The Progress of Nursing in the 21st Century from a Patient's Perspective

Intoduction: Nursing is a human-oriented scientific discipline. Due to various world events, pivotal changes have occurred in this field. These changes have led to improvement in nursing care that is being provided in health facilities today. The Aim of the Thesis: The aim of the thesis was to determine changes to the field of nursing through the eyes of care recipients (i.e. patients) that happened during the 21st century. Furthermore, the goal was also to identify factors that influenced these changes and evaluate nursing care through the eyes of patients. Methodology: The thesis was carried out by means of a qualitative research survey in the form of semi-structured interviews with patients. The selection criteria for choosing an informant were residence in the South Bohemian Region and 18 years of age (or older). These informants were chosen by snowball sampling. The research survey included 20 informants. The interviews were recorded on an answering machine, literally transcribed and consequently analysed. Results: The thesis identified a total of 8 categories and 23 subcategories. The category of Scientific and Technological Progress was recognized as the central category. It is the driving force for most changes, including changes in education. The patients thought that the biggest change was modern equipment, new aids and devices in health facilities. They also mentioned innovations in the education of nurses and they agreed with the changes. The patients rated nursing care as a highly professional, expert and demanding field. They understood that more and more demands were being placed on the nurses. Moreover, the patients described the profession as prestigious, but also said that the public should be more informed about it. Conclusion: It is certain that nursing care will be influenced by further advances and innovations because science and technology cannot be stopped. In this context, the demands on nursing personnel will increase and the patients will be more demanding as well. Therefore, it is necessary to continue working on nursing care. However, it has to be taken into account how these changes and developments are perceived by the patients.
View of doctors on current nurses practise

In this bachelor's thesis we will deal with key aspects of the work of a nurse from the point of view of medical staff and we will also investigate the opinions of physicians on the current system of Czech nursing education. The present time has high requirements on nurses, especially on the work of a nurse and higher education. Today is very often said that nurses should be equal partners with physicians in the care of patients and take a respected position in a multidisciplinary team. The work is divided into theoretical and empirical. In the theoretical part of the bachelor's thesis we will deal with the history of nursing, current nursing and we will also focus on the doctor nurse relationship. In the empirical part of the work we will examine the view of physicians on the issue of education and work of a nurse. For processing of the empirical part was chosen qualitative research together with the method of semi structured interview. The research group consists of 8 medical staff from various clinical fields. Based on the interviews, the results were divided into 2 large categories and 8 subcategories. Gained results showed that physicians perceive current education as complicated and too long, preferring to devise a better system that would precisely define and adjust questions of increasing competencies and the related education of nurses and length of study. Furthermore, doctors stated that university studies bring benefits in the form of obtaining a university degree and higher prestige of the profession. Physicians consider the work of a nurse to be essential and have the opinion that the nurse is an indispensable part of well provided care for patients. We also found out that the most common reasons that affect the emergence of a conflict situation between the nurse and the doctor are stress, overwork, mutual disrespect associated with inappropriate behavior and violation of the competencies of both parties. Based on the statements, we can also state that doctors consider the nurse as their partner in the field of professional patient care and they do not have problem with accepting proposals or recommendations from nurses.
Role of the nurse in the administration of drugs in the workplace intensive care

This thesis deals with the role of nurses in the administration of drugs in the intensive care unit. The goal is to find consistency between the legal and factual state of drug administration, ie analyze, what are the non-medical healthcare professionals authorized in accordance with applicable legislation and internal regulations of the medical device to do and what they are actually able to perform safely. The theoretical part defines the terms related to the administration of medicines. I deal with the skills of paramedical staff, indications of medicines, their administration, pharmacotherapy, mistakes and evaluation of the quality and safety of health services. The theoretical part deals with the current status of this issue and also provides insight into the history of drug administration. In the empirical part of the thesis I work with the results that I received questionnaires. Respondents are nurses, paramedics, nurses of leading nursing management and physicians. Individual items are analyzed and used to answer defined research questions. The research implies that the nurse of nursing management are able to organize an intensive care unit activities and maintain procedures, to ensure quality and safe administration of medicines. The occupation of nurse as we know it today started to take shape thanks to Florence Nightingale, who pioneered military nursing practice. The first military nurses, including Florence, served during the Crimean war. The modern military nurse has the same duties as her historical counterparts {--} to identify and satisfy the needs of soldiers. She participates on the prevention process, cares for soldier's health, helps to create optimal conditions for their healthy lifestyle and training and provides first-aid training to soldiers. Nurses also take part on medical provisioning, assessments of military service capability and epidemic prevention. This research work has been elaborated using qualitative research methods. Its goal is to map the competencies of nurses in the Czech Army. Data was collected by means of semi-structured dialogues with nurses that participated on a military mission abroad. The results of the research with military nurses are elaborated by means of case reports and a slightly modified Ritchie and Spencer framework analysis, which was used to compile the category tables and charts that summarize the main research results. Three research questions were established to achieve the goal. The main goals are to find out whether nurses on foreign missions have competencies according to Czech legislation, whether they provide care on foreign missions by means of a nursing process, and what the conditions are for acceptance to a foreign mission. The results are designated for medical professionals as well as the general public and may also serve for study purposes.
