The System of health service funding of Czech republic and Germany

The main aim of this bacholor work is to compare the system of health care financing in Czech Repulic and Germany. Both countries try to find a system of financing that would provide primary health care for everybody on one hand and would not increase tax expenses for it on the other hand. It is not an easy task to find such a method of financing therefore the system is evolving constantly. Both of countries are members of the European Union and the financing of of their health care systems is based on the Bismarck model of financing from the public health insurance.The public health insurance covers more than 50% of the health care expenses in both of countries. These countries have got other source of health care financing as well. The expenses relative to GDP in % and the expenses per citizen are different in these countries.We can say that each state has its own original system of health care financing. The system itself will be set up in each country as a choice of the citizens.The system is good if it satisfies the requirements of citizens. All of the above information was drawn mainly from books, appropriate Czech laws, Fifth German Social Code Book, a web portal of Czech Statistical Office and a web portal of Institute for Health Information and Statistics in Czech Republic. Additional sources were Internet resources, that provide information about system of health service financing in Czech republic and in Germany. There are suggestions how to provide more justice for the patients in Czech republic at the end of this work. There are also proposed measures for extra funding needed by the Czech health care at the end of my bachelor work. These two sections are problematic in the Czech health care and it is necessary to solve them as soon as possible.

Regulatory Fees at Children´s Department in Klatovy Hospital The bachelor thesis deals with effects and impacts after an introduction of regulatory fees in the health service. An aim of the thesis was to ascertain effects of the introduction of regulatory fees at Children´s Department in Klatovy Hospital. First I investigated how the fees influenced an exploitation of the hospital care, and then I investigated whether the fees influenced a possibility of parents to accompany their children during the hospitalization. In the theoretical part I deal with basic definitions of terms related to the issue of regulatory fees. These include concepts of a health, an illness, a health policy, health systems, a structure of the health system in the Czech Republic and a definition of the regulatory fees, their types, an importance of their introduction, a protection limit, effects of the introduction and a consumption of the health services after the introduction of the fees. In the practical part I attend to a description of objectives and hypotheses, a research methodology and an interpretation of results. The research part was realized by using a quantitative form of the research, I used a method of interviewing, a questionnaire technique. As an auxiliary method I chose a document analysis {--} secondary data analysis. The research sample consisted of the parents accompanying their children for an examination at an outpatient clinic and parents accompanying hospitalized children. The research results showed that regulatory fees do not influence the possibility of parents to accompany their children during the hospitalization and although parents say that in case of repeated hospitalizations the fees could influence this possibility, they continue to accompany their children during their hospitalization and on the contrary, there is an increasing number of the accompanying. The research also showed that at Children´s Department a use of First Aid Medical Service (Baby Call) decreased after the introduction of the fees and thus it was not further misused. On the contrary there was no decline in the number of hospitalized patients. I suppose that this bachelor thesis can be used for a better orientation in this new area of the health care charges and for an outline of real impacts which were brought into the health care in the first two years. Just because of the relatively short period of the existence of the fees, I would recommend to continue monitoring the impact of the charging and to follow any further changes in legislation.
Vietnamese patient in the Czech health system

