Expectations of the Parents from the Provided Social Services in the Kaňka o.p.s. Tábor

On the field of social services devoted to children with health handicaps and their families there have been recently established lots of new organizations. So it has been becoming more and more up to date topic. The aim of this thesis, named Expectations of parents about the social services provided by Kaňka o.p.s. Tábor, is to find out actual information whether the parents´ expectations identify with providing of these services. Within this thesis there has been defined the following research question: Do the parents´ expectations correspond with the social services provided? The thesis has been divided into the theoretical and empirical part. The theoretical part has been devoted to the sphere of family, the family surroundings which has been the foundation stone of the life of the children with health handicaps. It is also devoted to parents of children with health handicaps in context of social services provided, characteristic of these families according to the law No. 108/2006 about the social services. There has been specified the social service of early care, personal assistance and day care centre offered by the Kaňka o.p.s. Tábor institution. There have been also mentioned particular therapies realized for the children with health handicaps. The therapies seem to be meaningful for these children at least because the children calm down, entertain, relax and develop during them. There have also been mentioned variety of special aids often used during the activities with these children, which make the care easier not only to the families but to all persons taking care of these children. You can also find there some non-profit institutions in the southern Bohemia region, which provide similar services as Kaňka o.p.s. Tábor does. And last but not least the theoretical part is devoted to the term Expectation itself - what do parents expect of the social services mingled with individual planning of social services, thanks to which the particular organization can be better joined in the process of expectations of the social services. The empirical part deals with the institution Kaňka o.p.s. Tábor with respect to the social services provided to the families of children with health handicaps, who attend this institution. There have besides other things been engaged the other services provided by the institution Kaňka o.p.s. That means therapeutic activities, free time activities, rehabilitation integration stays, one-time activities etc. This part has been enriched with the basic case history of individual clients whose families were part of the research. According to these case histories it is more obvious which clients with a health handicap really attend the institution Kaňka o.p.s. In view of the determined aim there was chosen the qualitative research strategy, the method of asking questions, and the technique of half-structured dialogue. The research was realized in the Kaňka o.p.s. Tábor institution and there were involved twenty respondents. For this research there was chosen the method of intentional choice, that means ten parents of children who attend the Kaňka o.p.s. institution and use the social services there and ten parents of children who also attend this institution and not only use the social services, but also the compulsory education. In the thesis there were determined the spheres of questions relating to the given issues of expectation of the social services provided by Kaňka o.p.s. Tábor. During the data processing, systematization and interpretation there was used selective coding within the method of the contents analysis. There were created categories which facilitate the organization and classification of the data. The result of the thesis has shown that in the research there were involved predominantly women in the age from 40 to 50 and most of them were mothers of the children.

Cooperation of a Canistherapeutic Team with a Multidisciplinary Team in the Center Arpida in České Budějovice

