Theory and Practice in the Care of Patients with Percutaneous Endoscopic Gastrostomy

The bachelor thesis deals with the care of patients with percutaneous endoscopic gastrostomy. It is divided into theoretical and practical parts. The theoretical part describes enteral nutrition, malnutrition, percutaneous endoscopic gastrostomy, and nursing care. The practical part includes processing and evaluation of the results obtained in the research investigation. Two objectives were set for the bachelor thesis. The first objective was to determine the level of knowledge in nursing care of the PEG patient. The second objective was to determine whether nurse working in the selected wards follow the procedures according to the standards of nursing care when treating PEG. To meet the objectives, a semi-structured interview was chosen which included twenty-three questions. Interviews were conducted with ten nurses in the internal medicine and oncology wards treating patients with PEG. The results support the fact that the level of education and length of experience influence the nursing care of the percutaneous endoscopy gastrostomy patients. It was found that some probands do patient preparation before PEG insertion and patient care on day 1 of PEG insertion. The probands are able to administer nutrition, care for the puncture area and administer medications and fluids into the probe. Problems were encountered in suctioning of gastric contents and rotation of the cannula. Most probands adhere to the department 's standard and continue to educate themselves with the help of seminars, books, or the internet. Afterwards, the interviews revealed that probands mostly encountered complications such as redness around the puncture site or pulling out the PEG probe. It was also investigated how nurses record nursing care in the documentation and how they educate the family or the patient upon discharge to home care.

0BABSTRACT This thesis is focused on enteral nutrition. This issue is an integral part of intensive and resuscitation care, but it is also a part of standard and follow-up care. There are more and more new resources and the products for enteral nutrition are improving. The theoretical part deals with malnutrition, discusses the different components of nutrition. Much of the thesis forms ways of application of nutrition. The issue is a nasogastric tube, a nasojejunal tube, percutaneous endoscopic gastrostomy, percutaneous endoscopic jejunostomy, feeding button or sipping. There are further expanded the introductions, nursing care, indications and contraindications of these options. The following chapters analyse the modes of enteral feeding, advantages and disadvantages of enteral nutrition compared to parenteral. The practical part is focused on research using anonymous questionnaires in the area of enteral nutrition. The survey was conducted in four departments of three teaching hospitals in Prague. It was focused on nurses and paramedics in medical intensive metabolic care units. The survey had 73 respondents. Nursing care for a client with percutaneous endoscopic gastrostomy at the Department of Subsequent Care.

The bachelor thesis has been divided into a theoretical and a practical part. The theoretical part dealt with enteral nutrition, percutaneous endoscopic gastronomy. The theoretical part is concluded by a chapter about nursing clients with PEG probe. The practical part contains two aims. The first aim was to map out specifications of nursing clients with percutaneous endoscopic gastronomic probe. The second aim was to find out what problems there are in nursing clients with PEG probe from the nursing staff perspective. Two reseach questions were posed: Question number 1 was focused on the specifications of nursing clients with PEG probe. Question number 2 aims at problems in nursing perceived by nursing staff in clients with PEG probe. The empirical part of the bachelor thesis was processed via a qualitative research survey, using semi-structured interviews and involved observation. Interviews were carried out with eight probants in the aftercare department. Results have proven that interviewed probants manage PEG client nursing using a PEG probe. They manage to nurse the injection spot, serve nutrition and medication through the PEG probe. Problems have been spotted in sucking stomach content away, and client positioning before nutrition serving. Further, the research has proven that probants stated PEG probe blocking as one of the most frequent complications, together with an inflammation of the injection spot, everything was consulted with a doctor. The results of this bachelor thesis could be used for purposes of educational seminars in aftercare department, thus increasing nurses´ awareness as well as imporvement in PEG probe client nursing
The role of Nutritional Therapist in Education of Patients with Enteral Feeding Tubes

Enteral nutrition is the only way for many patients to ensure a sufficient supply of nutrients. The bachelor thesis describes the indications as well as the contraindication of enteral nutrition. The chapter on composition of individual nutrition and possible application techniques is also included. I mention the possible complications associated with enteral nutrition, too. The greater part is then devoted to percutaneous endoscopic gastrostomy (PEG). Enteral nutrition is for many patients only possibility how to survive. During my long practice on the gastroenterology clinic, I have found that there is insufficient educational material for patients with established PEG who are leaving home. Educating these patients how to apply enteral nutrition at home and take care of the probe is in our facility in the competence of a nutritional therapist because of the workload of nurses and their little experience with PEG. The nutrition of patients with percutaneous endoscopic gastrostomy in practice

