The main aim of this bachelor´s thesis is to describe the treatment-rehabilitation procedure of patients after cardiac surgery complicated by stroke. In the theoretical part, I describe cardiac surgery in general, operative approaches, complications related to cardiac surgery, including stroke. I also focus on the rehabilitation of patients after heart surgery in a hospital facility. The second half of the theoretical part is dedicated ischemic and haemorrhagic strokes, symptoms, methods of treatment. At the very end, the thesis is dedicated to the rehabilitation of patients after a stroke. The practical part is processed in the form of qualitative research. Preoperative physiotherapy in patients with planned cardiac surgery

The bachelor thesis investigates preoperative physiotherapy options for patients with upcoming cardiac surgery. The thesis examines physiotherapeutic options before the cardiac surgery, which aim to be beneficial in the postoperative phase. The first goal is to map physiotherapeutic methods available for patients with upcoming cardiac surgery. The second goal is to compile educational material for patients who are planning to undergo a cardiac operation. I formulate two following research questions to achieve these goals: 1. Which physiotherapeutic methods are available for patients with upcoming cardiac surgery? 2. What effect has the educational material for patients with upcoming cardiac surgery? The theoretical part focuses on the following areas: anatomy of thorax and heart, physiology of bloodstream and heart, and on respiratory system. The next topic is a description of basic concepts of cardiac surgery and physiotherapeutic methods, which we can use in cardiac rehabilitation. The practical part of the work is based on qualitative research - case studies of four probands with planned cardiac surgery. Before the surgery, the input examination was performed to set starting values. The output examination took place right before the patient's release to home care for final evaluation. Probands were found thanks to the department of cardiac and chest surgery in České Budějovice hospital. The research was held during proband's hospitalization, and the whole therapy process was monitored. Based on kinesiological analysis input and output, I evaluated the effect of preoperative physiotherapy as positive for three out of four probands. Proband 1 had a significant extension of maximal respiratory mouth pressures; her thorax increased its volume and duration of inhaling and exhale. Despite lowered expectations, Proband 2 improved her maximal inspiratory and expiratory mouth pressures. Chest volume of Proband 3 increased during measurements. Due to the health condition of Proband 4, I could not perform her output examination.

Lenka Chvojková Title of bachelor thesis: Physiotherapy in patients with long-term heart assist devices Abstract: This thesis deals with the issue of physiotherapy in patients after the implantation of long-term heart assist devices (in this case of the type HeartMate 3). The goal of this thesis is to find the possibilities of physiotherapy within cardiovascular rehabilitation after surgical implantation of heart assist device and subsequent application of this knowledge in practice. The thesis is divided into theoretical and practical parts. The theoretical part consists of basic information about the heart muscle and heart failure as the most common cause of implantation of the long-term heart assist device. Next, both heart assist devices in general and concretely HeartMate 3 are presented. Finally, there is a chapter devoted to cardiovascular rehabilitation. The practical part was created based on professional practice in the IKEM hospital in Prague at the turn of July and August 2020. There is an elaboration of case studies of two probands using functional testing such as the 2 Minute Walk Test and Barthel Index. Physiotherapy in patients after cardiac interventions

In my thesis I tried to focus on the issue of physiotherapy and curative rehabilitation by the patients after cardio-surgical interventions during their stay in a hospital facility. In the opening part I am presenting a brief overview of used cardio-surgery techniques and accesses with their short evaluation. I dealt in brief with the issue of wound healing and the scar formation, including a summary of the most important post-operation complications and accesses, which enable to avoid these complications. One of partial tasks solved in my thesis was the search of physiotherapeutic techniques after the heart operations. Here I focused especially on modern and recently used strategies. I adduce their short definition and explanation of possible benefits resulting from their use. In the second part of my work I am directly evaluating the progress of physiotherapy of two patients with the identical diagnosis who were operated on by two different techniques. Both the patients were taken to the planned performance ? the exchange of the aortic valve as a consequence of their stenosis and present insufficiency at the cardio-surgical department of the hospital in České Budějovice. In one case the classical medial sternotomy was performed, in the other case it was decided to perform a mini-invasive intervention ? the right-side mini-thoracotomy. With both the observed patients the same exercise was practiced just from the first day of the physiotherapy. During the whole period the values of vital functions were carefully checked and registered. The physical and mental state of patients was also followed including the painfulness of the wound. I collected the data for my thesis by means of interviews with the physiotherapist and the patients participated in the observation. I also elaborated a detailed casuistic and analysis of patients? personal data. I cooperated actively with the present staff. The exercises were done twice a day, in the morning and in the afternoon. The whole rehabilitation unit lasted approximately 15 minutes each time. The exercise was started in the bed where simple mobilization exercises were done, some elements of the breath and vascular gymnastics and elements of Vojta?s reflexive locomotion. Gradual verticalization of the patient enabled us to use even more complicated exercises. The exercise was finished with a walk in the corridor and on the stairs. Some respiration aids were used, described in the following part. From the rehabilitation process, which was in progress on both the patients immediately from the first rehabilitation day, results clearly that the patient with medial sternotomy (a woman) showed a considerably greater painfulness every day, which was very limiting for her. Even her mental condition was worse. This patient was not able to perform with a delay of several days those exercises, practiced by the patient with the mini-invasive entry already without any troubles. Considerable differences were also found in the walking, both in the distance and in posture while walking.
