Communication of nurses with hearing impaired patient on covid units

Introduction: For nurses, communication is a basic instrument in the nursing process. During the Covid-19 pandemic, nurses were obliged to wear protective equipment to prevent the spread of the disease. Protective equipment, especially masks, have a negative impact on nurse - patient communication. They dampen the volume of spoken speech, prevent reading and recognition of facial expressions. Nurses had to deal with these communication barriers while maintaining the nursing standards. Purpose of this work: The aim of the bachelor's thesis was to find out the specifics when communicating with a hearing-impaired patient. Methodology:The empirical part of this thesis was processed using the method of qualitative research analysis. To collect data, a semi-structured interview was used with nurses who worked at a covid-19 unit during the Covid-19 pandemic. The conversations took place in the period from March to May 2023. The conversations were recorded with the participants consent on a mobile phone by the use of the application Záznamník (recorder). The interviews were written afterwards in text, using the Microsoft Word program. Written data were then analyzed by the method of open coding, using the "pencil and paper"technique. The data obtained were divided into three categories, which were subsequently divided into subcategories. For better illustration, the individual categories and subcategories were drawn up in clear diagrams and described in detail. Results:The work results were divided into three categories, which are: hearing-impaired patient, barriers to nursing care and nurses' communication with hearing-impaired patients. Both categories are divided to the period before the Covid-19 pandemic, the period during the pandemic and the period after the end of the pandemic. Research indicates that the Covid-19 pandemic has changed the nurses' approach towards barrier nursing care and nurses have gained invaluable experience in the sense of communication with hearing impaired patients. They learned to improvise and use all available communication tools. They shared information, advice and psychological support with each other. Conclusion: This work documents the view of nurses on barrier nursing care and their influence on communication with hearing-impaired patients. The research showed that nurses overcame the communication barriers and were able to maintain the standards of nursing care. They adapted to demanding conditions, learned to use simple tools which made it easier for them to communicate with hearing-impaired patients. They deepened their mutual cooperation. At the same time, it revealed that most nurses coped with these problems based on their intuition. For nurses, this experience created the need for professional training in the field of communication with hearing-impaired patients. The work provides an overall view of the issue of communication in the nursing process when using protective equipment. It presents tools that can help overcome the communication barrier. The results may help other nurses in practice.
Attitude of nurses to barrier nursing care on department of Infectious Diseases and on other departments of Internal Medicine

This bachelor's thesis deals with the nurses' approach to the barrier nursing care in the isolation ward and in other wards of internal type. The barrier nursing care is a system of work and organization measures which prevents the creation of nosocomial infections and the transmission of pathogenic germs in hospitals or social facilities. The nosocomial infection is an infection occurred in the direct connection with the patient's stay in the relevant facility. The barrier nursing method is a preventive measure which protects the sensitive individuals from the formation of the nosocomial infection. This method has to be respected by every medical worker without regard to the ward type. The most frequently occurring disease caused in connection with the stay in a hospital facility is the MRS A infection or the Methicillin-resistant Staphylococcus aureus. The barrier care is a complex of many methods including the sterilization, isolation of infectious patients, personal treatment of the staff, treating of the diseases of the medical staff, hygienic hand wash and the following hand disinfection. In the internal type ward, patients with diseases of internal organs are hospitalized. This ward type counts among the most basic wards of a hospital facility. The isolation ward is very specific because here, the patients with an infection disease or with a justified suspicion of such a disease are hospitalized. This implies that in the isolation ward the barrier care has to be respected more strictly. In the research part of the thesis, two objectives were determined: Detection of the nurses' approach to the barrier nursing care in the isolation ward and other internal type wards. Monitoring of nurses' knowledge and skills in the field of the barrier nursing care in the isolation ward and other internal type wards. On the basis of the mentioned objectives of the thesis, following research questions were set: How is the nurses' approach to the barrier nursing care in the isolation ward and the internal type ward? Are there any differences between the knowledge of the nurses in the isolation ward and in other internal type wards? Are there any differences between the skills of the nurses in the isolation ward and in other internal type wards? From the research survey follows that the knowledge and skills of nurses in the isolation ward and nurses in other internal type wards are not absolutely identical. From the research followed that the nurses' approach to barrier nursing is more effective in the isolation ward. Further, it was found out that the answers from a part of the respondents don't completely correspond with the data found out during the participating hidden observation. While the answers of the nurses from the internal ward nearly complied to the desirable providing of barrier nursing, during the observation was found out that the theoretical knowledge is not corresponding with the real practice in the form of practical skills. The results of the thesis can serve as a basis for teaching of nursing subjects or as a contribution to expert courses, tutorials or conferences dealing with this topic. Further, the results will be offered to expert periodicals where we would like to give to the expert and also general public a notice about the importance of providing of an effective barrier nursing care and contribute to paying more attention to this topic in all wards of health facilities.

