Classification systems in the hospitals

Cílem této diplomové práce bylo nastudovat problematiku klasifikačních systémů v nemocnicích, které sledují především hospodářské požadavky hospitalizace. Následně bylo navrženo a naprogramováno webové rozhraní v prostředí Caché Server Pages (CSP), které poskytuje přístup k databázi CLINICOM. Pomocí webového rozhraní, je možno provádět klasifikaci pacientů do tříd MDC (Major Diagnostic Category), archivaci dat, výpočet DRG a další vybrané úkony. Webovou aplikaci by mohli využívat technicko-hospodářští pracovníci nemocnic a klinik jako jednoduchou pomůcku při své práci, respektive jako učební pomůcka biomedicínských oborů pří výuce zdravotnických informačních a klasifikačních systémů.
Classification systems in the hospitals

Classification systems in health care in the Czech Republic come increasingly to the fore when billing for services provided by hospitals. This work presents a review of classification systems and is focused on systems dealing with economic demands of hospitalization (DRG) in Czech Republic and abroad. In the Czech Republic, hospitals are working with IR-DRG system; therefore it is discussed in a separate chapter. The work also includes design of web application using technologies from InterSystems. The application serves as a module of the hospital information system Clinicom to calculate hospitalization rates by classification into the DRG.
Modulation of synaptic transmission in the development of painful states

My thesis introduces the topic of nociceptive signalisation and processes involved in the formation and spreading of neuropathic pain. This study focuses on the mechanisms of nociceptive synaptic transmission mechanisms in the level of spinal dorsal horn and its modulation by paclitaxel, a chemotherapeutic drug inducing neuropathic changes. The attention is put especially on the possibility of glial activity participation in paclitaxel side effects. This idea stems from the existing hypothesis of the functional connection between TLR4 and TRPV1 receptor activity. TRPV1 is well known for its participation in chemical, thermal and nociceptive sensory transmission. Minocycline antibiotic is considered as an inhibitor of microglial activation therefore it was used for blocking neuroinflammation. The experimental part is comparing an impact of substances applied to the model of tachyphylaxis used for monitoring of nociceptive transmission changes according to decreasing activity of TRPV1 receptors. Electrophysiological recording of miniature excitatory postsynaptic currents from neurons in the Rexed laminae I. and II. of spinal dorsal horn was used. The results of my measurements show that minocycline is able to suppress acute effects of paclitaxel application in vitro if the spinal slice is incubated...

The main Czech reimbursement legal documents are presented and formalized and their influence on volume of provided care tested. The role of TLR-4 receptors for modulation of nociceptive signalling.

The ability to feel pain is subjective, but a crucial factor allowing us to maintain life with minimal body impairment. However, if the pain crosses the physiological signalling function it becomes a life restrictive symptom that is often difficult to treat. Development of pain and its change into pathological chronic symptom and the role of different molecular and cellular mechanisms in this process are best to be studied during its development, nociception. In this work the basic characteristics of nociceptive signalling, with focus on neuropathy, a state originating from nervous system damage is given. The exact mechanisms of neuropathic pain development are not known, which is one of the reasons why these painful states are difficult to treat. Neuroinflammation was lately shown to play an important part in the neuropathy development. In this context a review of Toll-like receptors is given. These receptors have a fundamental role in the innate immunity and their role in the nervous system was documented recently. In this review a focus is given on TLR-4 subtype that was shown to be involved in a modulation of nociceptive signalling. Survey of the methods of costing of hospitalization

The thesis, Survey of the methods of costing of hospitalization, deals with the characteristics of the classification system DRG used for costing of hospitalization. The first half of the theoretical part of this work describes the Czech health care in general and specific ways of financing it. We focus on the costs related to the emergency care in hospitals which are classified by the DRG system. The second half of the theoretical part studies the actual principles of costing of hospitalization. The method chosen for this is called Activity Based Costing (ABC). It defines the procedures of costing which are being used in hospitals. At the end, we provide a description of the current and suggested procedures of costing of hospitalization and how they are utilized to set up parameters of the reimbursing mechanism. The practical part of this thesis maps the situation of hospitalization financing in Klatovska nemocnice, a. s. We suggest a new way of financing based on the methodology DRG Restart. Base on the data obtained in Klatovska nemocnice, a. s. we summarize and compare the results of the new method to the current one.
Funding of Health Care Service of Federal Republic of Germany

Financing healthcare is a problem not only in Germany but in many countries of Europe. Increasing total expenditure of health care resulted from many reasons, such as the aging population and the associated declining number of young people who contribute to the system. Another problem of financing health care, are ever increasing prices of medical devices, drugs, materials and energy. Since the beginning of the health system, Germany has been trying to stabilize the system through many reforms. The largest part of the revenues of the health system consists of contributions from members of statutory social insurance. Yet in Germany, financial participation in patients was implemented almost from the beginning of the health care system. Private costs are the second largest holder of health costs. Over the last 20 years, Germany has been among the countries that have a relatively balanced budget for financing health care. Among the most advanced countries in the world, which is seen in total expenditure of health care, had these expenses in recent years at around 11 percent of GDP.
