Specifics of measuring pressure in children

The number children and young people with high blood pressure has been increasing. The current lifestyle is one of the contributing factors to the disease. The most adverse is poor quality diet and excessive intake of salt. Further, the energy expenditure is lower, e.g. due lack of physical activities. Many children and young people do not have enough information about the risks of high blood pressure and its prevention. It is critical to identify groups with a risk of hypertension, i.e. children and young people with the so-called ?high normal? blood pressure. At the beginning we set four objectives: to learn the blood pressure measuring techniques suitable for children ? using digital and mercury tonometers, to find 25 ? 30 children with the ?high normal? blood pressure, to instruct nurses, particularly in the primary care, about the correct technique of blood pressure measurement in children and to compare the values of blood pressure measured with mercury and digital tonometers. The research was conducted in basic and secondary schools in České Budějovice and in a pediatric surgery. The researched group consisted of children and young people aged 13 ? 18. To achieve the first objective we measured blood pressure in 613 children with a digital tonometer and to achieve the second objective we measured blood pressure in 50 children both with mercury and digital tonometers. The results have been presented in form of tables. We found out that it was not necessary to examine more children, as we had expected, to get a sample group of 30 children with the ?high normal? blood pressure. Further, we found out that there was no significant difference between the values measured with a mercury tonometer and with a digital tonometer. The results have shown that it is essential to use the correct technique for blood pressure measurement in children. Based on our findings we have developed an educational material for nurses.
Analysis of eating habits in people with hypertension with the help of NutriDan programme.

Hypertension, i.e. elevated blood pressure, ranks among the commonest civilization diseases affecting heart and blood vessels. The incidence rate covers about 25% of adults and it may have deadly consequences. The first step in treating the condition is a change in the way of life. The patients are encouraged to stop smoking; to take reasonable exercise; to reduce stress and, last but not least, to eat healthily. A diet can be assessed using NutriDan, a software leaning on a database of hundreds of foodstuffs having well-verified nutritional values. Health-care professionals can use the software tool to easily calculate the average daily intake of individual components present in a patient's diet. The tool considers the patient's age, physical activity, and the current nutrition. In compiling the paper the author used the software to analyze the nutritional habits of hypertonic patients. The research objective was pursued with reliance on the methods of qualitative survey and interrogation, using the technique of semi-controlled interview to be then made into casuistries. Contacted were hypertonic patients in České Budějovice - with a half of the respondents below 63 years of age and the other half over 63. To identify the quality and quantity of food, the author analyzed their nutritional records (their diet) for 7 days and evaluated the findings with the NutriDan tool. The survey covered 14 respondents whose nutritional records were analyzed to answer the researched question asking whether the daily volumes of energy, sodium, fats and cholesterol that the hypertonic patients received through their diet corresponded with standards recommended by the NutriDan software. The analyses showed that it was not the case. Results arrived at in the paper can be instrumental in mapping the issue of observing the diet provisions in hypertension patients, while the deficiencies found could be utilized to streamline the process of education followed to foster healthy life style.
OPTION TREATMENT HYPERTENSIVE IN ČR AND THEIR INFLUENCE OVER NURSE CARE

The diploma paper was prepared on: Alternative treatment of hypertension in the Czech Republic and its impact on nursing care. Hypertension is one of the most common diseases of the cardiovascular system and it is the most frequent cause of morbidity and mortality in the Czech Republic. High blood pressure occurs more and more often nowadays and it is frequently underestimated by patients. Therefore, its treatment as well as its prevention is important. A nurse should be able to guide patients with hypertension along the right path and to advise them, especially in relation to preventive precautions against hypertension origination, but also a nurse should be close during the treatment of high blood pressure using other methods. Two types of interviews were carried out with nurses and patients at the cardiology department of hospital Nemocnice České Budějovice, a.s, at the cardiology department of hospital Na Homolce, a.s. and at the department of internal medicine at hospital Nemocnice Klatovy, o.p.s. In both the interviews, we first focused on the identification data of the nurses and the patients, and then questions were directed at answering our research questions specified in advance. According to the results of the data obtained from the interviews with the nurses and the patients, it was found that there was no difference in the nursing care provided to patients treated pharmacologically and patients with non-pharmacological treatment. The individual variants of hypertension treatment therefore have no impact on nursing care. Research question 1 was: Are there any differences in nursing care between pharmacological hypertension treatment and alternative hypertension treatment? Research question 2 was: Do alternative hypertension treatment methods affect nursing care? Both of these research questions have been answered by the data obtained from the interviews with the nurses as well as with the patients. Research question 3 was: Do patients view alternative hypertension treatment as part of the treatment of this disease? This question was answered by the data obtained from the interviews with the patients. Since the survey found that there are not any differences in the nursing care provided to patients with pharmacological therapy or non-pharmacological treatment, and that alternative hypertension treatment methods have no effect on nursing care, the two groups of patients should be educated properly and checked for high blood pressure. An appropriate recommendation is to organize additional research to the problems of educating patients on regimen precautions during non-pharmacological treatment of arterial hypertension.
Possibilities pregnancy after transplantation of the heart

