Nursing care for patient after lung surgery

The operation of the lungs is being performed on the base of a variety causes, among which the most common causes are cancer, inflammatory diseases, accidents and a variety of complications of diagnostic and therapeutic procedures. After the surgery, patients are taken to the intensive care unit (ICU). The nurse must meet the patient´s biological, psychological, but also laboratory and medication needs. The attainment on this thesis which is titled as "Nursing care for patients after lung surgery" was to determine the method of nursing care for patients after lung surgery. Three research questions were also asked to achieve this. The necessary information were obtained through qualitative research, semi-structured interviews with nurses from the Department of Surgical Intensive Care Unit, Hospital Czech Budejovice a.s. The research was conducted during March 2015. Three categories were generated from an analysis of data: Specific care for patients after lung surgery, Education, Rehabilitation. This research suggests that nurses meet frequently with this specific care for patients after lung surgery that means several times a month. Further information from this research is that the patient after surgery gets parenteral nutrition first, on the second day starts with a liquid diet and the nurses are following the care with most commond diets as diet no 0, 1, 3, and diet no 9 if the patient is a diabetic. After the surgery, the oxygen is being provided to a patient. The results showed that the nurses must always check to see if the airways are clean, that the patient does not have any mucus in the lungs and that he's able to cough without problems. Nurses are checking the blood gas values by using a Pulse Oximeter. Emphasis is placed on the post-surgery rehabilitation to allow patient to be fully ventilated. For a different lenght of times the indwelling urinary catheter (hereinafter PAK) is fed to patients with respect to the operating performance. It depends on the patient's health condition. The nurses must check the functions of the thoracic drains which are often inserted after surgery. First, the patient has an active suction or active mobile suction, then Bülauov drainage. Frequency of change of the surgical wound dressings is mainly set by the doctor. Postoperative pain is monitored in a verbal way. Prescribed medication is administered also by using the epidural line. Subsequently, the patient receives painkillers IM inj. According to the nurses the post-surgery complications are breathing problems, bleeding from wounds, pain, risk of infection, high waste of chest tubes, the inability of the lungs to maintain a vacuum and subsequent collapse of the lung. The education of the nurses is very multidirectional and each one applies different methods to enrich their knowledge. Physical Therapists are responsible for the rehabilitation of patients after lung surgery. Nurses have a very good knowledge in a nursing care which is applied in their practice. However in some ways the information obtained from the nurses disagree with the procedures set forth in the literature. The conclusion of this work should be aimed to the improvement of nursing care for patients after lung surgery and to correct the shortcomings in the providing of nursing care. This work can be used as educational material for nurses starting job at the Department of Surgical Intensive Care Unit, Hospital Czech Budejovice a.s.

Patient satisfaction regarding the quality of nursing care administered by the surgical department`s intensive care unit of Ceske Budejovice Hospital, a.s. Patient orientation - the effort to comply with his/her requests to achieve his/her satisfaction - is the current trend in health service. The overall satisfaction rate as well as the satisfaction in individual areas of medical care is the important indicator of the quality of care in a hospital. For these reasons patiens are asked to evaluate the quality of medical care. The outcome can be used as the source of suggestions for improving health care quality. The goal of the bachelor`s thesis is to find out the overall satisfaction rate with the nursing care of patiens hospitalized at the surgical department{\crq}s intensive care unit of České Budějovice hospital, a.s.. I set up the hypothesis: The patiens hospitalized at the surgical department{\crq}s intensive care unit of České Budějovice hospital, a.s. are satisfied with the provided nursing care. The research sample consists of patients of the surgical department{\crq}s intensive care unit. The questionnaire method was used. 120 patients were polled and 95 questionnaires were handed in. The final result shows that the vast majority of patiens hospitalized at the surgical department{\crq}s intensive care unit of České Budějovice hospital was very satisfied with provided nursing care. The results of the monitoring of patient satisfaction are a very good feedback for the personnel. Patient satisfaction is directly connected with satisfying patient needs and mutual communication between nurses and patiens. The conclusions of the bachelor{\crq}s thesis will be given to the management of the surgical department, the departmental sister and the nurses at the intensive unit care and can be used for reflection on provided nursing

