This bacelor work deals with the subjective perception of significance of substitution treatment by it's clients. It's aim is to find out, how the clients of this treatment perceive it's significance primarily in their lives. Out of the data gained from the research it is obvious that despite some negatives of the substitution treatment it's clients considers it to be very positive especially because it allows them to get involve in the society and to live the "normal life". Suprisingly, in comparison with the orientation of intact population the clients almost completely ignore the influence of substitution on their state of health and the risk of transer of infection diseases. Bio-psycho-social-spiritual model in the buprenorphine substitutional treatment

This thesis is focused on the application of holistic, bio-psycho-social-spiritual model of dependence to concrete therapeutic method, namely to the substitution treatment of persons dependent on opiate addictive substances with synthetic opioid buprenorphine. The aim is to describe the essence of BPSS model of addiction, with the specifics of opiate addiction, and the treatment system based on it. Also, to show substitution therapy as one of the most effective methods that improves quality of a dependent person's life. Further, to confirm the need for a holistic perception of substitution treatment when ersatz drug use itself is not a real chance for dependent persons, unless such treatment is accompanied by psychosocial and spiritual therapeutic approach. Another objective is to map the risk that, in practice, the substitution therapy initiation in a holistic range brings in the context of contemporary reality. The work will consider approach bases to opiate addicts, especially the optics of lifelong abstinence versus controlled use, leading to better quality of dependent person's life. Case study of a client included in methadone substitution programme

of bachelor thesis Keywords: methadone- susbstitution therapy- client- substitution programme-causes The bachelor thesis deals with the effect of client participation in the complete substitution treatment programme with methadone, which aims to demonstrate a positive effect not only on the client itself, but also on its vicinity. The theoretical part deals with methadone, its effects and the progressive placing into the treatment of addicted clients, firstly abroad and then in the Czech Republic. It deals with forms of treatment working in the Czech Republic with a detailed description of a complex substitution therapy. At the end of the theoretical part are discussed the causes of origin of an addiction on the addictive substances. The practical part is a case study of two clients - married couple, what describes in detail their life before the treatment, during the treatment and at her one year after the treatment, when she has been included in the substitution programme for ten years and is ending with a voluntary departure from the program with the zero dose of methadone - the client successfully abstains. Subjective Perception of the Life Change of Persons in the Methadone Maintenance Treatment Program in České Budějovice.

The thesis focuses on the subjectively perceived changes in the lives of persons in the methadone maintenance treatment program, on their experiencing of the changes and subjective judgment of the treatment. The topic is treated according to the Bio-Psycho-Socio-Spiritual Model of Addiction. The theoretical part deals with the quality of life and the subjectively perceived well-being and defines six dimensions of well-being on which the practical part of the thesis is based. It also elaborates on the Bio-Psycho-Socio-Spiritual Model of Addiction in the context of the methadone maintenance treatment. Moreover, it describes the factors in life change and the motivation to change. The last theoretical chapter focuses on the opioid substitution treatment as such and more specifically on the methadone maintenance treatment. The practical part presents the results of the research. The aim of the thesis is to analyse how the persons in the methadone maintenance treatment program in České Budějovice perceive life changes. A qualitative research was chosen for that purpose. The data were obtained through the technique of narrative interview. The research group consisted of four respondents who were on the methadone maintenance treatment program. The results show that the subjectively perceived changes in the lives of the persons in the methadone maintenance treatment program relate to five out of six dimensions of well-being. No changes were recorded in the dimension concerning the sense of life, in other words, even after a several month methadone maintenance treatment these persons perceive no goal of their lives. Important consequences for work with these persons can be drawn from that, especially for therapists in opioid substitution centres, who should develop spirituality aspects of persons.
The Effectiveness of opiate substance treatment

The thesis deals with a substance treatment of opiate addiction and determines drug abuse problems. The paper defines free and regulated market and describes drug prohibition compared with alcohol prohibition. The next part considers the theory of substitute and complement which is a basis for determination of a substance treatment. The goal of thesis is evaluating the effectiveness of substance treatment in the Czech Republic by force of cost-benefits analysis with current information. The empiric part is based upon the information about costs of substance treatment and about lost productivity. The effectiveness of substance treatment is confirmed, the amount gives after deductions for expenses an affirmative value. The next conclusion is positive appraisal of effectiveness of partial financing of Subutex, that is paid by consumer presently.
The Importance of Substitution Programmes in Drug Addiction Therapy System

The title of my thesis is: The Importance of Substitution Programmes in Drug Addiction Therapy System. Substitution therapy has its unique place especially in the treatment of opioid addictions. It helps individuals with stabilization and improvement of their social, somatic and mental state and it is important for the society, when public spending on the care and repression are reduced thanks to substitution treatment. In this bachelors thesis I deal with the issue of the effects of substitution therapy on the lives of people who use it for a long time. First, in the theoretical part, I define the issues concerning the addiction on non-alcohol drugs and then I deal with substitution therapy, its history, its current situation, its forms, types and components that substitution therapy includes. Attention in this section is paid to the effectiveness in terms of individual as well as social; therefore, I focus also on the disadvantages and difficulties that are associated with substitution therapy. As the objective of the thesis I chose to discover the importance of substitution programmes in drug addiction treatment. To achieve this objective, I defined two subobjectives, namely: Mapping the subjective evaluation of mental, somatic and social satisfaction of persons included in a substitution programme, and the comparison of mental, somatic and social situation prior and after inclusion of the client in a substitution programme. Given these objectives, I established three research questions that ask about the effects of substitution treatment on the social, somatic and mental aspects of the respondents. The research was conducted in a qualitative form, by the method of interviewing and the technique of semi-structured interviews. The research results have confirmed the improvements or stabilization of social, somatic and mental aspects of persons included in long-term substitution programmes. As the core finding I consider the fact that there is some abuse of substitution therapy, in which case its effectiveness in these three levels is less visible than in persons observing substitution therapy in the prescribed manner. The results of my thesis can serve for the needs for further research, to inform the general public and professionals, and they also have a practical use when working with clients in substitution treatment.
Platelets counting and substitution therapy

Thrombocytopenia can be simply defined as a blood platelet count of below 150 giga/l. With the routine measurement of platelet number by automated cell counters it is a relatively common finding. Before initiating further investigations it is important to confirm that a low platelet count is genuine by careful inspection. Either a small clot in the sample or platelet clumping can cause artefactual thrombocytopenia and lead to unnecessary intervention.

Contribution of this work into a health - welfare sphere is to line up some basic informations about individuals problems, while they are trying to restore to health from drug dependence, but after that they have to also aling with orientation in society. They have to come through the contact with authorities, include themselves into a collective and adapt themselves for life "in a standart". Stated goal gave an orientation in the ways of their lewelling with stressing situations and also in finding out of specific tolerance dimensions against frustration. Drug abuse and addiction to drugs create costs that the society has to bear. First, they represent health resources used for treatment and further care and rehabilitation or re-socialization of exusers.
