Case Study of Physiotherapy Treatment of a Patient with the Diagnosis of Omarthrosis Posttraumatica

Title: Case Study of Physiotherapy Treatment of a Patient with the Diagnosis of Omarthrosis Posttraumatica Objective: Summary of theoretical knowledge and processing of case study of a patient with the diagnosis of posttraumatic omarthrosis. Summary: This thesis consists of two parts. The general part includes knowledge of anatomy, kinesiology and biomechanics, as well as characteristics of the most common causes of pain in the shoulder joint and diagnostics and therapy, focusing on physiotherapy. Special part is devoted to case study and rehabilitation plan for patients with a diagnosis of posttraumatic omarthrosis. The therapy was aimed at eliminating the reflex changes, relieve pain, strengthen weak muscles and treatment of movement stereotypes. Case study was prepared at the time of practice from 1. 2. to 17. 2. 2012 at the department of rehabilition at the district hospital in Kladno. Use of low-frequency magnetotherapy for patients with coxarthrosis

The Bachelor thesis deals with the use of low-induction magnetotherapy in case of patients with coxarthrosis. The aim of the thesis was an application of the low-induction magnetotherapy in treatment of particular patients and its subsequent evaluation. Coxarthrosis belongs to relatively often degenerative disease nowadays, increasingly frequent among younger patients. Pain and joint stiffness, particularly in the morning, and limited range of movements are usually the main symptoms, leading to constraints in performing of normal daily activities. Moreover, most natural human movement - walking is impaired by the disease. Low-induction magnetotherapy is nowadays often used method of physical therapy belonging to a non-contact low-frequency electrotherapy which has as its main task to reduce the pain in coxarthritis. The thesis consists of two main parts. The theoretical part contains a description of the anatomy of the hip joint, more specifically its construction, ligamentous apparatus, vessels and nerves, muscle apparatus, movements in the hip joint and hip joint kinesiology. Osteoarthrosis, its principles, causes, symptoms, diagnosis and treatment, and coxarthrosis in more detail are also described here. The last topic discussed in the theoretical part is low-induction magnetotherapy. Its principles, physical parameters and the effects of magnetic fields, indications and contraindications are explained. In the practical part, qualitative research method was used. The research group consisted of three patients who attended Medipont polyclinics in České Budějovice on outpatient basis. In addition to low-induction magnetotherapy, physical therapy was indicated as well. Low-induction magnetotherapy is not recommended for use as monotherapy but its combination with other treatments is reported as more effective. The research was elaborated in the form of case studies that include patients' medical history, input and output kinesiology analysis and course of individual therapy. Kinesiology analysis includes a visual examination, palpation, anthropometric and trigonometric examinations, examination of pelvis, muscle test, and examination of shortened muscles, movement stereotypes, breathing, as well as walking. During therapy, the PIR, joint mobilization, soft techniques, exercises to activate HSS, stretching and strengthening exercises for the hip joint, and exercises for the proper activation of the transverse and longitudinal arch of the foot were used in addition to the low-induction magnetotherapy. Output examination after ten therapies showed changes in health status in all three patients; for each one, however, to a different degree. For best results, I would advise to continue with this treatment in the future. Interviews with ten patients in Berta's Spa in Třeboň and Medipont polyclinics in České Budějovice were conducted to better evaluation of the effects of the low-induction magnetotherapy. Patients rated their subjective experience with application of the low-induction magnetotherapy.
The psychological aspects of degenerative diseases of the central nervous system.

The degenerative diseases affect not only the central nervous system, but also attacks neurological, mainly motoric, functions. These diseases undoubtedly influence mental functions as well. In the theoretical part, the thesis focuses on a possible correlation between the psychical disposition - mainly emotional, and its influence on the development or progression of this kind of disease. Furthermore, the thesis concentrates on the cognitive alternations accompanying given diseases, on the patients' behavioral changes and emotional alternation. The thesis also presents coping strategies and recommendations concerned with the issue of degenerative diseases of CNS based on the recent studies. A research proposal in the empirical part focuses on the effects of social support and coping strategies on the patient with ALS disease and it also explores the patients' motivation and potential future goals which interfere with the quality of life. The issue of mentoring patients with degenerative diseases of the spine for autotherapy for long-term kinezioterapeutical plan

This bachelor thesis focuses on instructing the patients with a degenerative disease of the spine for autotherapy from the kinesiotherapeutic long-term plan point of view. The right instruction for autotherapy is one of the most fundamental elements not only due to its correct understanding, but most importantly for the right home exercising. Unless the exercises are done correctly, a patient's conditions may worsen, which we need to avoid. In the theoretical part individual anatomic structures of the axial system are described, including vertebrae and their interconnection and of course the spinal muscular components, greatly contributing to both the static and dynamic stability necessary for all forms of motion. Then, there are the most frequently occurring degenerative diseases of the spine and their diagnosis possibilities mentioned here. In the next part different ways of treatment are described. The main treatment for the degenerative diseases of the spine is a conservative treatment involving variety of methods and concepts in terms of physiotherapy. In case this treatment is not successful, a surgical treatment comes into effect, which does not yet guarantee a 100% positive result beforehand. In that case we are talking about the so-called failed back surgery syndrom caused by a number of factors, and thus the treatment is not clearly defined. Apart from its therapy the main aim of the thesis is to evaluate the efficiency of the variety of ways of instructing patients because there are many approaches through which it is possible to instruct patients on dong exercises at home. For my thesis I chose three options, which are a verbal instruction, an instruction according to pictures and a video instruction. To achieve the aim, the qualitative method of data collection was used. The sample group consists of three patients with a generative disease of the spine. In the practical part based on the initial kinesiology examination I, besides the therapy, deal with the patients instructions on autotherapy and I assess their efficiency. During the therapy each patient was informed about a few possibilities of the instructions which they should subsequently follow when exercising at home. During the first visit an initial kinesiology analysis was conducted. The second visit included the therapy and an oral instruction on how to do the exercises. These instructions and exercises were the same at the third visit. The fourth as well as the fifth session was finished with the instructions for autotherapy through pictures. The sixth and the seventh session involved video instructions. At the end of each session every patient demonstrated the assigned exercises which were being corrected at the same time. During the final therapy session the output kinesiology analysis was performed and the success of the therapy was assessed. Simultaneously an uninstructed interview was conducted with each patient in which the patients expressed their subjective opinion on the executed therapy and the ways of the instructions. All the patients state the most appropriate instruction was the audio instruction because of its simplicity and clarity.