This thesis deals with the cooperation between the canistherapeutic and multidisciplinary team at Arpida, the registered center for the rehabilitation of disabled people.Animal assisted therapy and preferably canistherapy becomes an integral part of the therapy in a variety of social services, we can see it as a part in educational institutions and even medical devices. It is proved among the many studies that canistherapy is based on the interaction between man and dog and also affects the overall quality of life.Since the beginning of the targeted use of animals in therapy specialists have gained a lot of experience, but in the field of canistherapy they still see the lack of link between services, as well as other shortages among the cooperation with other experts.The author in the theoretical part of the thesis deals with description of parts of animal assisted therapy.She outlines the historical origins of therapy using animals, describes a division, methods and the forms used.The next chapter is devoted to the therapy using a dog. It describes the beginnings of canistherapy in the Czech Republic, conditions for practicing the professional kind of therapy and targets of canistherapy.Because the author sees the importance of linking treatment with the help of a dog within other therapies, the following chapters is devoted to a coordinated rehabilitation, its resources and members of the multidisciplinary team. The thesis also mentions the concept of circular care.As an extension of the circular care, the social worker stands on top. One chapter is also devoted to the quality of life. The last chapter is devoted to a brief description of the Arpida center, a multidisciplinary team and canistherapeutical teams which attend the Arpida center.This thesis continues with practical part where the author describes the aim, research questions and research methodology. The aim of the thesis was to determine the extent of cooperation between canistherapeutical and multidisciplinary team in the center of Arpida.For research in the thesis a qualitative strategy was used, meaning an interviewing method and a technique of semistructured interview.The multidisciplinary team experts in the Arpida centre, and the clients´ parents regularly visited by the canistherapist became the research group. It was an occupational therapist, physiotherapists and special needs teacher among the experts. Two main research questions were established: HVO1: In what multidisciplinary team expert´s care is the client using canistherapy? HVO2: Does their professional activity go before or does it follow the canistherapy? The interviews were recorded on a Dictaphone and then transcribed into written form. The results are published in the form of open coding, where the author has defined categories. The research results are divided into chapters according to the categories that have emerged from open coding. The results show that collaboration between multidisciplinary and canistherapeutical team does not exist. Thanks to the openness of informants and their answers, the author has found out that cooperation among these experts is possible. The interviews identified areas where cooperation could begin and with such experts would be the best. Results are supplemented by a table which categorizes streamline and SWOT analysis. The author, in the discussion conclusion has applied her knowledge of practicing professional canistherapy and has proposed recommendations for practice and cooperation in the fields of physiotherapy and occupational therapy.It is very easy to see from the research conclusion that cooperation is possible, and therefore the author would like to recommend the work to a multidisciplinary team from and parents of clients, who see canistherapist regularly. This thesis could serve as an inspiration for canistherapists who cooperate with other professionals and want to elevate their volunteer activities on valid therapeutic or rhb method

THE ISSUE OF PROVIDING HIGLY SPECIALIZED CARE TO PATIENT WITH INTRA-AORTIC BALLOON COUNTERPULSATION

This thesis is focused on the issues linked with providing highly specialized care to critically ill patients with intra-aortic balloon pump (IABP) in intensive care units. IABP helps these patients overcome period of acute phase of heart failure or overcome period of time necessary before definitive intervention or surgical solution. A nurse has an irreplaceable role from the very beginning of patient's preparation before insertion of mechanical support (psychological as well as physical), assumes the assistance and instrumentation during insertion of heart support, observation during counterpulsation to psychological support of the patient leading to disconnection (weaning). This thesis is divided into two parts. Theoretical part is organized in several theme units focused on description and method principles, main indications, contraindications, and complications occuring with counterpulsation. The next part is dedicated to nurse's role in the problematics of saturation of bio-psycho-social needs of IABP patients. And the last part characterizes the specifics of intensive care. The core of practical part was qualitative research as per defined goals. The main objective was to map demand for quality nursing care in connection with IABP. This aim was reached through definition of four sub-aims and five research questions. The research questions were focused firstly on knowledge preconditions of nurses and problematic areas of care for IABP patients. Secondly, research effort was focused on the area of insufficient needs of patients in the bio-psycho-social area and on quality of information provided to patients. Imaginary centerpiece of research investigation consists of analysis of case studies of patiens, graphic visualisation of their thought map was used to survey the specifics of nursing care. For the evaluation of needs and mapping of patient's awareness the technique of individual half-structured interview with open questions was used. Same was used with nurses for mapping of problematic areas of nursing care. At the same time casuistry was formed as classical method of description followed by analysis of nursing case. Its meaning was to clarify optimization of means, processes and nursing interventions, i.e. mapping the specifics of nursing care of IABP patients. Research group consisted of patients selected intentionally with regards to the research problematics. For a complex conseption of the research the group for qualitative investigation consisted of four patients of cardio-surgery unit and one patient of coronary unit. Research investigation was realized in coronary intensive care units in České Budějovice hospital and Faculty hospital Plzeň and in Cardio-surgery unit of FN Plzeň. Research group for investigation of nursing problematic was formed by nurses of coronary units and cardio-surgery units of above mentioned hospitals. Eight nurses participated in the interviews. The selection of nurses was finalized only after the research topics were developed in detail. At that time sample selection was not bringing any new information any more, hence theoretical saturation of factual reality was reached. Based on analysis of research results within the goal defined and focused on the level of nurses' theoretical knowledge of nursing patients during IABP therapy it was observed that even though nurse's knowledge is sufficient, it is also significantly inconsistent. Relatively vast reserves were found especially in the area of communication with patients, hence in complex care for patient's psychological state.

