Design of Bed for Anesthesiology and Resuscitation Hospital Department
Andrášová, Lenka ; Buganská, Tamara (referee) ; Rubínová, Dana (advisor)
The thesis focuses on analysis and design of a special hospital bed designed for the department of anesthesiology and resuscitation. The design was based on the rules set by the ČSN EN 60601-2-52 norm. The bed is adjusted for the use at the department of anesthesiology and resuscitation not only with its look but also with its functions. The design emphasizes the ergonomics of the bed, safety and the needs of the patient as well as of the staff and also an easy maintenance of the bed. Some of the main innovative elements are: basal stimulation system placed directly in the bed, used colors, more infusion-rack holders or a headboard fixed to the construction of the bed if not needed. The bed was designed especially to provide more comfort to the patient.
Interaction between nurses and family members and their view of patients admitted to the anaesthesia-resuscitation department
DAŇKOVÁ, Pavlína
The present Master's thesis examines the topic of anaesthesia-resuscitation ward visits, interactions, and communication, seen from the perspective and views of nurses and family members of the patients. The primary objective was to examine the interaction between nurses and family members of patients admitted to the anaesthesia-resuscitation wards; the second objective was to investigate the nurses' and family members' perceptions of these patients. The author has achieved the goals mentioned above through qualitative research using two methods: semi-structured interviews with family members of patients and questionnaires involving nurses working in the anaesthesiology-resuscitation wards. The research has established the following: nurses keep an open, positive attitude towards supporting the patient's family; nurses' cooperation with the family members does not depend on their shift difficulty; however, the demandingness of the shift can be influenced by the amount of time the nurse spends with the patient's family; and the nurse's view of the patient is not subject to their education or length of work experience. It is hard for families to see their relatives admitted to the anaesthetic and resuscitation ward; such experiences are associated with very negative feelings; moreover, the family members expect the nurse to behave and act in a role-defined manner. Families usually want to be involved in their loved one's care and appreciate when the nurse is helpful, proactive, supportive and does not shun them; on the other hand, the unfamiliar and strange environment of the ward, and their loved one fighting for life, impacts negatively on families. The present thesis has resulted in drafting an information leaflet that nurses can use for the education of anaesthesia-resuscitation ward visits, containing basic information about the ward, general instructions and a description of the patient, accompanied by illustrations.
Care of the diet for pacients in an Anesthesiologic-resuscitation unit
BABIKOVA, Taťána
This Bachelor Thesis is on the problems with care about the nutrition for patients at anesthesiologic-resuscitation unit. The oftenest causes of disability to get meals normally in the acute medicine are: extensive operations, injuries, tumors or infections. From the beginning the patient must be fed only parenterally. After the regeneration of the intestinal passage the intestinal tract is loaded only parenterally. So, it is very important to choose proper methods to make their effect the largest. This thesis is divided into two parts: the theoretical and the empiric. The objective of this work is to map care about the nutrition for patients at anesthesiologic-resuscitation unit. For this aim we determinated the following questions: How do nurses at anesthesiologic-resuscitation unit take care about patients with parenteral nutrition? How do nurses at anesthesiologic-resuscitation unit take care about patients with enteral nutrition? For the realization of the empiric part of the thesis we choose qualitative research investigations. The collection of data was carried out by means of demi-structured dialogues wit nurses. The dialogues were rewrote to Microsoft Word 2016 word for word. Thus, obtained information was coded and then categorizied. The research investigation took place during April 2020. The research showed that nurses have enough knowledge of the problem with nutrition and they will manage to take care about patients with parenteral and enteral nutrition. We found out that at anesthesiologic-resuscitation unit parenteral nutrition is primarily launched and that is mostly through central venous catether or exceptionally through peripheral venous catether. After the stabilization the patient gradually moves on to the enteral nutrition in the form of so-called sipping up to the total applying the enteral nutrition more often through nasogastric tube or oral. The results of the search are realized in the project of the seminary for nurses which will be used for the deepening of their knowledge about these problems.
