Case study of the physiotherapeutic treatment of a patient with total knee replacement

This thesis deals with problematics of total knee arthroplasty (TKA) in a complex range. It is divided into two parts - the general one and the special one. The general part of the thesis targets on theoretical knowledge of the anatomy and kinesiology of the knee joint, knee arthritis and TKA as the most performed method of therapeutic intervetion in advanced stages of knee arthritis. Secondly, the special part contains the case study of the physiotherapeutic treatment in patient after TKA. Non-invasive methods and approaches in a range of the knowledge of the bachelor's studies on the Faculty of Physical Education and Sport, Charles University, were used during the pshysiotherapeutic treatment. Improvement of the patient's clinical status in almost all sections was the result of the physiotherapeutic intervention.

Title: The case study of the physiotherapeutic care of a patient with total endoprothesis of a knee joint Aim: The purpose of this bachelor thesis is the processing of a case study of the physiotherapeutic care of a patient after total knee replacement during an internship in the ÚVN hospital. Summary: This thesis is divided into general and special part. The general part sums up anatomy, kinesiology and biomechanics of the knee joint. Apart from that, there is some information about gonarthrosis, indications and contraindications towards total endoprothesis of the knee joint. Last but not least we look at the specifications of rehabilitation regarding this diagnosis and used physiotherapeutic methods. The objective of the special part is a case study of the patient. It consists of complex kinesiologic evaluation, therapies during the patiens stay and the outcomes of the rehabilitation. Nursing specifics in the care of clients after high correction osteotomy of high proximal tibia over time.

Osteotomy means cutting a bone using a saw most often. It is used to adjust axial deviation, prolongation, or shortening of the bones. Over time, when there were changes in the development of the surgical technique of corrective osteotomy, changes in nursing care also occurred. This breakthrough concerns not only nursing staff, but also clients in particular. Because of the development of the surgical technique, the hospitalization time shortens and if the client does not perform a physically demanding job, where is a heavy load on the operated lower limb, the treatment time is reduced. This leads to a faster return of clients to normal life and employment. Nursing care becomes less demanding for staff than the older surgical method. The bachelor thesis is a theoretical work, which deals in the introductory part with knee joint anatomy, knee joint examination methods, explanation of the concept of gonarthrosis and the way of treating this disease. This part is followed by history of osteotomy and an explanation of the surgical method. An important part of the thesis is the description of nursing care of clients with high correction osteotomy as time goes by, which includes preoperative preparation, postoperative care and, last but not least, rehabilitation. The aim of the work is to describe, how the nursing procedures changed with the older surgical method. The second objective is to map out nursing procedures using a modern surgical method. There are the opinions of doctors and nurses of various medical disciplines, who have experienced nursing care of clients with high corrective osteotomy, mentioned. Gained knowledge could serve as a guide to nursing care of clients after high corrective osteotomy of the proximal tibia.
Economic evaluation of surgery compared with pharmacological treatment for patients diagnosed with gonarthrosis

The main purpose of my diploma thesis is the application of economic evaluation principles; costs quantification and the consequences quantification of two different approaches in the gonarthrosis therapy from the perspective of Health Insurance Company and from the perspective of the patient; furthermore the evaluation of the effectiveness and utility ratio and the final determination for more convenient option. Considered alternatives are the hyaluronic acid injection and the total knee arthroplasty realization. Effects for the expenditures at cost-effectiveness analysis are measured by WOMAC Index. Effects for the expenditures at cost-utility analysis are measured by HUI2 score system. The research includes two sections: (1) the case study of two patients and (2) the clinical trials and nationwide Health Insurance Company´s data analysis. Particularly because of the brief observation time period, the pharmacotherapy is quantified as a more convenient alternative.
The influence of pharmacological substances from the SYSADOA group on knee osteoarthritis.

Title: The influence of pharmacological substances from the SYSADOA group on knee osteoarthritis. Objectives: The aim of this work is to determine the effect of three - month usage of pharmacological substances from the SYSADOA group on the subjective perception of the difficulties associated with the knee osteoarthritis and on the reological changes in the joint. Methods: In collaboration with rehabilitation and orthopedic centres 18 probands with knee osteoarthritis in the second degree were obtained. These probands were divided into two groups - the experimetal group (including those who have taken Proenzi 3 plus) and the control group (including those who have taken placebo). With all the pacients the input clinical examination and the medical history were done. Probands had been using these preparations for twelve weeks. After this time, the changes in the subjective perception using the WOMAC test and reological changes in the joint were evaluated, using a special knee rheometer. Results: The results of objectively measured parameters clearly proved that the Proenzi 3 plus has a positive effect on the rheological properties of the knee osteoarthritis. Title: Case report on physiotherapeutic care of a patient with total left knee replacement on the left side caused by the gonarthrosis. Summary: This thesis focuses on case report of a patient after total left knee replacement. The work contains two parts - the general part and the specialized part. The general part deals with structure, kinesiology and biomechanics of the knee joint, general definition of gonarthrosis, its cause, clinical symptoms and its treatment. It also describes the gonarthrosis, indications and the types of total knee replacement prosthesis. The most important topics of the general part contain descriptions of therapeutic methods and procedures, focused on improving patient's health in the post- operation phase. The specialized part constitutes the main part of this thesis. It is processed via case report form and describes the physiotherapeutic treatment of a specific patient, who underwent a total knee replacement surgery. In the conclusion of the specialized part of the thesis, I evaluate the effects of the theraphy. Late consequences of knee injury.

This thesis deals with the issue of knee injuries and gonarthrosis. The thesis has theoretical and empirical character. The theoretical section is divided into three main chapters. The anatomy of the knee joint is described in the first chapter. Second chapter deals with knee injuries; this part is focused on the injuries to ligaments, menisci, cartilage and patella. Gonarthrosis, its etiology, symptoms, treatment and prevention, is compiled in the last chapter. The empirical section contains the results of research. The research was performed as an anonymous questionnaire study. Respondents were older than 18 years and after the knee injury. The sample consisted of 102 respondents. The aim of the empirical section was to find out what kind of injuries the clients have experienced, their attitude towards the after-treatment of the injuries, their knowledge of gonarthrosis and whether they occupy themselves with its prevention. The results were processed into charts and graphs. The research show, that the most common injury is one to the anterior cruciate ligament. A third of the respondents does not follow the treatment regimen, most frequently they fail to walk on crutches or to wear a knee brace.
