Nursing care of intoxicated patients

This thesis deal with problems of nursing care of intoxicated patients. Nowadays increasing amounts of toxic substances associated with increasing number of hospitalized people with diagnosis intoxication. Nurse work with intoxicated patients can´t be done without good knowledges about this issue as same as without providing comprehensive nursing care. In theoretical part of this thesis we paid atenttion to general description and to the division of intoxication by journeys into the organism. We mentioned history of toxicology, division of intoxication by the cause of intoxication and drug intoxication. Then we paid attention to the toxicological information center, the diagnostics of intoxication, symptomes of some intoxications, treatment and elimination methods, specific antidotes, nursing care and communication with the intoxicated patient. For this thesis we set two goals. No. 1 : To find out what are problematic areas in nursing care of intoxicated patients from nurse´s point of view. No. 2: To find out specifics of nursing care of intoxicated patients. For this thesis were also set three research questions. Question no. 1 is what are problematic areas of nursing care of intoxicated patient from nurse´s point of view. Second question refers to discovery what role has nurse in taking care of intoxicated patients. And the last research question finds out specifics of nursing care of intoxicated patients. For processing the empirical part of this thesis was used a qualitative research for which we used semi-structured interview. Respondents were 9 nurses work at the internal medicine department, intensive care unit of internal medicine and urgent care department in Hospital of České Budějovice a.s. The data gathered from the interviews were subsequently analyzed and processed into three categories. The first category are the problematic areas in nursing care of the intoxicated patient. Second category is the role of nurses in the care of intoxicated patients and the last category is the specifics of nursing care of intoxicated patient. This survey has provided answers to our research questions. Nurses sees as the biggest problem in taking care of intoxicated patients mainly their lack of cooperation and aggression and also a problem in communication with this patient. Second question, what role has nurse in taking care of intoxicated patients, respondents said that the basic role is to fulfil doctor´s orders, take blood samples for toxicological examination and educated patients. Specifics of nursing care of intoxicated patients constitued of security against further injury and intoxication, prevent further absorption of toxic substances into the organism and elimination of toxic substances from the body of the patient. From the results of this work follows that nursing care of intoxicated patients is mentally challenging for most of the nurses mainly because reasons related to lack of cooperation and aggressive patients.

Comparison of chosen nursing in prevention of immobility syndrome in a senior nursing home in Germany and the Czech Republic for lying immobile patients.

As the population is getting older there is increased number of geriatric patients. However there is also increased number of immobile seniors, who are not able to take care of themselves. Comprehensive health and social security is being provided in homes for elderly, which fulfil the role of long-term institutional care. Nurses must be trained for this highly professional and knowledgeable nursing care aimed at geriatric problems. For geriatric clients of home for elderly it´s important realization of prevention, treatment and social measures in the form of an individual approach. That these measures can be implemented it is necessary to evaluate the health status of seniors, their physical performance, self-sufficiency and mental functions in connection with the social situation and to formulate a nursing diagnosis. As a result of long-term bed rest can lead to health complications affecting all body systems, so-called immobilization syndrome. How to prevent secondary damage that threatens the patients White Theo arge already ill is focused rehabilitation nursing. Among rehabilitation resources in the prevention of immobilization syndrome in nursing practice arge included: positioning, verticalization, mobilization. Inseparable part of prevention measures is preventing health complications in case of immobile clients homes for the elderly arge suitably selected positioning aids, mobilizing resources, well-equipped bed with antidekubitus system. In the thesis there were two objectives. To Compare keeping preventive measures which prevent the formation of pressure ulcers and muscle contractures in nursing care for immobile patients in the Czech and German retirement homes. To map out the possibilities of aids in the prevention of complications in immobile clients in the Czech and German retirement homes. Bachelor thesis has been made by using qualitative research. For collection of data was used a method in-depth interviews with four Czech and four German nurses. This method involved non-standardized interviews through open-ended questions that were recorded by dictaphone and transcribed verbatim. The data analysis was carried out by hand coding or using a pencil and paper. By analyzing the data four categories were created, each divided into the home for the elderly in the Czech Republic and the home for the elderly in Germany: 1.Nurses knowledge ,2.Implementation of preventive measures , 3.Equipment for immobile clients, 4.General knowledge related to immobile clients. Research survey found out that there is not a big difference in adherence to preventive measures to prevent the formation of decubitus and muscle contractures in nursing care for immobile patients in Czech and German homes for the elderly and comparable are also the possibilities of using tools to prevent complications in case of immobile clients. The biggest gap in nursing care for clients is bad knowledge of contractures in both homes for the elderly. Also keeping documentation and records relevant to the implementation of preventive measures in the Czech home for the elderly is not satisfactory. Mobilization record is missing and positioning record should be more detailed. We therefore propose the creation of positioning and mobilization recording modeled according to the German retirement home and handing it to the leadership of retirement home in the Czech.

The situation in information and the mistakes analysis concerning the usage of the inhalation systems with the patients suffering from chronic lung illnesses.