Infectious complications belong to the most serious problems after endoprosthesis implantation. Their solution is very demanding for its surgical and psychological aspects with regard to its devastating effects and uncertain prognosis. The percentage of infections complications fluctuates between 0.5 and 2 % nowadays. From nurse?s point of view the care about these patients is very demanding and has its exactly defined specifics. The aim of the thesis was 1: To map the knowledge of the causes of occurrence of infectious complications after TEP of hip or knee joint among nurses. 2: To map the knowledge on nursing care of patients with infectious complications after TEP of hip or knee joint among nurses. 3: To examine adherence to aseptic procedures in caring about patients with infectious complications after TEP of hip or knee joint. Three research questions were set to reach the goals: 1. What is the extent of knowledge of the causes of occurrence of infectious complications after TEP of hip or knee joint among nurses? 2. What is the knowledge of specific care of patients with infectious complications after TEP of hip or knee joint among nurses? 3. What is the extent of knowledge of specific needs of patients with infectious complications after TEP of hip or knee joint among nurses? 4. To what extent nurses adhere to aseptic procedures in caring about patients with infectious complications after TEP of hip or knee joint? Qualitative research was applied to elaboration of the thesis. Semi-structured interviews and involved observation were the data collection methods and techniques. The research was performed at the infectious disease department of the České Budějovice Hospital. The survey was anonymous, the interviews and observation were conducted from 1 to 25 February 2013 . The research has shown that nurses know the causes of infectious complications occurrence after TEP. It is also obvious that nurses are aware of the specifics of nursing care about these patients and they also have theoretical knowledge of the needs. However the analysis of the replies has revealed that nurses have problems with applying some nursing procedures, namely becauseof partial ignorance, uncertainty and impossibility to try some procedures. The results have also shown that all the nurses try to saturate physical needs of patients but not all of them are able to saturate patients? physical needs as they mentioned in their replies and as we could observe. These are particularly drawbacks in communication in the field of motivation, mental support and self care support. We have also found that most of the nurses adhere to aseptic procedures in nursing care of patients with infectious complications after TEP, but the results have also revealed weak points. More than a half of the nurses of the monitored sample fail to perform hygienic disinfection in compliance with the standardized technique and a third of them do not disinfect their hands even before the act. We have found further drawbacks particularly in handling sterile and biological material. The research results have shown that despite lack of information and experience the nurses proved their general knowledge of care about these patients in their replies. However there is still a lot to learn, both theoretically and practically. This thesis was written to bring basic outline of the issue of infectious complications after total endoprosthetic replacement so as it can be used as information material for both, experienced or starting nurses that are in contact with patients with infectious complications after TEP. A multidisciplinary seminar aimed at the issues of infectious complications after total hip or knee endoprosthetic replacement (see Annex 12) is the practical output of the bachelor thesis
Nursing care of a patient with multi-resistant strains

