nurses perceived as a positive element for improvement of nursing care? 4. What experience have managers in the position of ward sisters in integration of healthcare assistants into the nursing team? The surveyed group consisted of six healthcare assistants, six general nurses and eight ward sisters-managers from minor inpatient facilities in the Pilsen region and Cental Bohemian region. Qualitative research was realized through a survey by questionnaire. Students of the last grade of the study field ?Healthcare assistant? and the study programme ?General nurse? were addressed. For the qualitative part of the research survey the following hypotheses were set: 1. Students of the study field ?Healthcare assistant? are oriented in the competences of healthcare assistants and general nurses in accordance with At No. 96/2004 Coll. 2. Students of the study programme ?General nurse? are oriented in the competences of healthcare assistants and general nurses in accordance with At No. 96/2004 Coll. 3. Students of the study field ?Healthcare assistant? perceive the determined competences for individual fields as a tool for quality professional care. 4. Students of the study programme ?General nurse? perceive the determined competences for individual fields as a tool for quality professional care. In total, 200 questionnaires were distributed, and subsequently, 142 of correctly completed questionnaires were evaluated. The output of this thesis is the verification of the present status of the issue in question. The results will be provided to nursing care managers in the researched healthcare facilities where they could help to improve the management of healthcare activities. The results may be also used to prepare informational and educational sessions clarifying the rules of cooperation of healthcare assistants and general nurses. Furthermore, the results could be offered to the Ministry of Health of the Czech Republic, namely to the Working Committee for transformation of non-medical healthcare professions in the Czech Republic the main work objective of which is the amendment of Act No. 96/2004 Coll.
The awareness of nurses working in anestesiology-resuscitation wards and intensive care units about competences determined in regulation No. 424/2004 Coll.
MARTÍNKOVÁ, Petra
This paper investigates the competences of nurses on staff of the anesthesiology & resuscitation departments and the intensive care units as such competences are defined in Decree 424/2004 Coll. The paper has been split into two sections: theoretical and practical. The theoretical section looks into the roles and competencies assigned to the non-specialized nurses and to those who have completed a specialism course, specifically the "intensive-care general nurses", and describes the kinds of patients hospitalized at the mentioned departments. The paper aimed (1) to examine the extent to which the nurses were informed on their competencies following from Decree 424/2004 Coll. and (2) to find out how consistent the nurses were in adhering to what the Decree required. A questionnaire-based survey was conducted at the Brno Faculty Hospital, the Pilsen-Lochotín Faculty Hospital, and the St. Anne Faculty Hospital in Brno; moreover, the questionnaires were distributed among the nurses also using an internet social network. At the early stage of the work the authors formulated two hypotheses: (1) the nurses are not familiar with all their competencies; and (2) the nurses fail to observe the competencies following from the Decree specified above. With the questionnaires answered and the answers evaluated, both the hypotheses proved to be true. The nurses were found not familiar with all their competencies and not performing all that the Decree allowed. We believe the paper could be instrumental in enhancing the vocational education of nurses and, consequently, in could improve the quality of nursing care provided at the anesthesiology & resuscitation departments and the intensive care units.