The thesis titled "The Level of Awareness and Analysis of Mistakes in the Use of Inhalation Systems in Patients with Chronic Pulmonary Diseases" maps the present problems of the care of patients with chronic diseases with bronchial obstruction whose medication contains some inhalation preparation. This thesis is divided into a theoretical and an empiric parts. The theoretical part deals generally with the issue of the care of patients with chronic pulmonary diseases, with focus on patients with chronic obstructive pulmonary diseases, who formed the biggest part of the research sample of patients. It also focuses on their treatment and the principles of proper administration of medicines by means of inhalation systems. The empiric part focuses on the patients' awareness and skills and on their attitude to their own skills in these terms.

Komplexní využití biomasy v jehličnatých mlazinách na malém lesním majetku

This bachelor thesis deals with the evaluation of silvicultural treatment educational interventions in coniferous spruce young forest stands on small forest property. On private spruce plots coppice pruning was done, which indicated the amount of carved individuals, their dimensions and quality. Material from first silvicultural treatments is possible to use for small landscape constructions, hunting equipment, decorative and technical brushwood, Christmas trees production and finally as a source of energy.

SPECIFICS OF PHYSIOTHERAPY OF PATIENTS TRANSFERRED AFTER ORTHOPEDIC OPERATIONS DIRECTLY ON THE SO-CALLED SPA BUNK

This bachelor thesis addresses the specificity of physiotherapy of patients, transferred to a so-called spa bed, after orthopedic surgery. Among the most performed orthopedic surgeries are the total hip replacement and the total knee replacement. Arthrosis is the most common reason for the implantation of joint replacements. It is a painful and life-aggravating disease which has a tremendous impact on the patient's quality of life. In order to lower the risk of the patient having to take a revision surgery of joints, a promptly initiated rehabilitation is necessary. It has to be intensive and professionally conducted. The present time-tested trend of rehabilitation treatment is a prompt comprehensive care, which takes place in a spa resort in the form of a direct transfer "from one bed to another", i.e. the patient is transported directly from the hospital to a spa resort. Often, patients would get to a spa resort with delay, while already having intercurrent problems like vertebrogenic and other problems. This thesis is divided into a theoretical part and a practical part. The theoretical part includes the anatomy of the hip joint and the knee joint, the description of the joint replacements including the indications and the contraindications, the specifications of particular types of hip and knee joint replacements and possible complications after surgery. The most important topics of the theoretical part are stated in the last chapters. The individual therapeutic rehabilitation methods, which have a positive impact on the health status of the patient after surgery, spa therapy and physical therapy included, are discussed and described in those chapters in detail. A qualitative research strategy and a personal case study technique (case history), with the use of a secondary data analysis, were used in the practical part. This strategy was carried out on three chosen patients, who after being discharged from a hospital after surgery had continued their treatment in a spa resort, i.e. a so-called direct transfer from a hospital bed to a spa. The research was conducted in Berta's spa resort in Třeboň, where the patients were transferred on the tenth day after surgery. On their first therapy session, all patients had their personal history taken and data from their medical documents was added. The practical part also includes a pre-therapy and a post-therapy kinesiologic analysis which included visual inspection, palpation, goniometric and anthropometric examination of the knee and hip joint area. Furthermore, a muscle test and an examination of shortened muscle groups related to the knees and hip joints, created by Janda, were conducted. The gait, stereotypes and neurologic functions were also examined. Following the evaluation of the pre-therapy data, the patients were introduced to the short-term therapeutic plan. The results were acquired by comparing the pre-therapy data and the data which was obtained during the last therapy session from the post-therapy kinesiologic analysis. The rehabilitation in the spa resort consisted of different treatment procedures each day and the patients were getting medical attention and care from doctors, nurses and physiotherapists. In each therapy, we focused on removing the problems at hand and only after that we moved to correcting other imperfections. After the completion of the therapy, the patients stated a decline of pain and an improved balance of the extremity and the whole body. The data of the chosen patients shows improvement of muscle strength, decreasing of swelling and widening of range of motion. These results were achieved thanks to a comprehensive rehabilitation and a positive attitude of the patients.