Nursing care of respiratory system of patient with a tracheostomy cannula at the Anesthesiology-Resuscitation Department
PEŠKOVÁ, Michaela
The thesis presents nursing care of respiratory system of a patient with a tracheostomy tube at the Anaesthesiology & Resuscitation Department. The thesis is divided into two parts, theory and practice. The theoretical part concerns anatomy and physiology of airway. Then it clarifies terms such as the Anaesthesiology & Resuscitation Department and tracheostomy. It briefly mentions the history of tracheostomy, its importance and types. It also describes nursing care of respiratory system. That part introduces the topic of aspiration, air moisturizing, airway lavage and also oral and nasal cavity care. The theoretical part also concerns the importance of communication with tracheostomy patients. Two objectives have been set for the empirical part. The first objective was to check the theoretical knowledge of tracheostomy patient care nurses at the Anaesthesiology & Resuscitation Department. The second objective was to check rule compliance during the tracheostomy patient care at the Anaesthesiology & Resuscitation Department. A qualitative research was conducted at the form of a structured interview where 22 questions prepared beforehand were asked. The research applied to a group of 10 nurses carrying out their duties at the Anaesthesiology & Resuscitation Department of České Budějovice Hospital. The results have shown that the theoretical knowledge of tracheostomy patient care nurses is quite good but in a few cases that knowledge could be deepened. Based on the results however we've come to the conclusion that patients are provided with a quality care. Although tracheostomy patient care nurses meet the required standard set by the department it would be beneficial to them as well as to the patient if they completed their knowledge at least in the form of self-study.
Selectit non-medical professions and their aplication in nursing care at comprehensive resustitation care unit
DAŇHELOVÁ, Monika
This Bachelor´s Thesis is focused on selectit non-medical professions and how to use the provision nursing care at the comprehensive resuscitation care unit. The aim of this thesis is finding out the knowledge of the top, middle and line management about the legislative competence of the chosen non-medical healthcare workers at the comprehensive resuscitation care unit. The author of this paper is trying to compare application of the competence the non-medical healthcare workers in provision nursing care at this unit. We make three research question for come to these goals. What is the knowledge about the legislative competence of provision nursing care at the comprehensive resuscitation care unit of point of view of top, middle and line management (nurse, practical nurse, paramedic, healtcare assistent). What is the application of the chosen non-medical health care workers (nurse, practical nurse, paramedic, healtcare assistent) in the provision nursing care at the comprehensive resuscitation care unit. Analytic part of this paper is doing through the qualitative research. These research comprise the answer of nine responders from four hospitals. The research sample comprise three top nurses from the each hospital, three head nurses and three station nurses which work at the ARO. Based at the results of the research, the healthcare assistent (now practical nurse) is not present at the ARO. The reason of this situation is less competence (she is not able to do work by herself). The more then half responders agree, that the nurse are more common at the ARO. The results of research show that the most responders know the legislative changes.
Invasive provision critically ill pacient on anesthesiology and resuscitation ward
HROMADA, Přemysl
The subject and objective of this work was to observe and describe the possibilities of invasive monitoring in the anaesthesiology-resuscitation ward. Attention was focused on the research of knowledge of the types of invasive monitoring on the anesthesiology-resuscitation ward in the nursing and paramedic job positions. A comparison of knowledge of the types of invasive monitoring of these workers was realized in two selected hospitals - Jihlava Hospital, Contribution Organization and České Budějovice Hospital, a.s. To realize these objectives, it was necessary at first to study the types of invasive monitoring and theoretically to describe selected types of monitoring. After that was realized the practical qualitative research of knowledge in this area. This research was conducted by semi-structured interviews with randomly selected anesthesiology-resuscitation staff in the nursing and paramedic job positions of those hospitals. The knowledge gained from the interviews was subsequently composed in the form of tables, and on this basis the information received from both hospitals was compared with each other in the examined group of workers. The benefit of this work is the acquisition information on the knowledge of the staff of the anesthesiology and resuscitation ward in the area of invasive monitoring of critically ill patients. It has been found that the knowledge of the employees depends on what kind of invasive monitoring these staff encounter most often in the ward. Staff who work in a regional hospital have wider knowledge than the staff who work in district hospital, because workers in a regional hospital do more often and more types of invasive monitoring, than workers in district hospital.