Cooperation between nurses and physicians from the perspective of nursing management
KADLECOVÁ, Ivana
Cooperation of nurses and physicians from the perspective of nursing management Master's thesis was focused on the cooperation of nurses and physicians from the perspective of nursing management. Nursing care and nursing management has an irreplaceable role in health care. The world of medicine, nursing care and health care is an extremely complex organizational system, which is not surely represented only by physicians and nurses, but these, however, are still the dominant persons on the scene. The first objective of this thesis was to make a survey of cooperation between nurses and physicians during the nursing process in the care of patients from the perspective of nursing management in departments of surgery and internal medicine. The second objective was to identify problem areas in cooperation between nurses and physicians from the perspective of nursing management in departments of surgery and internal medicine, and the third objective was to find out the way the nursing management tackles problem situations in cooperation between nurses and physicians in departments of surgery and internal medicine. To meet these objectives four research questions were stated: 1. In what areas do nurses and physicians cooperate during the nursing process while providing care for patients from the perspective of nursing management in their respective departments? 2. How do nurses assess and perceive cooperation with physicians from the perspective of nursing management in their respective departments? 3. Are there problem areas in cooperation of nurses and physicians from the perspective of nursing management in their respective departments? 4. How does nursing management tackle problem situations in cooperation between nurses and physicians in their respective departments? The research was conducted in the hospital, where I was given information under the condition of anonymity of the respondents interviewed. The responders were chief nurses and senior staff nurses working in departments of surgery and internal medicine. The investigation conducted is a qualitative research. Interviews with the structure of questions to non-standardized interviews were conducted with the respondents according to question structures of the non-standardized interview. Subsequently, transcripts of interviews and data categorization in tables according to the Ritchie and Spencer´s framework analysis were made. The survey suggests the following conclusions. Nurses and physicians cooperate within the nursing process in providing care for patients from the perspective of nursing management in departments of surgery and internal medicine at the stage of problem solving. Nurses appreciate cooperation with physicians on the basis of mutual respect, but they still feel the prevailing dominance of physicians. From the perspective of nursing management there are problematic areas in cooperation between nurses and physicians. Nursing management tackles problem situation in cooperation between nurses and physicians by seeking common solutions. It will be possible to provide the health care facilities, where the research was conducted, with the outcomes of the research. Thinking about the possible improvement in mutual cooperation as early as during physicians´ and nurses´ studies, to introduce various measures to improve cooperation between nurses and physicians by organizing joint seminars and cultural events could lead to meeting the purpose. It is necessary to raise physicians´ awareness of nursing process and to carry out further research on this topic.
Safe Nursing Care
HAVELKOVÁ, Hana
Patients{\crq} safety is a global problem and we are increasingly thinking about the possibilities of improving the quality of care provided. This is a priority for all health care providers, including nurses. We must not forget the patients and their close family because they are our partners and the nurses should seek to ensure that the patients themselves are actively involved in the care of their health and play an important role in preventing some unwanted errors. The theoretical part of the thesis is devoted to a detailed analysis of the concept of safety and safe care and the associated negatives. The goal of the thesis was to determine which resources are used to maintain the level of safe nursing care by nurses with specialized training. Six hypotheses were determined. The quantitative survey technique was used to achieve the objectives of this thesis and to verify the hypotheses. The group consisted of nurses from all over the Czech Republic in the month of February 2010 studying on a specialized training course of the NCO/ NZO (National Center of Nursing/ Non- medical health disciplines) in Brno. The results imply that the nurses interviewed do not use enough resources to help reduce the risk of errors leading to an increase in the level of safe nursing care. The proposed measures are intended to increase the use of resources for maintaining the level of safe nursing care which is a clearly dependent on the awareness of their existence. This also strongly urges management to make more use of various resources and inform all medical staff not only nurses about new procedures and analyses carried out on a regular basis. Managers should conduct a regular and detailed analysis of past errors or incidents to establish the risk of error and take steps to minimize such unwanted events. The results of the survey will be provided to the professional public, published in professional journals and can serve as a basis for comparing other survey research.
Current situation and educational prospects of paramedics
KLINGEROVÁ, Helena
Current situation and educational prospects of paramedics This Bachelor paper endeavors to investigate the current situation and educational prospects of paramedics, i.e. medically trained rescuers. This topic has recently been much discussed, since a great number of young people are being trained and will be trained to become paramedics, and they should have a clear idea of what the qualification they wish to acquire or have already acquired can bring them. The theoretical section of the paper addresses both the historical development and the current situation encountered in the emergency pre-hospital care as provided in the Czech Republic, with the focus on the Emergency Medical Service of the South Bohemian Region (ZZS JčK), whose staff members constituted the surveyed sample. The theory of the subject gives a legal definition of paramedics and specifies their options of professional training & education. The paper aimed to map the employment possibilities of paramedics at ZZS JčK and to find out what the ZZS JčK staff thought about the prospects of this profession. The survey relied on the method of qualitative research based on the questionnaire technique. As follows from the results obtained, all the paramedics being trained cannot currently find a job with the Medical Emergency Service, but in case an Act on Emergency Medical Service is passed, new Emergency Centers will crop up and more paramedics will be needed. As regards the prospects of career for the paramedics, the employees of the Medical Emergency Service are optimistic, because in case the Service cannot offer them a job, they will be able to find employment elsewhere. Thus the hypothesis envisaging that with the number of trained paramedics growing their prospects of finding a job with the Service will diminish proved partly true. The second hypothesis expecting that the paramedic qualification brings better prospects for males than for females was not confirmed as true. Other results of the survey lead me to recommend that the paramedic training takes on a different form and different length, since most respondents believed the paramedics, on leaving the vocational training, were not completely ready for the field work.