Analysis of eating seniors with diabetes mellitus II.type

This thesis deals with the analysis of eating habits of seniors with Diabetes mellitus type 2. Many seniors with Diabetes mellitus type 2 are introduced to a proper diet either through their Doctor or a nutrition therapist. This diabetic diet assists them in maintaining proper blood glucose levels and helps prevent a variety of diabetic complications. Many diabetics, who understand their illness, begin to make changes in their lifestyles that will help them in the future. More thought is put into their lifestyle and how to improve their quality of life. For example, many diabetics become more active, begin to be more diligent with their dietary choices and discover a new meaning of life.

The employment of medical assistants in acute nursing care

The thesis is aimed at to what extent nursing assistants are used for providing acute nursing care and whether nursing assistants are interested in providing nursing care, and, in addition, it focuses on what the attitude of the management to employ nursing assistants is. Moreover, it deals with the historical development of the nursing education until today and with the impact on education of non-medical staff after the Czech Republic joined the European Union. It deals with legislation and decrees in force laying down conditions of work of non-medical staff {--} defining professions and their members, defining the scope of work (primary, secondary, tertiary care), defining requirements for obtaining a certificate of professional competence, defining lifelong learning, defining job competences etc. The aim of the bachelor thesis was to find out how much the services of nursing assistants are used in acute nursing care and whether they are interested in working in this area. The instrument for gaining these targets was the quantitative method in a form of a questionnaire intended for the pupils in the 4th grade at the Medical Secondary Schools in České Budějovice and Český Krumlov, specialization Nursing Assistant, and then the qualitative method in a form of a semi-standardized interview conducted with the middle and low management, in particular ward sisters and nursing officers on wards with acute beds. The questionnaire responses were plotted in graphs using descriptive statistics, interviews were summarized in a paper and synoptic charts. The results ascertained were described and evaluated in terms of disproving or proving the hypothesis and in terms of findings based on the questions raised in the final discussion.

Comparison of the Motivation and Professional Background the Various Forms of Animal Assisted Activities.

The aim of my thesis " Comparison of the Motivation and Professional Background of the Various Forms of Animal Assisted Activities" is to determine through literature, opinions of therapists and observation to determine what is the motivation, professional background and time demand to all forms of therapy. The first part focuses on the cross-section of views of the theoretical literature focusing on animal assisted activities. Here I describe the differences between different forms of animal assisted activities and I also deal with ethology of dogs. In the second part I find out through interviews and observations, I find actual evidence from animal assisted activities and I specifically discuss the use of animal assisted activities on the first grade of primary school. At the end of the thesis I summarize insights from observations, processed interviews and also from my own experience and I evaluate usage of animal assisted activities on the first grade of primary school.

Professional training and other educational opportunities of teaching assistants on the process of integration and inclusion of handicapped children

The introduction to my thesis outlines the view on the integration and inclusion as increasingly used terms, when their meaningfulness is understood by handicapped children´s parents every day. Thanks to the current trend of integrating disabled and disadvantaged children into mainstream education, the work of teacher´s assistents is necessary. It points out their function in inclusion, which is extremely important and hence their knowlege and skills in the handicapped children´s issue should be more than above average. The assistant tutors the child, explains specific tasks, as well as helps the child with his or her personal problems. The thesis also mentions an opinion on the correctness, so that a pedagogcially oriented university graduate had also the competence to bet he teacher´s assistant, because his or her knowledge should be sufficient. This however does not mean that they should not educate themselves any more, and get new information, that is on the contrary. The work with children is a very responsible activity and whoever does it should have the ambitions to improve their knowledge. This bachelor´s thesis is focused on the overview of options of the teachers´ assistants´professional preparation in the current toplist of the South Bohemia region and it maps the available educational courses and offers of school facilities. The thesis is also enhanced by a general overview offer of educational institutions in individual regions of the Czech Republic and school facilities providing additional education or lifelong study. In the theoretial part the thesis focuses on the clarification and explanation of the term ´teacher´s assistant´, its legal interpretation according to the laws of Ministry of Education, Youth and Sports of the Czech Republic. A certain part is devoted to the historical development of the teacher´s assistant´s proffession, and it specifies their work competence. It specifies the current legislative definition of the required education needed for the proffession of assistants, distinguishing the direct educational activity within the class and indirect educational activity that consists of auxiliary educational work. It also explains the difference between the terms ´integration´ - ´inclusion´ and the terms connected to inclusion, e.g. inclusive environment, inclusive school, or inclusive education. It also defines and introduces the reader to individual types of handicaps that might occur in handicapped children´s integration. It specifies the particular conditions of inclusion for given handicap and suitable attitudes towards a functioning integration. In the practical part the thesis specifically maps out the available educational institutions from the region of South Bohemia, their current offers, focus and requirements. Further on it deals with educational institutions offering the study for teachers´ assistants in all the regions of the Czech Republic, including the national organization of the National institute for further edcuation, which cooperates with the Ministry of Education , Youth and Sports of the Czech Republic and with the Eurofound for regional development. All the institutions were addressed during the period of 1 July 2014 and 2 August 2014, addressed through questionnaires sent by emails to specify their activity and the subject taught. A semi-structured interview took place with chosen institutions to clarify information. The research part is terminated with individual interviews with chosen teachers´ assistants from the region of South Bohemia, working at primary and secondary schools, that took the form of semi-structured interviews. The assistants commented on their education, possible obstacles or the perspective of the teacher´s assistant´s proffession.