Invasive monitoring critically ill pacient on anesthesiology and resuscitation ward
HROMADA, Přemysl
This work describes the current state of providing intensive care for critically ill patients at ARO, then dedicates with the concept of critically ill patient, invasive monitoring, and also acquaints with the ARO department and the subject of its activities. From the point of view of invasive monitoring of a critically ill patient, the theoretical part of this work presents individual ways of monitoring the cardiovascular system. For each of these KVS monitoring methods, the subject and importance of this monitoring, its indication, contraindication, performance technique, as well as possible complications and, last but not least, nursing care that is essential in the care of a critically ill patient are described. The practical part of this work monitors and compares the knowledge of the ARO staff of České Budějovice and Jihlava Hospital regarding the various ways of invasive monitoring of KVS. Caring for organ donors on anesthesiology and resuscitation department
BURDA, Michal
Transplantology is essential branch of medicine, which is subject to legislative norms and international conventions, but it must be understood primarily as an ethical problem, which solves a major life decision and save human life. It is not just about a medical site. Actual operations precede the long preparation, many examinations and also nursing care. Although this is a very broad field of medicine, there is not a large number of publications dealing with general questions. This work was created to unify information on transplantation medicine and especially information regarding organ donation and care of them. The theoretical part deals with the whole range of activities that precede the actual transplantation. It contains history, legislation, transplantology basic terms, allocation of organ donors, the determination of brain death, the care of organ donors, coordinating transplantations, national health registers. The practical part deals with mapping techniques in diagnosis and treatment and examines an approach to donors at the Anaesthesiology-Resuscitation Departments of selected hospitals. This part of the work also compares the procedures under the care of individual departments. As a part of this research I have done semistructured interviews with individual workers from Anaesthesiology-Resuscitation Departments; the results were evaluated and processed. In conclusion, I must say that we have a good care for organ donors in our country. Despite some theoretical deficiencies, it is clear that workers respect of established procedures and follow the latest instructions provided directly to individual transplant centers. This statement is supported by figures that confirm the upward trend and quality work of Czech health workers and doctors. The Czech Republic is nowadays one of the leading countries in the world comparable to transplant centers.
The issue of treating patient with polytrauma
DVOŘÁKOVÁ, Barbora
Polytrauma is an injury of one or more organic systems or body areas the mutual combination and extent of which may lead to direct death threat. Polytrauma stands on the first step of the mortality ladder particularly among those of 1 45 years of age. Not only timely emergency care, but also professional quality of the care play important role here. Treatment of a patient with polytrauma requires quality performance of physically and psychically demanding work The aim of the thesis was 1: To find out whether nurses at an ARD, an ICU and a standard department have knowledge of nursing care of a patient with polytrauma. 2: To find out the specifics of nursing care of a patient with polytrauma at an ARD, an ICU and a standard department. 3: To find out the differences in nursing care of a patient with polytrauma at an ARD, an ICU and a standard department. Research questions: 1. Where do ARD nurses see the biggest problems of nursing care of a patient with polytrauma? 2. Where do ICU nurses see the biggest problems of nursing care of a patient with polytrauma? 3. Where do nurses from a standard department see the biggest problems of nursing care of a patient with polytrauma? 4. How do ARD nurses care of a patient with polytrauma? 5: How do ICU nurses care of a patient with polytrauma? 6: How do nurses from a standard department care of a patient with polytrauma? 7: What opportunities for education in nursing care of a patient with polytrauma do ARD nurses have? 8: What opportunities for education in nursing care of a patient with polytrauma do ICU nurses have? 9: What opportunities for education in nursing care of a patient with polytrauma do nurses from a standard department have? Qualitative research was applied to preparation of the thesis. Non-structured interview and involved observation were the data collection methods. The research was performed at an anaesthesia and resuscitation department, at an intensive care unit and at a standard traumatology department. The research was anonymous. The research has shown that ARD and ICU nurses as well as those at the standard department are aware of treatment of patients with polytrauma. We observed that they cared about patients' needs and all activities they perform comply with nursing care standards. They are aware of complications that might occur in a polytraumatized patient. We have also found that mainly the difference in communication with a patient belongs to the specifics of the above mentioned departments. The results also show that the care provided at the ARD and the ICU is acute, while the standard department mostly provides subsequent care. We found that the biggest difference in treatment of patients with polytrauma at the above departments is in communication with a patient. Communication is at the minimum at ARD, where work with monitors prevails. Nurses of the ICU and the standard department cope with different problems. These are lack of space and electronic equipment at the ICU. The care of a patient with polytrauma who is not self-sufficient and requires permanent care and attention of a nurse is physically and psychically demanding at a standard department. It is not sufficient according to the nurses. The problem is in their opinion in high number of patients, lack of beds, extensive attention to some patients and last but not least the growing administration conducted by nurses. This brings the problems in nurse-patient communication at a standard department. The nurses are aware of the fact that effective communication has positive impact on patient recovery as it moderates pain and confusion and helps against anxiety and stress, helps relieve pain, reduce complications and accelerate convalescence. It would be interesting to prepare a seminar for the ARD, ICU and standard department healthcare staff to improve nursing care of a patient with polytrauma in relation to that.