Vietnamese minority has been living in the territory of the Czech Republic already for several decades. It is discrete, flexible and has its closed community. It is clear, however, that it is not easy for Vietnamese to incorporate in society of other country. The reasons are a different culture and language barrier. All this is also valid in the environment such as medical facility which can have a stress effect on an appropriate individual. Several tens thousand Vietnamese have currently been living in our territory. Therefore there is a high probability that a citizen of Vietnamese nationality can become a client in any of Czech medical facilities. The aim of this work was to elaborate specialities of nursing care which members of Vietnamese minority wish to respect. Before the beginning of the research we defined 4 research questions. Research question No. 1: Do members of Vietnamese minority use health care in the Czech Republic? Research question No. 2: What specifics do members of Vietnamese minority have in connection with hospitalisation? Research question No. 3: How do members of Vietnamese minority understand situation when a member of their family becomes sick? Research question No. 4: What do members of Vietnamese minority expect from a nurse? We answered these questions in the research part in the qualitative form of research using the method of an interview. The interview contains in total 138 questions and as the base material for its composition served the conceptual models of M. Gordonová, M. Leiningerová and the model of J. Gigerová and R. Davidhizerová. It also contained a model situation which led the respondents to the imagination of themselves being a patient/client at hospital. The interview was fully anonymous and in Czech language. On the basis of the data obtained there were created case reports from which categorisation tables and diagrams with the most important research data were made. It was found out from the results that members of Vietnamese minority expect almost the same like most patients/clients using Czech health service. However, there are areas such as family or catering in which they have their specifics. If nurses know these areas they are able to provide a multicultural/trans-cultural care.
Analysis of trim of the department of health service of SR for solving crisis situations

The diploma thesis deals with the state of readiness of the health service of the Slovak Republic for solving crisis features connected with mass infliction of the population. The crisis features are mostly connected with risk to peopleś health and lives therefore the health intervention, which increases the demands made on support of usual health care, seems to be necessary . The health service shall be ready to solve them as soon as possible with use of all necessary force and means. The thesis analyses the level of readiness of the health service for managing emergencies connected with risk to health and lives of a larger number of people. It expertises legal surroundings of health support of crisis management, it explains the basic terminology, evaluates the level of health provision of rescue works and views the impact of reorganisation of the health service on crisis health support. It complex analyses the influence of the whole real level of the health service of the Slovak Republic on solving crisis situations. The diploma thesis proposes pursuant to the performed analyses organizational measures for the better quality of the process of solving crisis features and for increasing its effectivity.
The use of social and health services for people with multiple sclerosis

Multiple sclerosis cerebrospinal is a chronic inflammatory disease of the central nervous system which leads to demyelination of nerve fibres or to their direct loss. Multiple sclerosis is the most common neurological cause of chronic disability in young adults. There is no known effective cure for this disease. It is only possible to reduce the progression rate of the disease by recently available medication and their combinations. Social and health services bring significant benefit to life of people with multiple sclerosis. Health services are important not only for medical treatment but also for support because they try to improve or preserve physical condition and affect also mental condition. Social services become more important as the disease progresses when people are no longer able to cope with routine everyday activities. The objective of my thesis was to suggest suitable health and social services for people with multiple sclerosis. Furthermore, to find out which of these services are used by ill people. The hypothesis of the thesis was: Some types of social services for people with multiple sclerosis are not quantitatively provided unlike health services. In my thesis I used quantitative research, interviewing and the questionnaire method. Questionnaires were filled in by adults with multiple sclerosis in selected organisations {\clqq}Roska`` in the Czech republic. During the research I discovered problems with insufficient awareness of social services and not using contributions and allowances for social services. On the contrary, the results of the research imply that health services are being used. I assume that the hypothesis was confirmed. I find the contribution of the thesis in the fact that I suggested recommendations for suitable health and social services for people with multiple sclerosis. After completion of local contacts, this suggestion could be used as an informative brochure which would be available to people with multiple sclerosis.

This thesis is focused on health service, while the first chapter briefly describes the nonprofit sector and subsidized organizations, which is the most usual form of the health facilities. The next two chapters compare the establishment, management and accounting of public non-profit institutional health facilities to private health facilities. It is also mentioned the issue of financing of medical equipment and its evolution over time. The practical part is made of management of the Clinic of Dr. Pírek in the years 2006 to 2008.
Health Service Financing in the Czech Republic and reformative efforts of system stabilization

The subject of this thesis describes system of health service financing in the Czech Republic in connection to the actual situation. After introducing current problems there is an analysis of reformative efforts of system stabilization. The final part of the thesis is focused on the evaluation of reformative attempts and it is compared with selected health services in the Europe.