Physiotherapy in patients after injuries of the spine and spinal cord

In connection with our lifestyle every year in our country but also abroad there is a growing number of people with spinal cord injuries. The spinal cord injury usually occurs while a spinal injury, where the spinal canal is compressed by luxated vertebra or bone fragments. Affected are more often young people, especially men. The most common causes are falls from height, accidents and sports injuries. A person with this condition not only has limitations in terms of loss of movement or sensitivity but is often threatened by various health complications. Therefore a complex care and intensive rehabilitation is very important, which is in the Czech Republic secured by the spinal program lasting 6-9 months. Rehabilitation is needed not only in the acute period after injury, but has a unique place in the chronic stage of spinal cord injury as well. The aim of this thesis was to explore issues of trauma and spinal cord injury from the perspective of physiotherapy. Based on the defined targets following research questions have been determined. What kinds of physiotherapy procedures may be used onpatients in the subacute stage and what procedures in the chronic stage of spinal cord lesion. A qualitative research in the form of case reports has been executed. Data was obtained from interviews, observation, document analysis and kinesiological analysis. Standard tests related to the spinal lesion were performed, only the most relevant tests and the results that were found during therapy in case reports were entered. The sample included four patients. Two patients in the subacute and two patients in the chronic stage of spinal cord injury lesion etiology. The research was conducted at the Spinal Unit Department of Traumatic Surgery, University Hospital Brno and at the Rehabilitation and restoration center Borovany. Physiotherapy in subacute phase of spinal cord injury is directed at the prevention of pressure sores, contractures, pain, maintaining optimal mobility of joints, focusing on respiratory rehabilitation, maintenance and strengthening of preserved motor potential, activation of the motor transient spinal cord lesion with efforts to activate the motor skills in the most distally way, improving the overall physical condition. The aim is to use and support functional ability of the patient to the fullest extent and quality with the subsequent effort to develop alternative mechanisms.Of the special methods the ones on the neurophysiological basis are the most commonly used treatment methods. An important part is the gradual verticalization of the patient, training equilibrium reactions such as training seat, support for upper limb, keeping in certain positions and functional training, thus the mobility on the bed, self-care, transfers, training wheel chair control, practicing functional replacement grip. Patients return to life with a big handicap, are often dependent on the assistance of others, learn to use a wheelchair and other aids. They have years of life ahead in which they have to cope with the conditions subsequent their injury. In the chronic period, it is important to maintain physical fitness, preventpossible complications, especially pressure sores and joint contractures. Physiotherapist addresses the consequences of overloading the exposed muscles, monitors the vertebral problems, joint pain or muscle imbalance. During this period, patients can attend ambulant physiotherapy or use maintenance stays in rehabilitation centers. The purpose of the study was to mention the basic and the most important areas in the care of spinal patients and to highlight the importance of physiotherapy comprehensive care not only within the spinal program, but also from the perspective of the long-term care of these patients as the rehabilitation should accompany the patient's whole life.