The Making and Use of Educational Cards in the Job of a Nurse at the Surgical Ward The objective of the present work called ?The Making and Use of Educational Cards in the Job of a Nurse at the Surgical Ward? is to propose a unification, consolidation and simplification of information given to a client so that the educational process might be transparent, effective and acceptable for general (patient) public. At the beginning of the research the author set four goals: Goal 1: To find out whether the education of clients prior to selected surgical procedures is effective or not; Goal 2: To find out the methods of education of clients at the surgical ward prior to selected surgical procedures as viewed by nurses; Goal 3: To make (create) an educational means (educational cards) for clients prior to selected surgical procedures. Goal 4: To check the created educational means in the nursing practice. The author also set three questions for her inquiry: Question 1: What knowledge do clients have prior to the surgical procedure for a successful postoperative development? Question 2: What educational methods do nurses apply with clients prior to surgical procedures? Question 3: What positive aspects do clients perceive when working with educational cards? The inquiry took place in the Znojmo Hospital with the matron?s kind permission. To achieve successful results in her work, the author applied qualitative examination, the method of a questionnaire, the technique of a non-standardized interview. The object of the inquiry consisted of clients and nurses of the surgical ward. Having analysed the answers of all respondents, the author put together and subsequently checked in practice educational cards for clients prior to surgical procedures. The results of the present bachelor work can be used further in practice, e.g. the created educational cards can be left at the surgical ward at patients? disposal for the whole period of their hospitalization. Is to propose a unification, consolidation and simplification of information given to a client so that the educational process might be transparent, effective and acceptable for general (patient) public.
Caesarea section- perioperative nursing care during regional and total anesthesia

In my thesis I focused on perioperative nursing care for women with a caesarean section in regional and general anesthesia. The thesis is divided into a theoretical and research part. In the theoretical part of my thesis history, conditions, indications and contraindications, and also complications during a caesarean section performance are described. Information about anesthesia which is used for a caesarean section is also given. Conditions for general, epidural and spinal anesthesia administration are described. Advantages, disadvantages, indications, contraindications and complications of different types of anesthesia are also mentioned. A substantial part of the theoretical part is devoted to perioperative nursing care. The preoperative nursing care is described, the preparation for it is divided according to the type of anesthesia. Nursing care during the intraoperative and mainly postoperative phases is mentioned in detail. The aim of my thesis was to identify differences in perioperative nursing care for women with a cesarean section in regional and general anesthesia. My work was also focused on women's level of awareness about possibilities of anesthesia for caesarean section. Another goal of my work was to make up standards of perioperative nursing care for women during a caesarean section delivery in regional and general anesthesia, which is administered in the delivery room. Two hypotheses were stated. The first hypothesis is the supposition that women after a caesarean section need a different nursing care, which depends on the type of anesthesia administered. The second hypothesis is the supposition that women are adequately informed about the possibilities of anesthesia for a caesarean section. Quantitative research conducted through questionnaires was chosen to conduct the survey. Questionnaires were distributed to women after a caesarean section at the confinement department. Research results are shown in graphs .The both hypotheses were confirmed by the research investigation. The research has revealed a new finding that women need different nursing care not only after a caesarean section, but also during the intraoperative phase. I used this finding to develop a standard of perioperative nursing care for women during a caesarean section delivery in regional and general anesthesia. The standard is designed for midwives and is intended to improve nursing care for women in the course of a caesarean section delivery during the intraoperative phase. The research has proved that women with a cesarean section need a different perioperative nursing care, which depends on the type of anesthesia administered. It is therefore necessary to differentiate between the two groups and provide them with a different nursing care. The research has also shown that women feel sufficiently informed about anesthesia for a caesarean section. Women´s awareness is a prerequisite for a good cooperation with the patient and successful nursing and medical care. The nursing care provision according to the perioperative care standard will ensure a quality professional nursing care for women before, during and after a caesarean section.
Effectiveness of early Physiotherapy After the Operation Total Acetabulofemoral Articulation

