Case Study of physiotherapy treatment of a patient after acute brain stroke

Title: Case Study of physiotherapy treatment of a patient after acute brain stroke. Objective: To officialy to confirm the ability to work well with patiens after a stroke. To gain the needed theoretical knowledge about the given issue and to utilize the practical physiotherapeutic skills. Method: The first part of the bachelor thesis is theoretical. It describes the origin, risk factors and consequences of a stroke. Further described are typical clinical symptoms, possibilities for treatement and technical uses od physiotherapy. The thesis is also dedicated to the subluxation of the paretic arm and the neglect syndrom. The second part contains casuistry of the chosen patient after the ischemic stroke with left - handed symptomatology. Results: Improvement of the walking stereotype, reduction in the painfulness of the paretic arm, increased strength in the paretic lower arm and improvement of the exterior and deep perception. Conclusion: The use of physiotheraphy on a patient after a stroke appeared to be very important, due to partial return of damaged functions. Problem of painful shoulder in hemiparetics patients

This bachelor thesis is focused on a painful hemiparetic shoulder as a frequently observed complication in patients suffering from stroke. Basic information about stroke, anatomy and kinematics of shoulder joint is included in a theoretical part of the thesis. The largest portion describes a pathogenesis of painful hemiparetic shoulder, its prevention and commonly used physiotherapeutic approaches. The practical part of the thesis consists of two case reports of patients with stroke suffering from painful hemiparetic shoulder. In both cases, a combination of physiotherapeutic approaches applied in ten cycles over a period of 4 to 6 weeks was used. To prove an effectiveness of the therapy, I used kinesiological assessment as well as a faces pain scale for an evaluation of shoulder pain. To assess a mobility of the shoulder, I used a goniometry and functional tests for upper limb. Results of the therapy are described in a discussion part of the thesis and indicate that the combination of physiotherapeutic approaches may decrease pain as well as improve the mobility of affected shoulder. Title: Case study of physioterapy treatment of a patient after hemoragic stroke with left hemiparesis. Objectives: The aim of the thesis is to obtain academic findings about stroke concerning anatomy, pathology, therapetuic approaches and methods which can be utilised in cases of stroke. The specific part focuses on the therapeutic care of a patient after hemorragic stroke which was taken during a month practice at Rehabilitation clinic Malvazinky, Medditera s.r.o. Summary: The theory of the thesis discusses anatomy and pathology of human brain in terms of strokes. There is a brief mention of ischemic strokes and a detailed elaboration on hemorraghic strokes. Furthemore, the same part of the thesis epitomizes the risk factors and diagnosis of strokes. There is also a detailed treatise on the physioterapeutic care of stroke. The practice of the thesis introduces a case study of physioterapy treatment of a patient after hemoragic stroke with left hemiparesis. CASE STUDY OF PHYSIOTHERAPY TREATMENT OF A PATIENT AFTER STROKE Objectives: Theoretical analysis of after stroke management and elaboration of physiotherapy treatment schedule in the form of an after stroke patient case study. Methods: This work was done with patients consent within an uninterrupted practice at Central Military Hospital in Prague at the Department of Physical Medicine and Rehabilitation. The practice was held from 9th January to 3rd February. The work is divided into two main sections - a general (theoretical) and a special. In the general section is researched the theoretical background for status post hemorrhagic stroke. The general section includes basic characteristics of the illness, etiopathogenesis, incidence, diagnostics, treatment and options of physiotherapy methods concerning the illness. The special section contains the after stroke patient's case study, the review of proposed therapy and the therapy effect evaluation. Results: The patient's condition evolved during the treatment. The patient developed better muscular strength and joint movement, the swelling was reduced, right hand side of his body and limbs was strengthened and his overall stability and gait was improved. Title: Case study of physiotherapy treatment of a patient after the ischemic stroke Objectives: The main aim of this study was to verify my ability to work with the patient, his examination, making conclusions, proposed treatment and its subsequent implementation. Other aim was mapping of the literature on the subject. Methods: The first part of my bachelor thesis is theoretical and it is aimed on stroke. There are described anatomy of cerebral artery, knowledge about stroke, its clinical symptoms and treatment options in this part. The description of spasticity with its influence on therapy and its treatment and the techniques used in physiotherapy are also there. The Brocca aphasia is described in the last section. The second part is the case report of a patient P. V. with diagnosis: ischemic stroke with dextral symptomatology. This special part is based on two weeks' work with the patient in the hospital (Ústřední vojenská nemocnice - Vojenská fakultní nemocnice Praha). Title: The effect of regular training in Locomat system changing the stereotype of gait of patients after the cerebrovascular accident Objectives: The purpose of this thesis is to evaluate whether it is appropriate to use the Locomat system in therapy of gait of patients after the cerebrovascular accident, what is the effect of regular training of these patients in the Locomat system on their stereotype of gait and whether the training in the Locomat system of patients after the cerebrovascular accident is more effective in comparison with other physiotherapeutic practices. Methods: The thesis was processed in the Kladruby Rehabilitation Centre on 14 probands, who were divided after the entrance examination in order to form two equivalent homogenous groups. The first group of seven members was experimental, the second one was control. At the beginning of the study the rate of gait disorder was specified at each proband using standardized method of Wisconsin Gait Scale, they also underwent 10 - Meter Walk Test and 6 - Minute Walk Test. Afterwards all the probands absolved a three-week lasting standard regime of rehabilitation.
