Scanning and evaluation of a patient's hand with Dupuytren's contracture
Dalecký, Jan ; Vosynek, Petr (referee) ; Návrat, Tomáš (advisor)
This bachelor’s thesis deals with development of methodology for determining the angles between joints of the fingers of a patient with Dupyutren’s disease. The methodology works with 3D scan of the hand created with the help of a smartphone and rotating mechanism. Nowadays, the measurement is carried out with the help of a goniometer. However, this measurement is often inaccurate and may not be very pleasant for the patient. The main reason for the research is to improve the quality of the measurement results and evaluate the patient’s condition outside of the time spent in the clinic. An electronic database with the possibility of visual comparison of the patient’s conditions is also an advantage. The angle measurement process takes place on the computer in Fusion 360 and Blender software. Both measurement software and mobile scanning applications are selected to make this methodology as accessible as possible. All applications are freely accessible and user-friendly. The digitally measured values significantly match the real values and even give more relevant results for the evaluation of the severity of the patient’s condition.
Comprehensive therapeutic rehabilitation of patients after surgery of Dupuytern's contracture
DYRYNKOVÁ, Pavlína
The topic of this bachelor thesis is concerned on problematic of Dupuytren's contracture. In the theoretical part I describe palmodigital aponeurosis, kinesiology, Dupuytren's disease, its classification, symptomatology, relation with gender, age, nationality, possible association with diseases and further description of treatment options. The aim of the thesis was to compile and propose a therapeutic plan and then to evaluate it in terms of proband's subjective perception and to compare it with the measured results. In the practical part, I made three case reports of probands who underwent Dupuytren's contracture operation. I obtained the data by taking a history and processing the input and output kinesiological analysis, followed by observation and a semi-structured interview. The first two probandes underwent ten therapies in my bachelor thesis and third only four, because he had to end the therapy prematurely for health reasons. In the first two cases, the extension was the most limited, and in the third proband there was the greatest limitation in reduced muscle strength and gait pain. According to the initial kinesiological analysis, I created a short-term plan and focused on the therapy. I used the aspect, palpation, anthropometric examination, goniometry, muscle test, table top test, sensitivity and grip assessment, and I always asked propand about subjective perception of his illness. Outcomes showed overall improvement in both patients. The other patient had no restrictions after the end of therapy. The first patient, despite visible improvements, still suffered from hand stiffness in the morning and increased manual labor and tingle I. - III. finger for the probably starting carpal tunnel syndrome. Achieving goals during therapy can be understood by creating individual rehabilitation plans, evaluating objective tests and subjective perception of probands.
Possibilities of conservative and surgical approaches for Dupuyter's contracture
Klímová, Michaela ; Demeková, Júlia (advisor) ; Babková, Lenka (referee)
Dupuytren' s contracture is a disease that is characterized by its slowly developing of flecture contractures of the fingers and is conditioned by pathological changes of the palmar aponeurosis. The bachelor's thesis "Possibilities of conservative and surgical approaches for Dupuytren's contracture" deals with the overall issue of Dupuytren's contracture. The thesis includes knowledge about anatomy of palmodigital aponeurosis, clinical picture, etiology, epidemiology, diagnostics, conservative and surgical treatment a nd points to a crucial influence of physiotherapy as an integral part of the treatment. A significant part of the thesis is focused on physiotherapeutic methods which are most frequently used for subsequent treatment of the disease. In the conclusion of the thesis, there is a brief description of physical therapy and occupational therapy. The thesis is supplemented with two case reports of patients with Dupuytren's contracture, treated with 2 different forms of treatment. Subsequently, final state of health of patients was evaluated depending on the method of treatment. Powered by TCPDF (www.tcpdf.org)