The thesis deals with the possibilities of pregnancy of women in fertile age after the heart transplant. The aim was to find out, whether there are women after the heart transplant in the Czech Republic, who got pregnant, delivered a baby, or are trying to get pregnant, or are interested in pregnancy, at least. The qualitative research utilizes a survey method, which was aimed at physicians, who are looking after patients after the heart transplant. Furthermore a non standardized interview was held with the physician of the Outpatient{\crq}s department for the Transplants at IKEM and with her patient, who is currently trying to get pregnant. Personal documents of the patient trying to get pregnant were used, in order to make the research more specific. It is apparent from the results that there is a small group of women after heart transplant in fertile age. Thus, for ascertaining the result, the foreign literature was used, which describes already successful pregnancies after the heart transplants in such women. Pregnancy risks must be divided into risks for a mother and for a fetus. The pregnant woman after the heart transplant is most endangered by hypertension and further to that there is a danger of implant rejection, infection, as well as premature delivery. The fetus is most endangered by infection and premature delivery. A majority of newborns are delivered with a low delivery weight. The abortion rate is relatively low compared to a number of successful deliveries. In the event that a woman desires to get pregnant, she is always informed of the risks associated with the pregnancy after transplant. The risks, which endanger a mother and a fetus, can be decreased to a minimum through a well monitored pregnancy. The thesis can be used for increasing awareness of women, who want to get pregnant after heart transplant.

The comparison of some hemodynamics parameters, obtained by two non-invasive measurement methods - echocardiography and impedance cardiography (HOTMAN System) by patients with systemic hypertension

Echocardiography is an investigation of heart via scan. This enables to intend the moving and the locality of heart structures via scan pulse waves which are repulsed with acoustic interfaces. Impedance measuring of the thorax hemodynamics is based on changes of electrical impedance. These changes happen mainly because of the heart function. By its rhytmical function the heart periodically changes the conditions of blood flow through all vessels. By this we can explain periodical changes of impedance with frequence of heartbeat which we can observe by measuring. In this work I describe the comparison of two noninvasive investigation methods {--} echocardiography and impedance measuring of hemodynamics (Hotman System). We can use both of them to examine patients with high blood pressure (hypertension). My measurings are based on hemodynamics concept of hypertension. I did not take into account the actual treatment of patients. It is reviewed only patient´s actual situation and values of EF (ejection fraction) and CI (cardiac index) are compared in the same time by the same conditions.

Nowadays the number of people with hypertension is growing. Hypertension means permanent raise of blood pressure above 140/90 mm HG. Hypertension is dangerously disease, which could lead to death and which could evoke another dangerously death diseases. As a treatment of hypertension is encouraged healthy life style and drugs antihypertenziva. Aim of my work is identify to most prescribed antihypertenziva and their most prescribed sub group and overall consumption of antihypertenziva in monitored regions. I gained data of region Nymburk in years 2002 up to 2007 and of regions Kolín and Mělník in years 2004 up to 2007. These data are originally from Nymburk's branch office of VZP.
Phenomenon non-compliance in adult patients of cardiology ward.

The aim of the bachelor work is to analyze the rate of occurrence of the non-compliance phenomenon in adult patients treated at the cardiology ward at the Hospital České Budějovice a.s. The information for processing the work I have obtained through an anonymous questionnaire to patients. In the theoretical part, there is explanation of the non compliance phenomenon, hypertension and the importance of communication. In the practical part, there is evaluation of the obtained information and verification of the set hypotheses. At the end, there is my own conclusion of the achieved results.