The bachelor thesis is focused on patients with percutaneous endoscopic gastrostomy in practice. The thesis is divided into theoretical and practice part of the issue. The theoretical part deals with stoma history, nutritional status diagnostics and general information on stoma. The core of the theoretical part contains the most important part of the thesis: introduction to PEG. The next chapter is dedicated to energy values determination and enteral nutrition, followed by the section dedicated to aid tools for enteral nutrition application. The last chapter in the theoretical part describes nutrition complications in percutaneous endoscopic gastrostomy. The aim of my work was to evaluate nutraceuticals intake administered to PEG, and to explore patients' or care staff 's awareness of PEG care in general. To achieve my goals, I have used qualitative research. Using a semi-structured interview with a nurse, and with patient and nursing staff in two other cases, I have gradually discussed PEG issues. I have worked with six patients, each of them I have interviewed twice. In the next section, I have determined requirements for nutrients, energy, selected vitamins and trace elements, which I then compared with a real enteral nutrition intake. I have used Nutriservis application to evaluate and determine the diet. Nursing staff often does not have enough information about the care that patients with PEG require, and availability of respective literature in the Czech language is very limited. Nursing education in the past did not include PEG care. The method was introduced in the Czech Republic only in the 1990s. Nearly all patients in my research were underweight. Moreover, despite their weight was changing, the type of nutrition and its quantity remained the same. The most common patients' complication was diarrhea. Although literature reports that the PEG tube should last for several years, it lasts for a year or two, according to the nursing staff. The information contained in my work can help not only patients with PEG and their families, but also health care professionals who look after PEG patients

Specifics of nursing care for a patient with nutritional stoma.

Main goal of the thesis was to map out the specifics of nursing care for a patient with a nutritious stoma. Three research questions have been identified in connection to this goal. First research question was focused on mapping out the nursing care for a patient prior applying the nutritious stoma. Second research question was focusing on nursing care for a patient while the nutritious stoma is being applied, and the last third question researches the nursing care for a patient after applying of the nutritious stoma. The empirical part of the bachelor's thesis was processed through a qualitative research analysis, which was carried out by a form of semi-structured interview. The semi-structured interview was divided into three parts. First part focused on basic identification data, second part focused on percutaneous endoscopic gastrostomy and third part on percutaneous endoscopic jejunostomy. Collected data was processed with an open coding method "pencil paper". Research file consists of general nurses working at a neurological, gastroenterological, standard internal department and intensive care department at Hospital České Budějovice and at the department of long term intensive care for patients at Hospital Třebíč. The results of the thesis are divided into a separate categories which are: establishment of nutritious stoma, immediate nursing care about nutritious stoma after establishment, nursing of nutritious stoma, applying of fluids into nutritious stoma, applying of nourishments into nutritious stoma, applying of medicine into nutritious stoma, recording of care into documentation and education of the patient about nutritious stoma. The thesis provides an overview of the problematics of nursing care for a patient with nutritious stoma. Based on the results a brochure will be written containing basic information about care for a patient with nutritious stoma, which will be addressed mainly to nurses.
Patients' education by enteral nutrition

Enteral nutrition is a form of a nutritional support which should be considered for malnourished patients or those at risk of a malnutrition who have a functioning gastrointestinal tract but are unable or unwilling to maintain adequate oral intake. This bachelor thesis discusses issues of the enteral nutrition - it's indications, contraindications, methods of delivering and possible complications. A big part of the thesis is dedicated to a percutaneous endoscopic gastrostomy which became the technique of choice to provide long-term enteral nutrition support and is nowadays one of the most popular approaches to the gastrointestinal tract for long term home enteral nutrition. The last part of the thesis shows the importance of the education of patients on a long- term enteral nutrition and provides a short guideline on how to properly educate them. Nursing Care for Patients with Percutaneous Endoscopic Gastrostomy.

The topic of thesis is nursing care provided to people, whether we call them patients or clients, with percutaneous endoscopic gastrostomy. According to experts, ercutaneous endoscopic gastrostomy is a relatively simple surgical procedure the outcome of which is very effective. By using a medical endoscope, the physician introduces an ostomy cannula into the stomach, one end of which leads right into the stomach and the other is led out through the skin of the abdomen. The cannula is then used to administer nutrition.Two objectives were stated in the research. The first objective was to determine the challenges in nursing care for percutaneous endoscopic gastrostomy in clients within the nursing process from the perspective of nurses. The second objective was concerned about the level of utilization of nutritional screening in the nursing process in clients with percutaneous endoscopic gastrostomy. These objectives were followed by four research questions.To obtain data for the research survey, the qualitative research method was chosen. For the data collection an in-depth semistructured interview was conducted with five nurses from DÚSP in Černovice. Within the qualitative research, the interviews with nurses were supplemented with the respondents observation, followed by the findings recording into the record sheet.The research outcomes show that nursing care for clients with percutaneous endoscopic gastrostomy has its own specifics, such as at serving meals and taking care of the stoma catheter. It has also been proved that from the perspective of nurses this care is more demanding, for example by placing demands on the nurses´ knowledge relating to the PEG and its treatment. It is obvious that nursing care for clients with percutaneous gastrostomy is the care that must be provided in a comprehensive way, within a holistic approach to the client.The survey results will be provided to the medical facility where the investigation was conducted and they may serve to improve nursing care for clients with gastrostomy to create or modify the current standard of nursing care for the PEG site.
