Glycemic level check performed by a nurse in critically sick people in intensive care

This thesis deals with the control of blood glucose levels of critically ill patients in intensive care, focusing on hyperglycemia of the critically ill and possible therapy within the competences of nursing and medical staff. The theoretical part of the thesis deals with the issue of hyperglycemia of critically ill patients from the medical and nursing perspective as well as the issues concerning instrumentation. The first chapter describes briefly the history of insulin, relating to its discovery and introduction into practice. The following chapter describes the concepts of critically ill, intensive medicine, intensive care as well as specific nursing care. Short information about etiology, diagnosis, treatment and complications of diabetes mellitus is described in the third chapter. The following are the chapters, which are more specifically devoted to the issue of hyperglycemia of the critically ill, including chapters on glucose homeostasis of the critically ill, the evolution of blood glucose levels, instrument technique, insulin protocols, insulin therapy, details of insulin therapy in intensive care and on the principles of correct blood collection for the determination of blood glucose levels in the conditions of intensive care. Factors influencing the compensation of type 1 diabetes mellitus in pregnant women

Introduction: The diploma thesis deals with the issue of compensation of type 1 diabetes mellitus during pregnancy and its relation to the health of mother and child. It further summarizes and describes the variable factors that may have a direct effect on current glycemic variability and long- term compensation of diabetes. Aims: The theoretical part describes the characteristics of type 1 diabetes mellitus and the possibilities of therapy in pregnancy. Subsequently, factors influencing the compensation of diabetes in this period are summarized and described in more detail, especially food composition and possibilities of influencing postprandial glycemia, effects of type and intensity of physical activity and psychosocial aspects related mainly to stress from potential negative effects of glycaemia on the fetus. At the end of the theoretical part, the risks and complications for the mother and the fetus arising from long-term unsatisfactory compensation of diabetes are presented. Flexible dosage of insulin and a diet

This bachelor thesis describes diabetes mellitus and the possibility of introducing a new flexible treatment of insulin therapy in people with intensive insulin regimen. The theoretical part is intended to describe diabetes mellitus, causes of illness, treatment, principles of proper eating, the relationship between carbohydrate metabolism and insulin hormone and also discuss flexible dosage of insulin, history and ways of flexible treatment. The practical part of the work was aimed at monitoring the effect of regular patient education on the transition from intensive to flexible dosing of insulin. Within six weeks, patients attended educational meetings where they tried to learn how to work with the new regime. Results from glucometers and changes in glycated haemoglobin values were then used to evaluate the underlying research. Research has made it possible to determine to what extent flexible insulin dosing and proper diet can positively influence diabetes compensation, as well as the effect of motivation and regular patient education on final results. The follow-up patients showed great willingness and motivation to work together and adopt new insulin dosage regime. Insulin Therapy from A Nursing Perspective

Diabetes mellitus is a serious and incurable disease prone to specific complications, known since 1550 BC. Recent international studies have shown that approximately 347 million people suffer from diabetes and the number is still increasing. Diabetes is treated by taking dietary measures; in addition to diet and oral antidiabetic medication, exogenous insulin intake is the ultimate treatment option. It was the exogenous insulin administration in 1922 that brought up a revolution in the treatment of diabetes mellitus. Since then safer and user-friendlier insulins have been and still are developed. Insulin is stored in liquid form in vials or cartridges and is only administered intravenously; however, it is assumed that an equally effective application method will be found in the future. The aim of the thesis is to map out the specifics of nursing care of insulin therapy clients and to answer the following research question: ?What are the specific needs of clients with insulin therapy?? ?How is the nurse involved in the education of the client in terms of insulin therapy?? ?Do insulin therapy patients have enough information about diabetes mellitus and insulin therapy?? The thesis consists of a theoretical and research part. The theoretical part deals with information about diabetes mellitus, insulin, insulin therapy and the role of nurses in care of diabetic patients. The research part encompasses an inquiry carried out as an anonymous interview where ten diabetic patients with insulin therapy were asked semi-open and open questions. The acquired data were transformed into case studies and categorization tables. The research questions were answered and the objective was achieved. In conclusion, the research suggests that patients do not have enough information about diabetes and insulin therapy. Specifics of the nursing care of diabetic patients with insulin therapy depends on the patient's needs. Most patients mentioned the need for contact with other diabetic patients. Nurses are involved in the education of patients with insulin therapy primarily as teachers of application and handling the insulin pen. This thesis served as the basis for the creation of an educational material for nurses. This material may prove helpful for nurses as a guide in the process of training diabetic patients to use the insulin pen. Alternatively, it can be used by medical care students as a learning resource.
Public awereness about diabetes mellitus

Diabetes mellitus belongs to diseases frequently occurring in childhood and adulthood. Insufficient knowledge of this disease and an unsatisfactory compensation for diabetes leads to development of late complications that have a negative impact on one's, as well as the family's, life and create an economic problem for society as a whole. The thesis focuses on the main characteristics of the disease, symptoms, diagnosis and treatment. Complications of the disease, including advice for patients themselves and their prevention are described in more detail. Social aspects (work inclusion of a diabetic, invalidity and reduced work capacity, driving of motor vehicles) are also emphasised. The research part containing 18 questions aims to find out to what extent the Czech public is informed about the problematic areas of this disease. It also assesses the feasibility of obtaining information from individuals of different ages and places of residence (city vs. village) regarding diabetes. These hypotheses were stated within the scope of the research: 1. Individuals older than 50 years of age are better informed about diabetes than individuals of a younger age. 1. Individuals living in a city have a better access to information concerning diabetes than individuals living in a village. The data was obtained from questionnaire research, in which respondents older than 18 years of age participated. The data collection took place in Bechyně Spa Ltd. 58 % of the resultant questionnaires were completed by women and 42 % by men.