Occurrence of bacterial resistance appeared already five years after the antibiotics had been introduced in practice. Currently the occurrence of resistant or multiresistant strains is increasing. It is therefore highly important to adhere to barrier nursing techniques for clients colonized or infected with multiresistant strains. The risk of transmission of infection between patients and also between the medical staff is thus minimized. However, many of the nursing staff have not fully realized the importance of adhering to the barrier nursing care. A quantitative research by means of anonymous questionnaires was applied to collect the data. The research survey was carried out in the specialized departments of Nemocnice České Budějovice a.s. (The Hospital of České Budějovice). The following information was checked: identification data, the awareness of the nursing staff concerning the barrier care and the possibilities of the nursing staff to adhere to this care in practice when nursing clients suffering from multiresistant strains. 139 questionnaires have been distributed from which 110 (100%) have been included in the research. In the bachelor thesis the awareness of the nursing staff concerning the barrier nursing care and the possibility to adhere effectively to this kind of care for nursing clients suffering from multiresistant strains has been investigated, which was also stated in the aims of the paper (there were three of them), which were confirmed by the results of the research. Three hypotheses were set. The first one was to prove that the nursing staff adhere to the barrier nursing care for patients suffering from multiresistant strains, which was confirmed. The second one was to prove, that the nursing staff have enough aids to adhere to barrier care for patients suffering from multiresistant strains. This hypothesis was also confirmed. The third one was to prove that the nursing staff are acquainted with principles of barrier nursing technique as prevention of transmission of multiresistant strains, which was also confirmed. In general it can be stated that although the nursing staff are over strained in their work physically and psychically due to evident lack of nursing staff in all departments, the awareness and adherence to barrier nursing care and thus the minimizing the risk of transmission of resistant or multiresistant strains is evident. The research has shown that in spite of problems the nursing staff is to handle daily, the adherence to barrier nursing technique is important for them. The awareness in this field is on a very satisfactory level, as well as material equipment and aids necessary to adherence to barrier care. There is also a tendency from the part of the superiors to inform on topical subjects in nursing care for clients suffering from multiresitant strains in regular seminars organized within the hospital. The adherence to barrier nursing care is an essential part of care for clients suffering from multiresistant strains. The results of the paper shall be used to inform medical staff on this issue to make a positive impact on opinions of medical staff on adhering to barrier nursing care to prevent transmission of multiresistant strains.
Observance of anti-epidemic measures by neonatal ward staff.

At the neonatology department the anti-epidemical regime from the nursing point of view is focused particularly on the blockage of possible infection transfer ways and prevention of the hospital infection at the newborn baby who has hypo-immunity during this period and therefore he/she is more sensitive and endangered by the infection occurrence. Individual parts of the anti-epidemical regime create a barrier system of the newborn nursing care which is necessary to be regarded from the nursing point of view as a complex of precautions. They have to be followed in their complexity as this is the necessary condition which guarantees that the barrier system will provide the baby with the safe protection against the hospital infection during his/her stay at the neonatology department. The Bachelor thesis with the name Observance of the anti-epidemical regime by the nursing staff at the neonatology department is divided into two parts. The theoretical part describes particular components of the nursing barrier system preventing from the infection transfer focused primarily on disinfection and sterilization, hygiene and disinfection of hands and handling with sanitary linen. The research part of the work deals with the results of the nursing research. The research investigation was carried out by means of the qualitative research. Methods of the non-standardized interview and the involved hidden observation were used for data gathering. The research group consisted of five children´s nurses and one midwife working at the neonatology department in the Hospital České Budějovice a.s. In the Bachelor thesis there were set three aims and three corresponding research questions. The first aim tried to find out if the nursing staff knows and follows disinfection and sterilization precautions, the second aim established if the nursing staff knows and follows standard procedure for hand washing and disinfection and the third aim stated if the nursing staff knows and follows the correct handling with the sanitary linen. The first aim involved the research question how the nursing staff knows and follows disinfection and sterilization precautions, the second research question connected with the second aim asked how the nursing staff knows and follows standard procedure for hand washing and disinfection and the third aim involved the research question how the nursing staff knows and follows handling with the sanitary linen. The author tried to find out whether the nursing staff working at the neonatology department knows and follows some performances from the selected parts of the barrier nursing system concerning the disinfection and sterilization, hand washing and disinfection and handling with the sanitary linen. On the basis of the provided interviews and observations the Bachelor work concludes that the nursing staff working at the neonatology department knows and also follows the principles of the selected parts of the barrier nursing system care for the newborn babies. The author supposes that the results of the Bachelor thesis research investigation could be used as information material for a seminar concerning observation of the anti epidemical regime held for midwives and children´s nurses working at the neonatology departments.
The issue of nursing care of patients with meningitis

The aim of the bachelor essay was to find out whether nurses respect the rules of a barrier nursing regime when treating patients suffering from meningitis, and to discover the specific features of the care of these patients. The determined hypothesis was supposed to confirm whether nurses respect the rules of a barrier regime when taking care of such patients. To specify data, we determined a research question which was supposed to find out what type of specific care patients suffering from meningitis need.
Nursing specifics in patients with leukemia

In the past the non-medical public understood the term onco-hematology to include one thing {--} leukemia. Qualitative research was used to prepare this essay; interviews and monitoring of processes in the internal medicine ward intermediary treatment in České Budějovice Hospital were applied in the process of data collection.