Ethical Problems and Dilemmas in the Work of Officials at the Social Departments of Municipalities
LUDÍNOVÁ, Ivanka Anna
As a subject-matter of my bachelor's thesis I chose "Ethic issues and dilemmas upon the public officers' work within Social Services and Welfare Departments at Local Authority Offices of Czech municipalities", because I had considered the theme to be more than actual since such topics appear within the public administration activities all the time and each officer is then forced to sort these dilemmas and issues out as a part of their everyday work. Solving such problems is quite demanding job not only for the officers themselves, but also within all areas of human life. The bachelor's thesis was divided into two parts. As for the theoretical chapter, I defined key terms and definitions concerning the field of ethics by which the decision-making process of each officer is affected significantly, and which were essential for the research pursuit. I also dealt with the ethical theories which influence the social work a lot, the ethic codes were included as well as their functions together with their utility for public officers. I addressed to particular ethic issues and dilemmas connected with the social work in the following chapters. As for the elaboration of the research chapter of this bachelor's thesis and the accomplishment of stipulated objectives, a quantitative observation ? questioning survey ? was used. The original aim of my bachelor's thesis had been to detect all needs of relevant officers in the city of Pilsen regarding their further education within the area of professional ethics, and to find out whether they have to resolve such ethic issues and dilemmas upon their deciding. Based on the specified objectives, two hypotheses were formulated. The research section described the initial goals of investigation, methodology and techniques, which I had chosen for gathering the required information, while I described the search sample and the execution of data acquisition, too. Furthermore, I focused on the data interpretation and last but not least the discussion on the interviewing results. The intention of my thesis was fulfilled: the first assumption could not have been confirmed at all while the second assumption had been fully proved to be true. This approach can be useful upon preparation of further education for public officers within the social sphere, especially in connection with their abilities to cope with ethic-related troubles and situations which the officers in question have to face every single day.
Midwifery home visiting from the point of view of the public
DUŠKOVÁ, Marcela
Due to the Czech Republic's health care system, the need for medical care during pregnancy - even when it is progressing physiologically - has become deeply-rooted in the mind of the public. The theoretical part of this work deals with midwifery in relation to community care. The general theoretical section sums up the roles and competencies of midwives and the forms of their education, both qualifying and lifelong. The following section deals with actual procedures in the care provided by midwives during home visits, according to valid standards issued by CKPA (Czech Midwife Confederation). The questionnaire method, one of quantitative research methods, has been selected for the research part of the work. The questionnaire used consisted of 28 questions and has been processed during April 2009. The sample group was a large group of non-professionals with no limitations by age, gender or place of residence. A total of 150 respondents have been asked for cooperation, and 126 filled questionaries were returned to the researchers. The main goals of the work were determining the public opinion on home visiting by midwives and its necessity. Futhermore, determining the public knowledge of midwife competencies and awareness of the range of midwife services covered by health insurance. The following hypotheses have been examined by the researchers: Hypothesis 1: Expectant mothers prefer gynecologist's care to midwifery. This hypothesis has been confirmed. Hypothesis 2: Women are not interested in home visiting by midwife after the birth. This hypothesis has been refuted. Hypothesis 3: Most of all, women expect high-quality prenatal courses from a midwife. This hypothesis has been refuted as well. Hypothesis 4: Men do not consider home visiting by midwife as valuable for women. This hypothesis has not been confirmed. Hypothesis 5: Women with no access to medical care in their place of residence are in greater need of home visiting by midwife than women with such access. Because of the non-representative distribution of respondents in the sample group, this hypothesis has been neither confirmed nor refuted. Hypothesis 6: The public has no knowledge of the actual competencies of midwives. This hypothesis has been confirmed . Hypothesis 7: The public has no knowledge of the range of midwife care covered by health insurance. This hypothesis has been confirmed as well. This work is of informative nature. It sums up the opinions and knowledge of the public about midwifery. It also helps to outline the uncertainties of the public in this field. The work can be used for public education or as a starting point for future research.