View of sexually transmitted diseases in case of assisting professions

Name of the dissertation is:``View of sexually transmitted diseases in case of assisting professions``. I decided to deal with sexually transmitted diseases because the number of these diseases is relatively high and numbers of newly ill people keep increasing. This opinion is confirmed by the fact that, for example, number of HIV positive individuals in the Czech Republic slightly exceeded 100 in the year 1990 whereas the number HIV positive individuals was 1,344 in the year 2009, i.e. 19 years later. Considering also the fact that this disease is an incurable one and treatment of this disease is demanding on the ill people from thein mental as well as physical aspekt and considering also the total effect for society, it is very important indeed to pay attention on this issue. Theoretical part divides sexually transmitted diseases according the often used division in classic generál diseases. They include inguinal granuloma, gonorrhoea, soft chancre, syphilis and venereal lymphogranuloma. Another diseases which include HIV/AIDS, hepatitis, genital herpes, infections caused by the virus Human papillom, scabies or pubic louses were dealt with only marginally. The last chapter is focused on assisting professions. The goal of the work was to look at the issue from the point of view of assisting professions staff. To find out whether they are interested in the issue, what is their awareness, etc. This goal has been met. Conversations, for example, revealed that employees who are in everyday contact with people who live risky way of living (drug addicts, homeless people, etc.) are better aware of issue concering sexually transmitted diseases than respondents who work with children or seniors. None of the respondents underwent any additional training in this issue. The selected methodology was represented by quality research and the selected method was represented by partially controlled interview with six selected representatives from assisting professions. The overall knowledge will be used not only to find out but also the to complete or correct information on the issue for employees in assisting professions. Information on the issue can be further disseminated among clients via these professionals within primary prevention.

Oxygen therapy in newborns of nursing perspective

The thesis concerns the treatment of newborns with oxygen from the nursing perspective. Work has part of theoretical and of research. In the theoretical part I focus on the latest European Resuscitation Council best practices in the administration of oxygen as well as longer-term oxygen according to best practices of the Czech neonatology in inpatient Department. First, particular attention devoted to the reconstruction of the fetoplacental circulation of the newborn with subsequent changes in his lung and blood circulation after birth, because it is an integral part of the knowledge in the treatment of oxygen. The next section is devoted to the principles of the first treatment of the newborn, in particular with regard to the assessment of the postnatal adaptation and the decision to open the oxygen therapy by nurse or midwife assistant. The following are mentioned in the work at the same time valid recommendations for the resuscitation of the newborn, again with an emphasis on the question of the use of oxygen. In the next chapter is summarized the most common neonatal pathology, which requires oxygenotherapy. The following chapter covers ways to feed oxygen to newborns as early in the process of adaptation, so when a stay in the neonatal intensive care unit. There are described the proper techniques of monitoring and summarises the complications of treatment with oxygen in its inadequate application. Research section includes the identification of the knowledge of the nursing staff in the treatment of oxygen for newborns and then system in the education of nursing staff in matters of oxygen therapy. For research were established two objectives. The first objective is to map the knowledge of nursing staff in the principles of application of oxygen for newborns in the neonatal intensive care and resuscitation and in the birthing room. And the aim of the second was to determine what form is tutored by the nursing staff in the area of oxygen for newborns. On the basis of these objectives have been laid down 3 research questions. The first question is: What are the main differences between knowledge and best practice version of the nursing staff in the application of oxygen for a newborn? The second question is: Is the education of nursing staff in the resuscitation of the emphasis is on the practical implementation and on the systematic recurrence? And the third question is: How does the education staff in the principles of oxygen therapy beyond resuscitation? For research inquiries, he was elected qualitative research. For data collection have been used individual interviews with 5 children's nurses, who work in a specialized neonatologickém work in the field of neonatal intensive care units and in the field of bithings rooms of the Hospital Department in České Budějovice a.s. The Interviews were subsequently rewritten into a Bachelor's thesis and at the conclusion of the results of a detailed table was created, where results are presented. Of the investigation can be concluded that nursing staff have sufficient knowledges in the field of oxygen therapy in newborns and nursing and best practices in the application of oxygen. As regards education, which relates to the resuscitation of newborns is done regularly, once a year in the Department The results of the work could be used to identify deficiencies in training in the field of application of oxygen in resuscitation with the aim of directing the education of personnel in this area.