In the bachelor thesis called Effectiveness of an early physiotherapy after an operation of the total hip joint replacement, there are theoretical bases related to the given issue in the first part. In the research part there are worked up case studies of two clients who had the same disease, were operated in the orthopaedic ward at the Hospital of České Budějovice and a total hip joint endoprosthesis was made to them. The aim of the thesis is a complex evaluation of physiotherapy methodologies at the patients after an operation of the total hip joint endoprosthesis and an evaluation what could be improved in this issue, so that the postoperative rehabilitation could be a little more efficient. At each patient during the hospitalization were differences in the success of the therapy. In conclusion it is possible to say, that an important role in the effectiveness of the postoperative treatment is a completing of a preoperative rehabilitation. The patient, who completed the preoperative rehabilitation, better managed a verticalization, walking and self-sufficiency after the operation in contrary to the patient who did not complete the preoperative rehabilitation. The significant problem was that she would have had to commute for the preoperative rehabilitation and it was a problem for the patient. Patients waiting for the operation of the hip joint replacement are often constrained by a pain while walking and they are discouraged by a thought to commute to the preoperative rehabilitation. I propose to carry out the preoperative rehabilitation already during the hospitalization, several days before the operation. The thesis can serve as a theoretical study background for students from Medical universities or it can become, in terms of physiotherapy, an inspiration in a clinical practice at physiotherapists engaged in those issues.
Educational methods in surgical branches

Educational processes in surgical branches include not only remote or direct client´s education before an operation or after an operation but also the client´s preparation for check {--} ups.In my work there were two objectives. Objective 1 : to find out whether clients hospitalized in the surgery ward are educated by surgical nurses. This objective has been determined. Nurses educate clients during hospitalization in a surgery ward. Objective 2: to find out educational processes used by nurses in the surgery ward. This has also been determined. Nurses use mainly interviews, practices, booklets, prospectuses, handbooks. There were set two hypotheses:Hypothesis 1: I suppose that nurses in surgical branches educate clients hospitalized in the surgery. This hypothesis has been confirmed. Hypothesis 2: I assume that nurses in surgical branches use mostly interviews as the educational process. This hypothesis has been also confirmed.
Preoperative rehabilitative preparation provided by a nurse

Summary The bachelor thesis {\clqq}Preoperative Rehabilitation Provided by a Nurse`` is a research project. The thesis is devided into two parts theoretical and practical. In the theoretical part I deal with the branch of gynaecology, gynaecological surgeries, gynaecological treatment as well as the role of nurse working in a surgical gynaecology ward. Further I pay the attention to preoperative preparation and immobilization syndrom. Preoperative rehabilitation, however, forms the most essential part of the thesis.I am concerned with the prevention of postoperative complications occurance and practice of particular rehabilitation excercising. The practical part contains quantitative research provided by the technique of two questionnares ( for both nurses and clients), in which I try to confirm specified hypotheses related to project goals setting. The hypothesis 1 : {\clqq}The nurses in a surgical gynaecology ward don´t train postoperative rehabilitation practice with a client`` was confirmed but the hypothesis 2 : {\clqq}The reason for disusing postoperative rehabilitation by clients is non-practising rehabilitation in preoperative time`` wasn´t confirmed. During the research period there were distributed 156 questionnaires, 56 for nurses and the rest (100 questionnaires) for women after heavy gynaecological surgery. 139 questionnaires turned back , the rate of return was 89 %. The total set of acceptable and thoroughly completed questionnaires was formed by 51 questionnaires from nurses and 81 questionnaires from clients.
A nurse and physical examination of a client/patient

My thesis deals with physical examination of the patient by a nurse and its right technique. Physical examination generally follows the taking of the medical history - an account of the symptoms as experienced by the patient - in internal and surgical wards. It becomes part of the medical record. The theoretical part describes the complete physical examination and gives its definition. Physical examination is focused on the somatic condition of the patient. A systematic examination generally starts at the head and finishes at the extremities. The main organ systems are investigated by inspection, palpation, percussion, auscultation and per rectum examination. For physical examination a nurse uses her senses - sight, hearing, touch, sometimes also smell. My thesis examines the right technique of physical examination and why this examination should be carried out by a nurse. I also wanted to give reason why the physical examination should become a common part of the nursing process.