Verification of knowledge about self-management in patients with heart failure

Congestive heart failure (CHF) is a disease with increasing incidence, which requires an adequate and costly management. Repeated hospitalisations due to decompensated CHF contribute to disease progression and herald dismal prognosis. An important goal of heart failure clinics is reduction in number of hospitalizations due to decompensated CHF. The main approaches to reach this aim seem to be effective diagnostics and treatment of CHF, and importantly, education of patients and their families. We aimed to evaluate knowledge of CHF patients regarding self-care in secondary prevention of CHF. We constructed three hypotheses and three research questions to reach the goal of the project. The theoretical part described recommended life-style interventions, pharmacologic and non-pharmacologic CHF management, and specifically, the role of nurses in education of patients with CHF. The practical part described the methods of analysis, the study group and the process of evaluation. To fulfil the study goal, we performed collection of data using qualitative and quantitative methods of research. The qualitative part of the research project included a non-standardized questionnaire for patients with CHF. A total of 260 questionnaires were distributed in six heart failure clinics. 183 completed questionnaires were returned and analysed. The obtained data were evaluated using descriptive statistical methods. Pearson´s chi-test and Student´s t-test were applied to test the hypotheses. Collection of qualitative data was performed using the technique of a semi-structured interview based on pre-prepared questions. The study respondents included general nurses from out-patient heart failure clinics. Six of nine contacted nurse respondents agreed to participate. Answers were recorded on Dictaphone. Data were analysed using the method of "card display". The quantitative part tested the following hypotheses: 1. CHF patients know symptoms and signs of decompensated CHF. 2. CHF patients follow the life-style recommendations. 3. CHF patients perform daily self-monitoring of their weight, blood pressure and pulse. Our study revealed insufficient knowledge of patients regarding symptoms of possible CHF decompensation. This finding was alarming because suboptimal knowledge of symptoms of decompensated CHF may cause inadequate response of the patient and may delay initiation of treatment. Assessment of compliance with life-style measures revealed that most patients did not use tobacco products and had a limited alcohol consumption. Most individuals reported a regular use of heart failure medication and knew the timing of the next follow-up in their heart failure clinic. Life-style measures included also some dietary restrictions modified by the current nutritional status. Respondents preferred recommended food over inappropriate foodstuffs, however, they did not follow neither the salt restriction nor the optimal frequency of meals. Most respondents did not report a regular exercise A significant ignorance was found in the process of self-monitoring. Most respondents did not monitor their body weight, blood pressure and pulse. The qualitative assessment evaluated the following questions: 1. Do we educate the CHF patients appropriately? 2. Are we able as nurses to pass correct information to CHF patients? 3. How is the process of education implemented in practice? Based on the above mentioned methods, we found that the process of education is a domain of doctors. Nurses have only a limited role in education of CHF patients...

Usage of IT tools while organizing ESN CZ National Platform

Student organisations offer many opportunities to try out numbers of real projects with real budgets and deadlines. However, the high turnover of members in these organizations and imperfect knowledge transfer between the generations slow down the improvements and realization of different projects. This diploma thesis describes planning, preparation, implementation and evaluation of a specific project within the Erasmus Student Network Czech Republic (ESN CZ) which aims to meet representatives of all student organizations involved in ESN CZ. Through the description of the event, introduction to project management and their interconnection reader gets a comprehensive view on the issue of organizing the ESN CZ National Platform (ESN CZ NP). A following review of the IT tools available on the market, their evaluation and selection of a few specific applications offer options in IT project management support. Selected IT tools are demonstrated in real situations, including the recommendation for other useful functionalities. The diploma thesis is dedicated to project management from the perspective of a student organization dealing with the preparation of specific event in the context of the general phases of project management, creating manual for future coordinators ESN CZ NP.

Software testing in Microsoft Visual Studio

In software development process, there can be misunderstandings, communication problems between individual members of the development team and mistakes when writing code or designing applications. These problems need to be resolved before the application is released into the production environment. To discover these errors it is necessary to properly test the software product. All these activities are engaged in discipline called Software Quality Assurance (SQA), which is an integral part of the software development process. The thesis discusses this discipline and procedures for software testing using the Microsoft Visual Studio 2008. The main objective of this work is to create comprehensive methodology for automated testing of web and Windows applications in Microsoft Visual Studio and to verify that procedures in practice. The reader will learn the theoretical basis of software testing at the beginning of the thesis. In next chapter, there are described various features that Microsoft Visual Studio 2008 offers for software testing. Following chapter describes the applications testing in IBM Rational Functional Tester, which is suitable for Windows applications testing (Microsoft Visual Studio 2008 does not support this kind of tests). In other part of the work -- Case Study -- the reader will learn web and Windows applications testing. Case study can serve as a basis for further methodological manual for teams engaged in software testing. The usage of instruments for testing is explained in real examples. At the end of the work there are set out recommendations for the team collaboration using Microsoft Team Foundation Server, and there are also compared the testing instruments and reviewed functions that will come up with Microsoft Visual Studio 2010.

Fair Trade - Its Functions and Impacts on the Trade with Thirld World Countries

The aim of this work is to provide a comprehensive view on the issue of trade with Third world countries using the concept of fair trade. First, the discussion focuses on how fair trade should look like and how it should be different from the free market. Furthermore, this work deals mainly with the functions and effects of Fair Trade licensing, which sets out properly against the aforementioned interventionist concept of fair trade. After unloading the basic characteristics of licensing, its evolution and structure of the relevant licensing authorities, an economic analysis of the effectiveness of Fair Trade licenses in achieving the set goals follows. Possible impacts of Fair Trade on the society as a whole are discussed as well. The main aim is to answer the question whether Fair Trade is really the right approach to trade with poor producers from Third world countries and whether it is really necessary to abandon the free market conditions to achieve fair treatment and rewarding.

Complete scenographic project for Charles Gounod's FAUST

My bachelors work is focused on a study of different costume approaches to Fausts theme. I compare selected foreign inscenations since the end of eighteen century to present. Selection of performances was made upon the existence of picture materials and theatre reviews. I tried to reflect the change of costume tradition and discover what parts of original costume remained unchanged till now. The best example for this observation was costume of Mefistofeles. In second part I?m dealing with treatment of my practical assignement ? costume and set design work for Gounod opera Faust and Marketa.
